2 Welcome!! We are looking forward to working with you: Service Outline / what do we do? The Specialist Diabetes Nursing Service provided by C&CDHB covers all three campus s: Wellington, Kenepuru and Kapiti There is an expectation that the student will visit all areas and will require own transportation Team made up of doctors, nurses, health care assistants, dieticians, podiatrists and support staff Your Preceptor You will be allocated one main preceptor. This preceptor will be responsible for helping you complete your objectives. However, you will also be preceptored by other members of our team during your placement with us. It is your responsibility to ensure the health care professional you are working with is aware of your objectives for the day/week. You must provide evaluations and/or other paperwork to your preceptor in a timely fashion (i.e. not on the due date!!). Your preceptor will not complete any evaluations if you give it to them on your last days of your placement. If you have any concerns or questions do not hesitate to contact Christine Expectations of Student Nurses / Code of Conduct Generally shift times: 8am 4.30pm. However, this may vary depending on clinics. Please ring your preceptor if you are going to be late or absent Dress code: student uniform with name badge, long hair must be tied back and cardigans must not be worn when working in clinics or on the wards Please bring all objectives and assessment sheets for this placement Cell phones must be turned off while on the wards or in a clinic session No carrying of water bottles in the clinical area Meal breaks: 30 minutes for lunch, time varies; breaks for morning and afternoon tea as able Access to food outlets or required to bring food: café on site at Wellington and Kenepuru, or bring your own lunch
3 Security: you are responsible for your own valuables, no locker provided Access to areas within the service /codes : some sites only accessible after 8 am, some areas need a swipe card If you are not achieving your objectives or have any issues or concerns please see your preceptor (before the last week). Please ensure all documentation you need to complete for placement is accomplished before the last days in our centre your preceptor will not complete any paper that is given to him or her if it is given in the last days of your placement. If Unwell or unable to attend placement ***Please ring on a daily basis if going to be absent*** Orientation You will receive orientation on site and will be given an information folder on the first day, which will include: What to do in the event of a cardiac arrest What to do in the event of a fire Any other unit/ward specific issues Swipe cards Who are our Clients? People with diabetes and endocrine conditions We offer a specialist diabetes service to assist: people with type 1 diabetes women with diabetes in pregnancy people on insulin pumps children and adolescents with diabetes people with complex type 2 diabetes other health professionals Our Mission
4 Our aim is to help clients achieve and maintain optimal diabetes management that fits with their lifestyle. We work in partnership with GPs, nurses, specialist doctors or other health providers. Common Medications Information of medications and insulin s will be given during the placement. Skills you may become familiar with during this Clinical Placement For the delivery of safe diabetes care you should be able to: Describe the two main types of diabetes mellitus State the normal blood glucose range Perform blood glucose monitoring on self and be able to teach the procedure to a person with diabetes Identify 2 main oral hypoglycaemic agents Describe the effect of oral hypoglycaemic agents on blood glucose levels and be aware of their side effects Demonstrate a fundamental knowledge of insulin eg. action/side effects Demonstrate competence in preparation and administration of insulin Describe preferred sites for insulin injection and rationale for rotation of sites Describe signs and symptoms of hypoglycaemia List possible causes of hypoglycaemia Describe the treatment for mild and severe hypoglycaemia Have an awareness of sick day management for patients with diabetes Demonstrate awareness of potential complications of diabetes Demonstrate awareness of healthy lifestyle and ways to prevent diabetes Useful Readings (links) NZ Guidelines Group Website see Type 2 Diabetes Healthpoint web page.look for diabetes and endocrine site
