2 207 CANCER COMMITTEE Chairman s Report The Cancer Committee ensures the quality of clinical care services, patient safety and development of innovative cancer programs at Cancer Treatment Centers of America (CTCA) in Tulsa. Each year, the committee sets goals and perform studies to improve all cancer-related activities at our hospital. The membership of the cancer committee is multidisciplinary, representing physicians from diagnostic and treatment specialties, as well as non-physicians from administrative and supportive services. This year, Tammi Holden became the Cancer Program Administrator, Dr. Asha Karippot replaced Dr. Andrew Buck as the medical oncology representative and Dr. Michael Payne resumed his position as radiation oncology representative upon completion of his fellowship. Dr. David Wakefield was appointed the psychosocial services coordinator. Also, we are excited to announce the hiring of our new gynecological oncologist, Dr. David McIntosh. All physicians represented on the Cancer Committee are board certified. Jay Foley President & CEO, CTCA Tulsa The Cancer Committee uses evidence-based guidelines and a multidisciplinary composition in conducting cancer conferences. In 207, the committee held two cancer conferences each week and discussed 42 cases, with over 95 percent of cases being discussed prospectively and every case being considered for a clinical trial, with Targeted Agent and Profiling Utilization Registry (TAPUR) being the trial with the largest accrual. CTCA Tulsa holds accreditations from multiple agencies, including the Joint Commission and the State of Oklahoma. The Commission on Cancer granted our center full accreditation in February 208, and our radiation department is accredited by the American College of Radiology. We also received commendation status from the American College of Surgeons National Cancer Database, as well as QOPI recertification for three additional years. Also, our lung program passed its survey and is certified for two years. We have ten (0) departments to address psychosocial needs at any point along the care continuum. For 207, the top interference issues reported by patients, as accessed using the Symptom Inventory Tool (SIT) were work, activity and sex. Traci Owen, RN, began to address sexual wellness in the surgical clinic, especially with ostomy patients. Our cancer rehabilitation services help patients cope with activities of daily living affected by the cancer experience and enables them to resume normal activities. Rehabilitation services include, but are not limited to, lymphedema, pain management, physical impairments and disabilities, lifestyle and weight management programs, physical and exercise therapy, reflexology, massage therapy, chiropractic care and occupational therapy. At CTCA Tulsa, palliative care is provided by a multidisciplinary team that is comprised of a certified hospice and palliative physician, two registered nurses who are hospice and palliative certified, a social worker, dietician, chaplain and a representative from our mindbody medicine team. Palliative care is not simply hospice care, it refers to patient-andfamily-centered care that optimizes quality of life. The availability of palliative care services is an essential component of cancer care, beginning at the time of diagnosis and being continuously available throughout treatment, surveillance and, when applicable, during bereavement. This year, the Cancer Committee s clinical goal was to implement a software tool to ensure patients are being treated according to evidence-based guidelines and National Comprehensive Cancer Network (NCCN) guidelines in the care of patients with breast, lung and colorectal cancer. The committee began the year with a focus on newly diagnosed breast cancer patients and will continue to expand its use as data becomes available. 2 CTCA Tulsa 208 CANCER REGISTRY ANNUAL REPORT

3 The Committee s programmatic goal was to improve the usage of the Transitional Care Management Program, which is a collaboration between our inpatient and outpatient care teams. It impacts patient safety and readmission rates by helping patients understand the purpose of medications and symptoms and problems to look for after discharge. As a result, from the first quarter to the fourth quarter of 207, our readmission rates decreased by percent. Process Improvement & Daily Excellence (PRIDE), the daily management system employed by CTCA, empowers Stakeholders to communicate problems and areas of concern effectively and provide collaborative solutions on a daily basis. In early 207, the Quality of Life (QOL) team collaborated with the scheduling team to proactively improve the scheduling process and patient satisfaction. In 207, the Cancer Committee performed a quality study on nutritional care for primary lung cancer patients. Lung cancer patients tend to be more hypermetabolic compared to other types of cancer patients, which can lead to loss of muscle mass and total body weight. Our protocol at CTCA Tulsa is to provide nutritional evaluations to all primary lung cancer patients. Unless contraindicated, the indirect calorimetry (IDC) is used as the gold standard for energy requirements and evaluation. With the knowledge gained from the test, we may be able to offer improved nutritional support for our lung cancer patients. In 207, CTCA Tulsa hosted or participated in 09 community events with 53 community organizations that featured 33 medical providers and 23 employers that reached 86,08 individuals and facilitated 94 expert speaking opportunities. During these events, we provided educational speakers, materials and resources to help organizations increase cancer awareness, promote early detection and encourage cancer-preventing lifestyles, designed to help reduce cancer incidence in our region. Of the participants polled, 96 percent committed to making a healthy lifestyle change because of the educational lecture on prevention. This year, we expanded our skin cancer screenings to additional public schools, and we continued to provide free mammograms and ultrasounds through our collaboration with Oklahoma Project Woman. Our lung cancer screening program has been so effective that all five CTCA hospitals have adopted it. We also added a new cancer screening awareness program, Prostate Pep Talk, to encourage men to take initiative for their health and get screened for prostate cancer. I am especially proud of our Cancer Committee at CTCA Tulsa and all that the members have accomplished, including our CTCA Tulsa registrars, both of which hold a current CTR certificate and attended state and national meetings this year. This group is instrumental in assisting our hospital and Stakeholders in providing the Mother Standard of care to our patients. 