Modue II Seizure First Aid, Action Pans, and Deegation Issues
Learning Objectives: Modue II Provide appropriate first aid for a student during and after a seizure Identify when a seizure is an emergency and know the appropriate response 2
Learning Objectives: Modue II Describe the steps in the seizure action panning process Identify the essentia components of a Seizure Action Pan Know key issues reated to the deegation of medication administration 3
Routine First Aid: Care and Comfort Most seizures are not medica emergencies Basic first aid may vary depending on whether there is: No change in awareness or consciousness Atered awareness Loss of consciousness Don t give anything by mouth unti the student is back to norma state and abe to swaow normay 4
Interventions for Seizure First Aid May be used at any time during a seizure (VNS magnet) May be prescribed after a specific number of seizures, ength of seizure, or change in pattern (Rescue med) 5
No change in Consciousness (Simpe Partia Seizure) Stay cam Time seizure Reassure student that he or she is safe Expain to others if necessary Protect student s privacy 6
Atered Awareness (Compex Partia Seizure) Speak softy and camy Guide away from potentiay harmfu objects such as tabes, chairs and doors Aow for wandering in a contained area If asts 5 minutes beyond what is routine for that student or another seizure begins before fu awareness is regained, foow emergency protoco DO NOT restrain or grab (may resut in combativeness) DO NOT shout or expect verba instructions to be obeyed 7
Loss of Consciousness (Generaized Tonic-Conic Seizure) Protect from potentiay harmfu objects Observe and time events Ensure airway is unobstructed Cushion and protect head Turn student on one side Remain with student unti fuy conscious Foow the student s Seizure Action Pan 8 DO NOT put anything in mouth DO NOT restrain
When is a Seizure an Emergency? First time seizure Convusive seizure asting more than 5 minutes Repeated seizures without regaining consciousness More seizures than usua or change in type Student has diabetes or is pregnant Seizure occurs in water Student is injured Parents request emergency evauation Foow the seizure emergency definition and protoco as defined by heathcare provider and incuded in the Seizure Action Pan 9
Use of PRN Rescue Medications Prescribed for seizure custers and proonged seizures Emergency protoco shoud incude: Medication name How and when it shoud be given Specific administration instructions What to do foowing administration Monitor responses and side effects Foow Seizure Action Pan emergency response protoco 10
Tonic-Conic Seizure in a Wheechair Do not remove from wheechair uness necessary Secure wheechair to prevent movement Fasten seatbet (oosey) to prevent student from faing but remove harness bet to prevent choking Protect and support head Ensure breathing is unobstructed and aow secretions to fow from mouth Pad wheechair to prevent injuries to imbs Foow reevant seizure first aid protoco 11
Tonic-Conic Seizure on a Schoo Bus Safey pu over and stop bus Pace the student on one side across seat facing away from the seat back (or in the aise if necessary) Foow standard seizure first aid protoco unti the seizure abates and chid regains consciousness Continue to the destination or foow schoo poicy 12
Tonic-Conic Seizure in Water Pace the student on their back and support head so that their head, mouth and nose are aways above the water Remove the student from the water as soon as it can be done safey If the student is not breathing, begin rescue breathing Aways transport the chid to the emergency room even if he/she appears fuy recovered 13
Seizure Action Pans
Seizure Action Panning Assess student needs and gather information Customize a Seizure Action Pan Teach schoo personne and taior interventions as needed 15
16
Questions for Parents of a Student with Seizures Encourage parent(s) to compete action pan and may take persistence May be hepfu to interview the parent(s) to obtain and carify information Update annuay and when any changes occur 17
Seizure Observation Record To be competed by schoo personne when reporting a seizure(s) Heps to identify seizure types, duration, triggers, and patterns Hepfu to use for panning appropriate seizure pans, safety precautions, and need for accommodations or changes 18
Exampe Copies of the Seizure Observation Record and Seizure Action Pan are avaiabe on the EFEPA website: -and-resources/schooinformation/
Seizure Action Panning Process Requires input and panning by the heath care provider(s), parent(s), student, and schoo nurse. Provides basic information about student s seizures, seizure first aid, safety, and emergency response 20
Seizure Action Panning Process Shoud generay be signed and approved by the treating heath care provider, parent, and schoo nurse Distribute to reevant schoo personne with parent(s) permission at the beginning of a schoo year, upon diagnosis or when a change in heath status occurs 21
Assessment & Information Gathering Gather seizure history and treatment information Speak with the student s medica team to carify treatment and emergency response protoco Observe and document any in-schoo seizures Speak with teachers and other schoo personne about: Possibe seizure precipitants (triggers) Observed or perceived impact on earning and behavior 22
Assessment & Information Gathering cont d Consider transportation issues Compete a schoo safety assessment Identify student activities that may need to be modified or necessitate specia precautions Identify environmenta risks and need for safety precautions Determine best method to communicate with parents and medica team 23
Communication Tips Set up a method for communicating with parents/guardians on a daiy or weeky basis Be a iaison for parents and teachers regarding any status changes Have teachers reguary note physica, emotiona or cognitive changes Create a substitute teacher foder with the Seizure Action Pan and other reevant information and keep this foder in a secure ocation 24
Deegation Issues: Laws and Mandates Appicabe Federa aws or mandates that may impact deegation incude: Section 504 of the Rehabiitation Act of 1973 Americans with Disabiities Act (ADA) Individuas with Disabiities in Education Act (IDEA) Loca and state aws, such as nurse practice acts and schoo district poicies, generay govern the administration of medications in schoos What are the state or schoo reguations in your area for deegation of medication administration? 25
Tips for Effectivey Managing Deegation Know state nurse practice act, schoo district poicies, and appicabe state and federa mandates and aws Recognize that identifying when a change in behavior or seizure occurs does not require a skied nursing assessment and is part of basic seizure first aid Bring parents and schoo personne together to attempt to find a workabe soution Expain to a parties that you are obigated to put the heath, safety and wefare of the student first 26
More Tips for Effectivey Managing Deegation When a schoo nurse deegates a task under his/her nursing icense the nurse is responsibe for the foowing: Ensuring that the deegate is appropriate Providing training and ongoing assessment and documentation of the competence of a deegate Ongoing assessment of the student's heath outcome 27
The Chaenge of Deegation Using Diazepam recta ge and other seizure rescue treatments may be handed in different ways depending on schoo district poicy, schoo poicy and parent preference. How is it handed in your schoo? What chaenges have you come across? How have you deat with these and simiar issues in your schoo? 28
Exercise: Review a Seizure Action Pan Review and discuss how you might compete and use the foowing sections of the Seizure Action Pan: Seizure Information Basic First Aid Emergency Response Specia Considerations and Precautions Use case study for discussion and practice 29
