6 UNIVERSITY SYSTEM OF FITNESS REIMBURSEMENT PROGRAM Employees are eligible for the Fitness Reimbursement Program if they participate in the University System of New Hampshire (USNH) medical plan administered by Cigna. The program pays up to $500 per individual or family, per calendar year. This $500 is payable to those fitness facilities which are participating in the USNH Fitness Reimbursement Program. Financial agreements for the monthly membership fees are between the fitness facility and the member. Members must work out at their fitness facility eight (8) times a month and log the work out activity in accordance with the tracking process (i.e., log book or usage report) established at the fitness facility for that fitness facility to be eligible for reimbursement for that member. Members who fail to meet the eight (8) times at their fitness facility in any given calendar month, are financially responsible to their fitness facility for that month s membership fee. The fitness facility must have proof of the member s attendance. The fitness facility is paid the month following the month attended by the member. If a family has two members who are employed by USNH and each are individual participants of the medical plan administered by Cigna, than each are eligible for $500. If both employees are covered under the same plan only one individual is eligible for the benefit. The $500 benefit is not available for any fitness facilities which are not participating in the USNH Fitness Reimbursement Program. Any unused balance is not carried over to the next calendar year. This benefit only covers the standard fitness facility membership. Any additional fees for services, such as tanning, child care or additional fitness classes not covered by the monthly membership, are the responsibility of the member. Members can participate at any time during the year. Members can only participate with one fitness facility per month and only transfer their membership to another fitness facility one time during a calendar year. Members may transfer their reimbursement benefit to their dependents; however, only one member in a family can take advantage of this benefit at a time and can only transfer this benefit one time, per calendar year. Fitness facility will not be reimbursed for the member if the $500 limit is exceeded. The member is financially responsible to the fitness facility for any outstanding balance. If the employee s fitness facility is not listed as a participating fitness facility, an employee can nominate their fitness facility by providing the following information to their Human Resource Department. Mandatory Information required: o Facility name, address & phone number o Confirmation that employee or dependent is a current member of the fitness facility they are nominating. Additional information including the Facility contact name, title and facility website should be included when Possible but not required. Please note that this nomination does not guarantee that the fitness facility will become a participating fitness facility. It is up to the fitness facility if they want to be part of the program. Fitness Facility Directory is current as of January 1, 2018 Cigna administers the Fitness Reimbursement Program on behalf of University System of New Hampshire but does not credential or select the fitness facilities.
