We spoke with Svend about his experiences in the past and asked for his evaluation of the latest technologies. 4 Witnessing the Evolution of Digital Impression Solutions INTERVIEW WITH DENTIST SVEN W. CARLSEN Since 1982, Dentist Svend Carlsen has used various differing intra-oral scanners for taking impressions digitally. We spoke with Svend about developments in digital impression systems and how technology can help dentists in their daily work. - It all started with a digital chairside solution, where we had both the technology to digitally record the tooth and the milling equipment to manufacture the crown, says Svend Carlsen. - In the beginning, we were all very enthusiastic about the new technology, but we discovered that results lacked the required accuracy. We were forced to regard our ceramic inserts only as well-functioning mega-fillers. Since then I have been constantly searching for a digital system that could meet the accuracy challenge. A little over a year ago, I attended an event that included a demonstration of 3Shape s TRIOS system. This experience renewed my belief that digital imprint technologies could in fact fulfill our needs. Today we use the 3Shape TRIOS system for most of our impressions in the clinic for single-tooth crowns and bridges. What do you demand of a reliable digital system? - With most of the digital impression systems that I have used, there has been a need to make small corrections before the final crown could be placed in the patient s mouth. This should not be necessary. It is also now a great benefit to be able to view an enlarged image of the prepared tooth, with every detail clearly displayed. This enables you to identify and immediately correct areas where the tooth preparation is not sufficiently smooth or sharp. This level of control means that only correct information is sent to the laboratory. It also provides effective control of our own work. - Patients are rather impressed that we can take an impression so quickly and so easily, says Svend Carlsen. And he continues: I think that the high degree of control that dentists can demonstrate with the technology creates a greater sense of security for patients. Patients can follow their own treatment s details on the screen and thus better understand what it entails. How does the new system compare to the earlier devices you have tried? - Previously, you had to apply spray or powder on the teeth before you could scan. This was very annoying because it s hard to put an even layer of coating on the teeth - and an uneven layer would ruin scan precision. With previous systems, it was also necessary to maintain a completely steady hand while scanning. The slightest movement of the patient or of the operator s hand was enough to create a useless image. The new scanners allow a high degree of movement freedom, and this makes it much easier to get a good picture. Here at our clinic, assistants scan as often as dentists. The system is very intuitive. You started with a chairside milling system, but the TRIOS system you use today builds on cooperation with the lab. How do you see these two different approaches? - I know from my own experience that chairside milling systems are tempting for many dentists. But in reality, at least as the technologies stand today, chairside milling is not worth the investment because dentists need to spend too much time perfecting the esthetic details. In relation to my business, I have no doubt that it s better for me to outsource my milling needs. More importantly, skilled dental technicians will usually provide better restorations than dentists can. They are simply better artists and have more practice. Have digital impression solutions come to stay? - I think that the existence of digital impression solutions in all clinics will soon be taken for granted. Even in a little town such as ours, all four dental clinics are equipped with their own digital impression system. Actually, we find that our area s high-tech profile affects competition because it helps us attract patients from other cities. We also see a clear trend among laboratories. More and more labs are working with advanced digital systems that allow them to mill crowns directly from our digital impressions - without first manufacturing a model. This means that we can get our crowns back from the lab even faster, concludes Svend Carlsen. 5 Countering business threats with CAD/CAM Traditional dental labs are today pressured from many sides in-clinic milling, price competition and regulatory demands all pose serious threats to their future business efforts. Luckily, digital technologies are already providing direct answers to counter these upsetting realities. By going digital labs can push right back. Workflow integration Using in-clinic milling, dentists can set up their own manufacturing service to produce the final crowns directly in their clinic. This naturally means less business for the lab. Dentists who invest in in-clinic milling are clearly motivated by the opportunity to significantly reduce their lab service costs. However, the ROI calculations they are presented with will often be based on prices from traditional labs that still use manual technician methods. Labs equipped to use CAD/CAM can be more productive and thereby provide restoration services at lower prices than traditional labs while maintaining a high level of quality. Digital labs are closing the price gap between lab services and in-clinic milling, making in-clinic milling systems less attractive and changing the dentist s investment priorities. Dentists who obtain cost-efficient services by using digital labs will potentially be able to realize more business profit from treating more patients rather than spending time on in-house manufacturing. Furthermore, the restoration quality, material choices, and high esthetics that dedicated modern labs are able to provide will once again weigh heavily on their scales. Price Competition For many labs, lower price/quality laboratories pose the greatest threat to their existence. An increasing number of dentists and competing labs choose to send or outsource their restoration work to cheaper labs or regions. While this arrangement implies certain disadvantages in relation