4 CANCER REGISTRY Report The Cancer Registry at Tulsa, Oklahoma has entered more than 2,700 cases into the database since its inception in 99. The CTCA Tulsa Cancer Registry maintains a data system designed for the collection, management and analysis of data on individuals diagnosed with cancer. The Cancer Registry has entered more than 2,700 cases into the database since its inception in 99. During 207,,84 new cases were accessioned into the Cancer Registry database at CTCA Tulsa. Of these cases, 558 were analytic (either diagnosed at CTCA Tulsa or received all or part of their first course of treatment at CTCA Tulsa), 304 were non-analytic (diagnosed elsewhere and received their first course of treatment elsewhere), and 322 were non-reportable cases. Excluding the non-reportable cases, we treated 574 male patients and 60 female patients in 207. We accessioned patients from 36 states, three patients from the Virgin Islands, one from the Bahamas and one from Guam. The largest portion of the patient population resided in Oklahoma. The American College of Surgeons Commission on Cancer mandates at least 90 percent of the analytic patients from the last five years and 80 percent from the reference year (99) are followed yearly. The CTCA Tulsa Cancer Registry meets this standard with 86 percent current follow-up on patients seen since 99 and 90 percent on patients seen in the last five years. The registry staff maintains the daily functions of the registry and ensures data accuracy with continuous quality improvement review by the Cancer Committee physicians. The registry provides the Cancer Committee with information from which clinical application and analysis of patient outcomes can be determined. 5 Ill-Defined Site 9 Breast 33 Lung CANCER CONFERENCE Summary During 207, a multidisciplinary team, including medical oncology, radiation oncology, radiology, pathology and surgery, attended 44 disease-specific cancer conferences focusing on breast, lung and head/neck cases, as well as general cancer conferences. The nursing and ancillary staff also attended these conferences to ensure comprehensive representation. Together, they reviewed cases in detail and discussed staging, prognostic indicators, national guidelines and plans for appropriate diagnostic studies, therapies and clinical trials. These prospective, patient-oriented and multidisciplinary physician care planning meetings provide a forum for the medical and hospital staff to consult with each other to offer the highest quality of care for our patients. Of the 42 cases presented, 98 percent were prospective patient presentations. 10 Cancer Prevention and Screening Programs AT CTCA TULSA 27 prevention events hosted in Lung Watch events Prevention Programs In 207, CTCA Tulsa hosted 27 community events that featured cancer prevention educational speakers and provided information; five Lung Watch events were also held. All 487 participants of Lung Watch events received detailed reports assessing their lung cancer risk to share with their physicians for follow up care if appropriate. Ninety-six percent of participants polled committed to making a healthy lifestyle change, such as incorporating a healthy diet, exercising daily or wearing sunscreen. Eighty-three percent of smokers were willing to quit and one hundred percent were provided smoking cessation information. 2 skin cancer screening events 6 individuals screened for skin cancer Prevent Aggressively Skin Cancer Screening Events CTCA Tulsa conducted two skin cancer screenings; one at Yukon Public Schools and the other at Tulsa Public Schools. During these two events, a total of 6 individuals participated. Twenty-two percent of the participants needed immediate follow up with dermatologist and were called by a CTCA clinician to make sure they had appropriate resources. 88 prostate cancer screenings found to have abnormal results Prostate Pep Talk Screening Campaign The CTCA Prostate Pep Talk screening awareness event encourages men to take initiative for their health and get screened, identify issues at an earlier stage and decrease the death rate in prostate cancer, which is one of Oklahoma s top three cancers. 11 Oklahoma Project Woman Breast Cancer Screenings CTCA Tulsa collaborated with Oklahoma Project Woman to help uninsured women receive free mammography and ultrasound screenings. 12 Accreditations AND CERTIFICATIONS CTCA Tulsa is accredited and recognized by several renowned professional health care organizations that assess and monitor the quality of patient care. The Joint Commission The Joint Commission s Gold Seal of Approval for Hospital Accreditation reflects a commitment to providing safe and effective patient care and a willingness to voluntarily undergo rigorous, unannounced onsite surveys. Accreditation requires compliance with standards related to areas such as patient rights, environment of care, infection prevention, leadership and medication management. Quality Oncology Practice Initiative (QOPI) The QOPI Certification Program, an affiliate of the American Society of Clinical Oncology (ASCO), recognizes outpatient practices that meet the benchmarks for breast, colorectal, non-small cell lung, non-hodgkin lymphoma, gynecologic and prostate cancers. This seal designates those practices that scored high on key QOPI quality measures and met rigorous chemotherapy safety standards established by ASCO and the Oncology Nursing Society. Commission on Cancer (COC) The COC recognizes cancer care programs for their commitment to providing comprehensive, high-quality and multidisciplinary patient-centered care. COC Program Standards require facilities to create meaningful processes for implementation of patient-centered care. National Accreditation Program For Breast Cancers (NAPBC) Accreditation by the NAPBC is granted only to those centers that are voluntarily committed to providing the highest standards of care to patients with diseases of the breast.
