3 A Letter to the Residents of Pennsylvania In this report, we will introduce you to the Pennsylvania Cancer Control Consortium (PAC3) and report on efforts underway to create a new strategic plan for cancer control for the Commonwealth of Pennsylvania. The PAC3 initiative is a new and innovative approach to advise and support the planning and implementation of a Pennsylvania cancer plan. The PAC3 is comprised of over 100 individuals and organizations with a particular commitment, capacity and/or area of expertise important to the fight against cancer. A broad spectrum of public, private and non-profit sections are represented, including heads of government, non-profit agencies, the insurance industry, communication specialists, business, cancer survivors, researchers and academia. As we continue the development of a statewide cancer plan, we will think creatively and honestly because while much progress has been made in fighting cancer, not all Pennsylvanians are reaping the benefit of that progress. Improved coordination within the state and among stakeholders is imperative to realize a fully coordinated approach to cancer control. National organizations and cancer leaders are strongly encouraging all states to take a bold new look at how we can overcome these devastating diseases. The continued involvement of all Pennsylvanians is key, and we look forward to working with you to see the PAC3 s mission fulfilled, its goals accomplished and objectives met. The focus and leadership provided by the Department of Health (Department) and our partners across the state will help Pennsylvania achieve major advances in cancer prevention and control. We look forward to unveiling this new plan in 2003 and for your continued support in this effort. 4 The Pennsylvania Cancer Control Consortium (PAC3) Through the vision and leadership of the Department of Health, the first-ever, comprehensive cancer control plan is being developed for the Commonwealth. A twenty-one member stakeholder Executive Team has been working since October 2001 to develop the strategy to launch the Pennsylvania Cancer Control Consortium (PAC3). The PAC3 initiative is a new and innovative approach to advise and support the planning and implementation of a cancer plan for Pennsylvania. The PAC3 is comprised of individuals and organizations with a particular commitment, capacity and/or area of expertise important to the fight against cancer in the public, private and non-profit arenas. PAC3 membership includes heads of government and non-profit agencies, the insurance industry, communications specialist, the business community, cancer survivors, researchers and academia. Currently, over one hundred partners are working on the development of the comprehensive cancer control plan through the PAC3 initiative. The Pennsylvania Cancer Plan will be a comprehensive statewide blueprint for meeting the growing challenge of cancer control among all sectors of Pennsylvania. Previous cancer control plans focused on the activities of the Department. The new comprehensive cancer control plan will have greater depth, scope and shared resources. Over the course of the next 12 months, PAC3 members will write and release a new Comprehensive Cancer Control Plan for Pennsylvania. This plan will: enhance coordination among cancer stakeholders in the Commonwealth in all aspects of cancer control -- from prevention and scientific research to information management and quality of life; establish consensus on priorities for the comprehensive cancer control plan; increase cooperation and collaboration among PAC3 partners; develop strong public-private partnerships; design and implement strategies and methods to reduce duplication of effort and increase shared resources among partners; and coordinate dissemination of cancer-related information and education. 10 3
6 Pennsylvania s Breast and Cervical Cancer Early Detection Program Breast cancer is the most common form of cancer diagnosed among women in Pennsylvania, and it is the second most frequent cause of cancer mortality among Pennsylvania women (surpassed only by lung cancer). The Department of Health, with funds from the U.S. Centers for Disease Control and Prevention, administers the HealthyWoman Program, a statewide breast and cervical cancer screening program. In Fiscal Year , over 6,000 women were served. The HealthyWoman Program provides mammograms, clinical breast exams, pelvic exams and Pap tests at no cost for women of low or moderate income, with limited or no healthcare coverage. Services are available for women ages 40 to 64, with a special focus on women over the age of 50. All cervical cancer deaths should be considered preventable. In 2000, there were 193 cervical cancer deaths among state residents. Screening, by Pap testing, can effectively reduce the cancer mortality rate. Cancer Control, Prevention and Research Advisory Board Act 224 of 1980 established the Pennsylvania Cancer Control, Prevention and Research Advisory Board. This 11-member board is appointed by the Governor, confirmed by the Senate, and advises the Secretary of Health on cancer issues. 