The Microbiome The gut ecosystem is incredibly complex and consists of more than a thousand microbial species, the balance of which is maintained by even more complex feedback systems! There are more microbes in the human gut than there are human cells in the body and the genetic diversity of the microbiome dwarves that of the human genome. Let s look at some quotes
100 Trillion Guests You may know about the many millions of other cells in your body, ones that don t carry your DNA. But I m talking about the 3 to 4 pounds of gut bacteria in your colon or large intestine. We are talking 10 times the number of human cells approximately 100 trillion microbial cells, and more than 600 different species! Not only are most of us rather mindless in what we eat, we re ignorant of what these microbes do for us and what they need in order to do it!
100 Trillion Guests It's now quite clear that the type and quantity of micro-organisms in your gut interact with your body in ways that can either prevent or encourage the development of many diseases. The ideal ratio between the bacteria in your gut is 85 percent "good" and 15 percent "bad." Maintaining this optimal ratio is essential for good health, as probiotics (healthy bacteria) has over 30 beneficial pharmacological actions that we know of.
100 Trillion Guests Types of microbes comprising the microbiome include: Bacteria Yeast Fungi Mold Viruses Parasites
100 Trillion Guests The density of microbes increases as we move further down/along the digestive tract. The dominant strains also change. biotics_continued.htm
Good Bugs Parental Influence A woman's gut flora can also influence the health of her child, and if your child's gut flora is compromised from birth, he/she may be at an increased risk of vaccine damage. In fact, this imbalanced gut flora may be the primary factor that contributes to children having an adverse reaction to a vaccine.
Good Bugs Parental Influence "As far as science knows, the baby inside the mother's womb during nine months of gestation is sterile. The baby's gut is sterile. The baby acquires its gut flora at the time of birth, when the baby goes through the birth canal of the mother. So whatever lives in mom's birth canal, in mom's vagina, becomes the baby's gut flora.
Good Bugs Parental Influence So what lives in mom's vagina? It's a very richly populated area of a woman's body. The vaginal flora comes from the bowel. So if the mother has abnormal gut flora, she will have abnormal flora in her birth canal. Fathers are not exempt because fathers also have gut flora, and that gut flora populates their groin and they share their groin flora with the mother on a regular basis.
Good Bugs - Birth A normal vaginal delivery permits transfer of bacteria from the mother to the infant. During cesarean deliveries, this transfer is completely absent. These infants are colonized with flora from the hospital environment.
Good Bugs Birth They are colonized with more anaerobic bacteria, especially Bacteroides, than vaginally delivered infants. Clostridium perfringens is most frequently isolated after cesarean deliveries. When colonized, cesarean delivered infants less frequently harbor E. coli, and more often Klebsiella and Enterobacteria.
Good Bugs - Breastfeeding In breast-fed infants, Bifidobacteria account for >90% of the total intestinal bacteria, with some Enterobacter and Streptococci. In bottle-fed infants, Enterobacteria, Streptococci, Clostridia, Lactobacillus and Bacteroides tend to colonise because of a more alkaline milieu and the absence of the prebiotic modulatory factors present in breast milk.
Good Bugs - Roles Help develop the infant s GI tract and immune system Resist colonization by bad bugs Help normalize the gut immune response Manufacture vitamins and other nutritional factors Stimulate gut motility (bowel movements) Reduce allergies: good bugs train the immune system to distinguish between pathogens and non-harmful antigens, and to respond appropriately. Produce chemicals that feed the colon cells Reduce inflammation
Good Bugs - Disruption Stress and autonomics (sym/para) Food: Sugar, alcohol, processed foods, etc. Infection (aggressive bad bugs) Inflammation Antibiotics Toxins Nutrient deficiency Gut transit time Surgery
Dysbiosis The term dysbiosis is classically applied to harmful, non-infectious relationships between the human host and yeast, bacteria, protozoans, amoebae, or other parasites located specifically in the gastrointestinal tract, and dysbiosis is now an accepted term in the medical literature. f
Dysbiosis I prefer to use a broad definition of dysbiosis that implies a relationship of non-infectious hostmicroorganism interaction that adversely affects the human host and then to specify the subtype based on the location: gastrointestinal, oral, sinus, genitourinary, dermatologic, or environmental. osis.pdf
Gastrointestinal Dysbiosis Gastrointestinal dysbiosis classifications: 1. Insufficiency low good bug levels 2. Putrefaction inadequate protein digestion 3. Bacterial Overgrowth small intestine overgrowth 4. Parasitic 5. Fungal 6. Fermentation overconsumption of sugars 7. Immune Suppressive / Inflammatory endotoxin suppresses immune function 8. Hyper Sensitivity / Allergic exaggerated response to even a normal amount of yeast/bacteria
Gastrointestinal Dysbiosis Published research lists dysbiosis as a cause of arthritis, B 12 deficiency, chronic fatigue, cystic acne, eczema, food allergy, irritable bowel syndrome, psoriasis and steatorrhea.
