Pneumonia is a common lung infection that children may get. This infection can be in one or both of the lungs. It is caused by a bacteria, virus, or fungus. What is acute chest syndrome? Acute chest syndrome (ACS) happens when red blood cells sickle inside blood vessels in the lungs. It is very painful. It is hard to find the exact cause of ACS. ACS can happen after an infection, pain crisis, or surgery. What are the signs and symptoms of pneumonia and ACS? Signs and symptoms of pneumonia and ACS: fever of 101 or greater once, or times in an hour coughing chest pain, back pain, or stomach pain shortness of breath fast breathing wheezing changes on chest x-ray lower oxygen levels Call your child s doctor right away if your child has any of these signs and symptoms or you have any concerns. 1 of 5

2 How is pneumonia and ACS in sickle cell disease treated? You need to call the Hematology Clinic right away. They will tell you to either come to the clinic or go to the emergency room. Your child may need: blood cultures and other blood labs drawn a chest x-ray antibiotics or fluids given through an intravenous catheter (IV) oxygen medicine for pain a blood transfusion How is pneumonia and ACS in sickle cell disease prevented? Your child should get all immunizations including: prevnar (at 2 months, 4, months, 6 months, and months) pneumovax (at 2 years, 5 years, and then every 5 years after that) influenza vaccine (every year) Your child will need to take penicillin 2 times a day until 5 years old. A medicine called hydroxyurea may be used to help decrease the number of ACS episodes. Your child may need to see a pulmonologist (lung doctor). Now that you ve read this: r Tell your nurse or doctor the signs of pneumonia and ACS. (Check when done.) r Tell your nurse or doctor what you would do if your child shows signs of pneumonia and ACS.
