2 Palm Beach County 2014 Diabetes Risk Community Assessment Data Palm Beach County Diabetes Coalition Mission Statement: The Diabetes Coalition of Palm Beach County is an alliance of healthcare organizations and community partners working together to prevent diabetes and improve the lives of those living with or those who are touched by the disease through: Education Awareness Quality Services Management Advocacy Vision Statement: Palm Beach County is a national model for controlling and preventing the incidence of diabetes. 2
3 Palm Beach County Diabetes Coalition In an effort to raise awareness of Diabetes Mellitus and its growing prevalence throughout Palm Beach County, 28 community organizations participated in administering 2,578 ADA risk assessments to 67 different sites. In addition some community members were able to receive free education, glucose level testing, cardiovascular risk assessment and linkage to services within Palm Beach County. The information gathered was analyzed by the FAU Christine E. Lynn College of Nursing Community Health Center and provides further insight on high risk populations based on administered ADA risk survey and biomarkers (Random: glucose, triglyceride, LDL, HDL, total cholesterol, systolic and diastolic blood pressure) for a portion of this sample (1071). 3