Gastrointestinal Dysbiosis Intestinal dysbiosis has been associated with the following: Vitamin & amino acid deficiencies (any symptom) Malabsorption of carbohydrates (energy!) Inflammatory bowel disease Autoimmune disease Colon and breast cancer
Sinorespiratory Dysbiosis Patients with acute and chronic sinusitis commonly display a rich mixture of bacteria and fungi in their sinuses. In a landmark publication in Mayo Clinic Proceedings, the authors found that almost all patients with chronic sinus congestion had occult sinus infections, and they concluded, Fungal cultures of nasal secretions were positive in 202 (96%) of 210 consecutive chronic rhinosinusitis patients.
Dental Dysbiosis The human oral cavity is heavily populated by microbes, and these microbes and their products such as endotoxin can enter the bloodstream to induce a pro-inflammatory response. The systemic inflammatory response triggered by mild oral/dental infections is now believed to exacerbate conditions associated with inflammation, such as cardiovascular disease and diabetes.
Genitourinary Dysbiosis Yeast/fungal overgrowth and UTI s caused by E. coli and other bacteria are relatively common and can cause urinary, prostate vaginal issues. Microbial contamination of the genitourinary tract may lead to a systemic pro-inflammatory response.
Cutaneous Dysbiosis Microorganisms from dermal infections such as acne can incite systemic inflammation either by dermal absorption of bacterial and fungal antigens.
Environmental Dysbiosis Toxic mold syndrome describes systemic health problems from exposure to fungal bioaeresols, that are associated with mold-contaminated buildings. Mold exposure can lead to pro-inflammatory immune activation.
Dysbiosis - Summary As you can see, there are many different dysbiosis classifications. For sure, the gastrointestinal ones are the ones you ll encounter most frequently, but it s important that you acknowledge all of the classifications because once in while, the others will be important factors in your client/patient clinical picture.
Hompes Method Practitioner Training Level II Small Intestinal Bacterial Overgrowth - SIBO Health for the People Ltd not for reuse without expressed permission Hompes Method is a trading name of Health For The People Ltt Registered in England & Wales Company # VAT #
SIBO - Introduction Small intestinal bacterial overgrowth occurs when bad bugs colonise, or when good bugs migrate and get into the wrong place in large numbers.
SIBO - Introduction Bacteria that are normal in the colon may produce deleterious effects within the delicate environment of the small intestine.
SIBO - Distribution
Risk Factors for Developing SIBO
Clinical Features of SIBO Gas-bloat Flatulence Abdominal discomfort Diarrhea Steatorrhea Irritable bowel syndrome Heartburn / GERD Weight loss
Clinical Features of SIBO Features associated with micronutrient deficiencies (Vitamins B 12, A, D and E, iron, thiamine, nicotinamide)
Conditions Associated with SIBO Anatomical Enteroenteric fistula Small bowel diverticula Surgically-created blind loops Intestinal strictures Resection of the ileocecal valve
Conditions Associated with SIBO Functional Intestinal dysmotility syndromes Hypo- or achlorhydria Inflammatory conditions Autonomic neuropathy Reduction of gut-associated lymphoid tissue (GALT)
Conditions Associated with SIBO Miscellaneous Anything that reduces gut secretions Anything that reduces gut motility Immunodeficiency of any kind Cirrhosis Radiation enteritis Diabetes Pancreatitis End stage renal disease Short bowel syndrome
SIBO and IBS 78% IBS patients tested positive for SIBO 48% of successfully treated patients no longer met criteria for IBS Pimentel M, et al. Am J Gastroenterol 95: , 2000;
SIBO, CFS and FMS 78% and 77% of patients tested had SIBO Both disorders overlap with IBS Pimentel M, et al. J Musculoskelet Pain 9: , 2001; Aaron LA, et al. Arch Intern Med 160: , 2000
Dysbiosis Pathophysiology
Dysbiosis and SIBO Testing In forthcoming lessons we ll look at how to test for the dysbiosis states discussed in this module so far.
Microbiome - References The Custom Probiotics website is excellent for an easy to understand overview of the microbiome. See also the PDF I created with specific quotes from various publications and courses I ve attended. Dr. Vasquez s article is excellent by means of reviewing dysbiosis.
Thank You! Thanks for consuming this information. I hope it s been helpful. In the next lesson 5C we ll look at yeast and fungal issues, which often go hand in hand with bacterial and parasitic problems.