to turn-around time and quality, the savings are too significant to ignore. CAD/CAM can change these trends by enabling labs to compete with low-cost services. The high productivity per technician enabled through the digital process helps to bring local lab prices down to a competitive level. For dentists, the slight price differences that may remain will become insignificant when weighed against local service benefits such as faster delivery, consistent quality enabled by CAD/CAM, and closer supportive relationships with the lab. Increasing regulatory demands As requirements stiffen, strengthen, and escalate to higher levels, it is becoming increasingly critical that dental laboratories become compliant with standards and regulations such as FDA and ISO. Labs need to employ documented procedures and methods that will ensure the consistent high quality of their products. Acquiring a regulatory-compliant digital CAD/CAM system can be the most efficient way to fulfill a large part of the regulatory demands imposed on labs, because the workflows and output produced by these systems will already be FDA and ISO compliant. As one of their selection criteria, labs should consider the provider s regulatory status before investing in a new system. So Going Digital is the answer but how? Many lab owners are aware of their challenges and may even know that digital dentistry is the right move. However, many hesitate to go there because of the baffling number of systems and conflicting claims about which one is best to buy. Labs are absolutely right to be careful. Lab owners need to make the right choice among systems a choice that will add business value and expand their long-term possibilities rather than limit them. Some labs continue to focus on short-term perspectives and fail to understand that their passage from traditional methods over to CAD/CAM is not one to be taken lightly, and perhaps one of the most important business decisions they have ever made. 8 3Shape presents its new innovations in Chicago Chicago, February 23 26, Shape was once again a magnet attraction at this year s triple Chicago events CalLab, Midwinter and LMT Lab Day. 3Shape presented its coming Dental System 2013, the TRIOS digital impression-taking solution, and held a wide range of demonstrations, training sessions and lectures. The 2012 Chicago activities were kicked off the 23rd of February at CalLab s Annual Meeting where all the major dental industry players were present. Partners and customers alike came to meet and network with 3Shape s large gathering of representatives consisting of both sales and development staff from both the USA and Denmark offices. The Midwinter meeting event is organized each year by the Chicago Dental Society - an association for dentists in the Chicago area. Over dental professionals and exhibitors were at the scene of this year s midwinter show at the McCormick Place West. As expected, 3Shape s TRIOS digital impression taking solution attracted many dentists from all over the world. 3Shape hooked up a complete Clinic-to-Lab restoration workflow simulation with a TRIOS system on one end and a 3Shape lab with Dental System on the other. On the TRIOS side, our staff took digital impressions from complete jaw models and sent these to the lab side - directly into its InBox through 3Shape Communicate. 9 Tips & Tricks Combine Attachment Tool with Add/Remove Did you know that you can combine Dental System s advanced Attachment Tool with the Add/Remove Sculpt functionality and gain even more freedom during design of advanced restorations. The following tool combination is very useful e.g. when designing connection holes for sophisticated Stress Distribution Arms (Schubverteilungsarm). 1) Place your attachment, e.g. a hole, but do not apply it yet (i.e. do not press the play button) 2) Add or remove material around the attachment. During the Add/Remove operation you still have freedom to move, turn and adjust the attachment. 3) When finished using the Add/Remove tool, simply go back to Attachment and apply the attachment (i.e. press the green Play button) See the video below for a demonstration. Tips & Tricks video Increase productivity with an extra PC Did you know that you can easily add an additional PC to your 3Shape Dental System and thereby increase productivity? When adding an extra PC you end up with dedicated Scan and Design PCs connected to each other in a network setup. The operator works with the Scan PC to digitalize all incoming cases, and the final scans are immediately made available for design on the Design PC through a shared database. The Scan PC can focus on scanning while the operator - or another colleague - takes care of the CAD designs on the dedicated Design PC. This allows you to optimize the usage of your scanner and effectively increase daily scan throughput. Follow the simple steps below to setup a dedicated Scan and Design PCs 1. Setup network Existing PC (i.e. the PC that was delivered with the scanner) and New PC must be on the same network and be visible to each other. Please contact your local IT support for assistance if needed. For a list of minimum specifications for the new PC, please see the Online User manual, Appendix A (System Requirements). 2. Make Dental System server installation on existing PC Perform Dental System server installation on Existing PC (Leave your dongle in Existing PC).

10 3. Make Dental System Client installation on New PC Perform Dental System client installation on New PC. Select Existing PC as server. Either browse for the Existing PC or enter the corresponding IP address manually. Use the shortcuts on the desktop to start the Dental Manager Client. 4. Using the new setup A) You create the order and scan on the Existing PC - just as before. B) You design your cases on the New PC. All Dental System icons are placed on the desktop as usual, and orders are directly accessible in Dental Manager through the shared database. TIP: To increase memory and post-processing performance for larger cases, it is also possible to scan using the New PC. Open ScanIt Restoration on the new PC, go to Scan Settings, Scanners tab, and add Existing PC to list. Finally, Select Existing PC in the Current scanner drop down menu.
