2 CANCER COMMITTEE REPORT This annual report highlights the services provided through the Cancer Care Program at Clark Memorial Health, which offers patients access to a multi-disciplinary team and a range of care. A strong cancer program can t exist without involvement from the medical community. Clark Memorial Health has a dedicated group of physicians who participate in a bi-monthly Tumor Board to prospectively discuss and plan the care of patients diagnosed with cancer. The Tumor Board includes surgeons and oncologists directly involved in the patient s treatment, as well as radiologists, pathologists, hospice physicians, oncology nurses, social workers, dieticians and the chaplain. This wide representation ensures that our Tumor Board truly covers every aspect of patient care. To maintain the ongoing quality of the care we provide, our Cancer Care Program is measured against benchmarks developed by the American College of Surgeons Commission on Cancer. The quality and performance goals established by the College ensure that consistent, quality care is provided to all Cancer Care patients. Clark Memorial Hospital continues to increase awareness of various prevalent cancers in our community at events providing screenings and relevant information. A farmer s market, car show and yoga demonstrations rounded out the activities for the day Child Safety Day held August 25, in attendance 30 hearing screenings 55 vision screenings 50 scoliosis screenings 40 fingerprint and photo IDs distributed Informational booths provided education on sun safety and skin cancer prevention, healthy eating and exercise, gun safety and resources on community engagement activities for children and families. A photo booth, balloon art, face painting, mascots, live animals, car seat checks and a Home Depot building station rounded out the activities for the event.

5 CANCER IN OUR COMMUNITY In collaboration with the Indiana Cancer Consortium, Clark Memorial Health hosted the Colon Cancer Summit on May 15, This was an opportunity for local healthcare professionals, providers, caregivers and survivors to gather and share insight on all aspects of colon cancer; from screening, diagnosis, treatment, recovery and support. The summit included speakers from local agencies and practices to report on current cancer data for District 9, screening guidelines and options, community involvement and support services from area resources. There were more than 60 people in attendance. The day also included a panel discussion as well as vendor booths with community partners such as Colon Cancer Prevention Project, ACS, Gilda s club and Norton Cancer Resource Center activities for specified cancers: Breast Booth at Community Health Fair with information and resources on mammogram screening guidelines, selfbreast exams and shower cards. Continued the direct mail program of reminder cards for women over 40 to schedule a screening mammogram with breast health information, resources and cancer facts. Hosted Breast Buddy Brunch at Hunter Station Diagnostic Imaging services to promote mammograms by encouraging women to get their screening together and support each other to ensure breast health. Clark Memorial offered brunch, a gift and chance at winning a basket of goodies to women who scheduled their mammogram with a friend/ relative during the event. Events held on March 16, June 22 and October 26 with 32 women scheduled with mammograms during these dates. One pair of ladies came to the event as they both had recently turned 40 and were having their first mammogram. Hosted booths at three local businesses in October to promote breast cancer awareness and screenings. Distributed educational material and shower cards for selfbreast exams as well as having the breast models on display demonstrating proper self-exam technique and recognition of lumps. SCREENINGS, OUTREACH AND EVENTS Lung Promoted Lung CT screenings at 30 plus events throughout the year. Hosted booths at four local businesses in January, April and November on lung cancer awareness and screening options. Partnered with the CCTP&C group on all four events. Partnered with the CCTP&C group to host the Great American Smoke Out event on November 15. As of November 30, 2018, 159 Lung CTs performed at CMH; 72 of these were self-referred. 11 scans were suspicious with recommended follow-up diagnostic imaging by end of 2018; 9 of those 11 had the follow-up within the recommended time. Of the 2 remaining, one follow-up was cancelled by the PCP and the other had no report as being done at CMH. Colon Three presentations given in March for colon cancer awareness, risk factors, screening guidelines and resources. Booth at Community Health Fair with information and resources on colon cancer, risk factors, screening guidelines and screening options. 33 FIT colon cancer screening kits distributed to attendees with contact information collected to follow-up regarding screening completion. Of the 33 kits distributed, only two attendees reported that they completed the screening as of October 1, Collaborated with the Indiana Cancer Consortium to host the 2018 Colon Cancer Summit on May 15 with more than 60 attendees. Continued direct mail of reminder cards for men and women over 50 to schedule colonoscopy or appointment with their PCP to discuss appropriate screening; cards include risk factor information and screening guidelines. Hosted booths at the Kicking Butts Southern Indiana Awareness Fair in March and at the Kicking Butts 5K run held in August providing education on colon cancer risk factors, screening options and distribution of 20 FIT colon cancer screening kits. Skin Hosted booths at the Community Health Fair and Child Safety Day on skin cancer prevention, risk factors, skin inspections and recognition and reporting of abnormal findings. Lobby Talk held in June on skin cancer awareness and prevention with 20 attendees.

