3 Octo)er 8 Executive summary MyPeBS is an innovative international research project, funded )y the European 7nion E7, gathering leading international )reast cancer researchers and e perts. 0he multidisciplinary project team of clinicians, radiologists, epidemiologists, geneticists, social scientists, and health economists ill ork in close colla)oration ith healthcare providers and advocacy groups to address the )ig uestions a)out the future of )reast screening. Organised )reast screening programmes have played animportant role in the improvement of )reast cancer survival rates )y allo ing treatment to )egin at a much earlier stage. Ho ever, these screening programmes have e perienced several disadvantages such as relatively fre uent false positive recalls, a small lifetime risk of radiation-induced cancer and overdiagnosis, hich has led to increasing criticism of the current one-size-fits-all approach to )reast screening via mammography. For current )reast screening programmes, aside from those omen at very high-risk of )reast cancer, age is the sole risk factor considered hen inviting omen to enter the programme. Advances in the understanding of )reast cancer )iology, risk assessment, and imaging have resulted in )etter tools hich could replace the current one-size-fits-all approach )y e tending to the hole population the personalised model used for omen at a high-risk. A more personalised screening strategy, )ased on an individual oman s risk, ould give tailored recommendations on hen to start, hen to stop, and ho often to screen omen ithin the )reast screening programme. MyPeBS My Personal Breast Screening is a uni ue international project that ill investigate this issue and consider hether a more personalised form of )reast screening could )e a )etter option for omen. 03

4 Octo)er 8 Context Breast cancer is a major concern for all of society. According to the latest availa)le data, )reast cancer is the most common cancer in omen orld ide. In the European 7nion alone, it is estimated that there are a)out 5,000 ne )reast cancer cases and,000 deaths due to )reast cancer every year. Early diagnosis is crucial to getting the )est care for )reast cancer and reducing the num)er of )reast cancer deaths. Most countries in Europe try to spot )reast cancer early through organised screening programmes, hich invite omen to get an X-ray taken of their )reast. 0his method, kno n as mammography, is idely used in )reast screening as it is so far the only tool that has )een sho n to help prevent deaths from )reast cancer. Ho ever, despite the proven )enefits of mammography, this method also carries ith it some risks, hich need to )e properly communicated to omen. Goals MyPeBS ill compare the current standard of one size fits all )reast screening ith a more tailored strategy, hich screens omen at higher risk of )reast cancer more often, and omen at a lo er risk of )reast cancer less often. 0he goal of this study is to ans er a simple, yet fundamental, uestion: is it )etter to personalise the method and fre uency of )reast screening )ased on a oman s individual risk? As the personal e perience of omen is central to the study, MyPeBS ill investigate hether the personalised approach is more accepta)le, paying close attention to the potential e tra orry for omen that may )e caused )y kno ing their individual )reast cancer risk. 0he project ill also evaluate if the resources used ith a personalised screening strategy justify the results o)tained. Finally, after the results of the study are kno n, MyPeBS ill propose general recommendations for more effective )reast cancer screening in Europe. 04

6 Octo)er 8 Consortium 0he MyPeBS project is coordinated )y 7NICANCER a large non-profit French national federation of hospitals dedicated to oncology and involves partners from different countries, including many leading doctors, scientists and prestigious institutions engaged in )reast cancer research. At the heart of the project is the randomised clinical trial, hich ill compare the t o )reast screening strategies. 0his trial ill )e run in 5 countries: France, Italy, 7nited Kingdom, Belgium, and Israel. 0he study is supervised and controlled )y an e ecutive committee, comprised of project partners, and an independent ethics, data monitoring and study conduct committee.
