It coud be sma changes, such as watching what and how much you eat or adding physica activity to your day. Or it coud be big changes that invove understanding your emotions and how they can affect your heath. The AHLP heps you discover changes big or sma that wi hep you ive heathier and earn to confidenty manage your heath. AHLP workshops are for Abertans interested in earning how to manage chronic conditions. Famiy, friends and caregivers are aso wecome to attend. We recommend participants are: Capabe of participating in group settings Abe to understand basic Engish At east 18 years of age or oder. Workshops vary by community, but in genera cover: Disease-specific education for conditions such as diabetes, obesity, high bood pressure and more Genera heath topics such as nutrition, reducing stress and seeping we Supervised exercise programs. Workshops are ed by heathcare professionas incuding heath educators, registered dietitians, registered nurses and exercise speciaists. 3 Better Choices, Better Heath 6 Anyone iving with: Diabetes Chronic pain Heart disease Arthritis Breathing concerns Depression/anxiety Weight management Cancer HIV Fibromyagia Simiar chronic heath conditions. 2½ hours each Nutrition, physica activity and stress affect your ife, especiay when you re iving with ongoing heath concerns. 4 Craving Change 3 2½ hours each Anyone who wants hep improving their eating habits. What you eat, how you eat and how much you eat are a part of your eating habits. Shaping them begins with understanding your triggers and reationships with food. In these workshops, you earn how you can form better eating habits that ast a ifetime. Topics incude: Food triggers, such as emotions, situations, events and cravings Strategies for ong-term, heathy eating habits How to form a heathy reationship with food. Diabetes: The Basics Two options avaiabe: 4 Or 2 Anyone at risk of deveoping diabetes Anyone diagnosed with pre-diabetes or Type 2 diabetes. 2½ hours each One 6½-hour session (pease bring a unch) and one 2½-hour session Take the test To determine your risk of deveoping diabetes, take this fast and easy onine test at take2minutes.ca. 5 Eating Away From Home and During Specia Occasions 1 2½ hours Anyone ooking for tips and strategies to prevent overeating whie away from home or during specia occasions. Food is often centra to hoidays, parties and specia occasions. It aso tends to be higher in caories, fat or sugar, and can ead to overeating and weight gain. In this workshop, faciitated by a registered dietitian, you wi earn how these occasions can affect what you eat and how much you eat. You eave with strategies to hep prevent overeating. Topics incude: How buffets, parties, vacation and hoiday eating can affect how much you eat Strategies to reduce overeating when you re away from home and during specia occasions. Getting Started: Panning for Success 1 2½ hours Anyone who woud ike to earn more about the benefits of maintaining a heathy weight and how to buid a pan to reach weight-oss goas. Maintaining a heathy bood pressure and choestero eve reduce your risk of heart disease, stroke, kidney faiure and many other inesses. In this workshop, offered in coaboration with AHS Nutrition Services, you wi earn what affects your bood pressure and choestero eve and what you can do to manage them. Habits can infuence the choices you make when it comes to your heath. Learning new ways of thinking and forming new habits can hep maintain heathy ifestye changes such as eating heathier or getting exercise. In this workshop you wi earn what heathy ifestye changes ook ike and strategies to hep you maintain them. Topics incude: How to make and keep heathy ifestye changes Strategies such as prioritizing, journaing, probem-soving, thinking skis and goa-setting. Managing Stress: Effectivey Reduce and Manage the Stress in your Life 2 2½ hours each Anyone who fees stress and woud ike to earn how to reduce or prevent it. Many things can cause stress. It can stem from your job, heath, a conficts in beief, or a major ife change. 8 Moving Matters: Incuding Physica Activity in your Day 1 2½ hours Anyone ooking to earn more about physica activity and how to incude it in their day. Physica activity is any activity that s part of your daiy ife. It can incude simpe things ike ceaning, shoveing snow, taking the stairs and carrying groceries. Incuding more physica activity in your day can hep you maintain a heathy weight and better manage chronic conditions. In this workshop you wi earn about physica activity and how to incude it in your day. Topics incude: Benefits and barriers to getting active Tips on how to increase your physica activity How to set persona activity goas. How much activity do I need? Aduts need 150 minutes of moderate to vigorous activity each week and kids need 60 minutes or more per day. Moderate activity means your breathing and heart rate are higher than norma and you may be sweating. Ready for Change 1 1½ hours Anyone who woud ike to earn more about making heathy ifestye choices to improve their heath. Living heathier starts with making positive, informed choices when it comes to things such as nutrition and physica activity. In this workshop, you wi earn how to make changes to improve your heath. 9 Respiratory Education 9 1 hour each Anyone who has been diagnosed with a chronic ung condition, such as Chronic Obstructive Pumonary Disease (COPD). COPD is caused by damage to the ungs from smoking and ong-term exposure to chemica fumes, dust or air poution. In this workshop you wi earn more about COPD, what you can do to prevent more damage and tips to hep you breathe easier. Topics incude: What is COPD Factors that affect COPD Hepfu breathing techniques Common respiratory medications and devices. Seep We 1 1½ hours Anyone who wants to seep better. Quaity seep refreshes the body and mind. It aso heps us make better ife choices and contro food cravings. In this workshop, discover what quaity seep means, how it reates to your heath and tips for better seep. 10 Stress Less 1 1-1½ hours Anyone who fees stress and is ooking for ways to reduce or prevent it. Stress can take many forms. Some signs incude feeing overwhemed, difficuty concentrating, changes in appetite, disrupted seep, chest pain, headaches and more. Some stress is good for you, but chronic, ongoing stress is not. 11 Supervised Exercise Information Session 1 1 hour Anyone ooking to earn more about the Supervised Exercise Program and what to expect. Learning what to expect and asking questions can hep you fee more comfortabe with exercising. In this workshop you wi meet the program s exercise speciaist, be abe to ask questions and decide if the program is right for you. Supervised Exercise Program vary by community. Ca for more information. 1-1½ hours For individuas with one or more chronic conditions interested in earning how to exercise safey and experience the heath benefits. The amount of caories you need in a day depends on how active you are, your gender and aso your age. Eating or drinking more caories than you need can ead to weight gain and obesity. In this workshop, taught by a registered dietitian, you wi earn ways to safey reduce your caorie intake and maintain a heathy weight.