6 TOBACCO PREVENTION and smoking cessation The CCTP&C Coalition provided education and resources on quitting smoking, tobacco and electronic cigarettes to all of Clark County in The CCTP&C Coalition collaborated with Clark Memorial outreach services on 25 events at schools, businesses and community festivals and fairs. We also partnered on many community based committees and groups advocating for stronger tobacco control and clean air ordinances including: Breathe Easy Jeffersonville Coalition, Minority Health Coalition, Head Start Advisory Committee, Judges Round Table, Clark County Cares, Indiana Cancer Consortium and attending many City Council meetings. The CCTP&C Coalition was very active with youth and school programs this year, including: JAG (Job s for America s Graduates) career development conference-the CCTP&C presented twice this year on smoking, the use of JUUL and E-Cigarettes. JAG is a national program that focus on juniors and seniors in high schools who have incurred adversity in life that could interfere with future success; the program offers guidance on how to stay focused on academics and put their best foot forward as they enter the workforce as adults. The CCTP&C Coalition worked with the administration of local high schools and middle schools to provide the Youth Tobacco Survey to gather current information and stats on teenage smoking and tobacco use, as well as youth perception of tobacco use in the community. The coalition worked with the Clark County Head Start program to administer the Family Tobacco Survey, as well as provide smoking cessation resources to the families of the Head Start students. The CCTP&C Coalition worked with the Jeffersonville Housing Authority in assisting with the implementation of smoke free housing policy and offered residents smoking cessation classes and resources for NRT. The Coalition coordinator and assistant coordinator attended a variety of training classes and programs to improve efforts of quitting tobacco in the community, these included: The Reality of Addiction; JUUL Electronic Cigarettes and their popularity with youth and young adults; Passage of Smoke Free Ordinances: Collaborative Success; Heated Cigarettes: How states can avoid getting burned; Vaping and E-Cigs among behavioral health population; FDA s Framework for Tobacco & Nicotine regulations. The CCTP&C coordinators will be applying for the grant from the Indiana Department of Health again in March of 2019 to secure funds and resources for the next two years with the hopes to continue their efforts to create a healthier community for Clark County. The Indiana Tobacco Quitline is a free phone-based counseling service that helps Indiana smokers quit. Services include: One-on-one coaching for tobacco users who have decided to quit. Resources for healthcare providers who want to improve patient outcomes. Best practices for employers who want to implement smoke-free policies. Support for family and friends who want to help loved ones stop smoking. Tools for tobacco control partners to complement their current programs. Services are available to you 7 days-a-week in more than 170 languages. A trained quit coach will work with you and provide solutions tailored to your needs. 7 THE NORTON CANCER INSTITUTE PAT HARRISON RESOURCE CENTER Number of Interactions by Diagnosis Interactions by Type of Service Massage Music Pt Ed Yoga Reiki Tai Chi Prosthetic Wigs Hope Scarves 2018 Interaction Types Calls Visits Hospital Visits The Norton Cancer Institute Pat Harrison Resource Center The Pat Harrison Resource Center at Clark Memorial Health has provided services to over 3800 members of our community this year. The staff at the resource center understand that cancer affects the whole family and all aspects of a cancer patient s life. The center offers a variety of support services to address the physical, spiritual and emotional well-being once a cancer diagnosis is made. The Look Good, Fell Better workshop was held quarterly and is a free program offered to women under current treatment for breast cancer to help them cope with the physical side effects of cancer. Gentle Yoga, Tai Chi and Reiki are services offered to patients to promote a holistic approach in coping with the stress and anxiety often associated with a cancer diagnosis. A new program offered in September was Aromatherapy for symptom management. It was well attended and received positive feedback and response. Other new services offered this year are a breast cancer support group as well as the center being a satellite location in Indiana for support groups through Gilda s Club.

8 CANCER PROGRAM REPORT The Cancer Program Practice Profile Reports (CP3R) is a web-based reporting tool that promotes continuous practice improvement at the local level and permits hospitals to compare their patient care to that of other providers. The aim is to empower clinicians and staff to work cooperatively and collaboratively to identify problems in practice and delivery, and to implement best practices to improve clinical management and quality of care across Commission on Cancer (CoC) accredited cancer programs. Below are the 2016 performance rates for Clark Memorial Health in comparison to the national performance rates for all Commission on Cancer accredited facilities. Cancer Program Practice Profile Reports (CP3R) 2016 CoC Required Standard BREAST MEASURES Clark Memorial Health EPR (Estimated Performance Rates) BCSRT 90% 100% Radiation is administered within 1 year (365 days) of diagnosis for women under the age of 70 receiving breast conservation surgery for breast cancer. HT 90% 100% Tamoxifen or third generation aromatase inhibitor is recommended or administered within 1 year (365 days) of diagnosis for women with AJCC T1C or state 1B-III hormone receptor positive breast cancer. MASTRT 90% 100% Radiation therapy is recommended or administered following any mastectomy within 1 year (365 days) of diagnosis of breast cancer for women with >= 4 positive regional lymph nodes. nbx 80% 88.1% Image or palpation-guided needle biopsy to the primary site is performed to establish diagnosis of breast cancer. COLON MEASURES 12RLN 85% 100% At least 12 regional lymph nodes are removed and pathologically examined for resected colon cancer. LUNG MEASURES LCT 85% 100% Systemic chemotherapy is administered within 4 months to day preoperatively or day of surgery to 6 months postoperatively or it is recommended for surgically resected cases with pathologic lymph node-positive (pn1) and (pn2) NSCLC. LNoSURG 85% 0% Surgery is not the first course of treatment for cn2, M0 lung cases.