7 Pennsylvania Cancer Registry The Pennsylvania Cancer Registry (Registry) is a comprehensive and responsive cancer data and information system that is used for planning, implementing and evaluating programs, policies and cancer research. The Pennsylvania Cancer Registry is located within the Department of Health, Bureau of Health Statistics and Research, Division of Statistical Registries. The Registry supports the Cancer Prevention and Control Section s efforts to reduce the burden of cancer by providing data to help define the disease burden, to determine the prevalence of behavior risks, to monitor the progress of prevention efforts and, ultimately, to make timely and effective public health decisions. The Pennsylvania Cancer Registry has received Gold Certification from the North American Association of Central Cancer Registries (NAACCR) for the three most recent data years (1997, 1998, and 1999). This certification signifies that data collected and maintained in the registry meets the highest national standards for completeness, quality and timeliness. The Registry participates in the National Program of Cancer Registries, a program administered by the Centers for Disease Control and Prevention (CDC) to support and enhance cancer registries. The Department also participates in the Centers for Disease Control and Prevention Behavioral Risk Factor Surveillance Systems (BRFSS). The BRFSS was designed by CDC to collect state-level data including cancer-screening statistics that states can use for comparison in planning and program development. The Cancer Prevention and Control Section also use these data to guide program development. Pennsylvania s Breast and Cervical Cancer Prevention and Treatment Program In October 2000, the Breast and Cervical Cancer Prevention and Treatment Act gave states the option to implement medical treatment benefits through Medical Assistance to eligible women diagnosed with pre-cancerous or cancerous breast or cervical conditions. The Pennsylvania Department of Public Welfare implemented the Breast and Cervical Cancer Prevention and Treatment (BCCPT) Program, which provides qualified women with full healthcare benefits for treatment services at no cost or a nominal co-payment. A coordinated effort between the Pennsylvania Department of Health and the Pennsylvania Department of Public Welfare transfers women screened through the HealthyWoman Program and diagnosed with breast or cervical cancer or pre-cancerous conditions of the breast or cervix into the BCCPT Program. Through the BCCPT Program, women receive full healthcare coverage for the treatment of breast and cervical cancer and any other healthcare needs, even if it is unrelated to her cancer or pre-cancerous condition. Within the first six months of the program, 174 women were enrolled into the BCCPT Program and received healthcare benefits for breast and/or cervical treatment. 8 5
8 Colorectal Cancer Local Municipalities Team Up to Fight Skin Cancer Colorectal cancer incidence is the third leading cancer site among males and the second most common site among females. The Pennsylvania Department of Health estimates 9,475 new cases and 3,390 deaths in 2002 (Pennsylvania Cancer Facts and Figures, 2002). The Cancer Prevention and Control Section provided funding to four of Pennsylvania s ten county/municipal health departments that identified colorectal cancer as a priority and expressed interest in working with the Department of Health to implement colorectal cancer educational programs in the communities they serve. With this funding, thousands of Pennsylvanians have been reached with important information on colorectal cancer screening. The Department is encouraging Pennsylvanians, particularly those 50 and over, to talk to their physician about colorectal cancer screening. The Department remains committed to increasing public awareness of colorectal cancer and increasing awareness of screening guidelines. Detected early, the five-year relative survival rate for colorectal cancer exceeds 90%. Only one-third of colorectal cancers are caught at the local stage. Early detection can literally prevent or cure this cancer. Informing and persuading the public and healthcare providers to screen for colorectal cancer can save thousands of lives. Melanoma incidence rates are rising faster than those of any other form of cancer in the U.S. (Manson, J, et al. 2000). In 1999, melanoma was the seventh most reported cancer in Pennsylvania males and eleventh among females. The good news is that skin cancer can be prevented. The challenge lies in changing the attitudes and behaviors that increase a person s risk of developing skin cancer. The Cancer Prevention and Control Section remains committed to preventing and reducing skin cancer in Pennsylvania. With funding provided by the Cancer Prevention and Control Section, communities across the Commonwealth have teamed up to conduct educational campaigns on skin cancer awareness. Several of the county/municipal health departments, such as Montgomery, Allentown and Philadelphia, have used these funds to reach over 8,000 individuals in their designated areas to reinforce skin cancer prevention in children and adults and promote sun-safety messages and behaviors through presentations and media efforts. 6 7
