Randomised Trials of Carotid Interventions Will the Changing Technology of Membrane Mesh Stents Shape The Future? Dr Sumaira Macdonald MBChB (Comm.), FRCP, FRCR, PhD, EBIR Vascular Interventional Radiologist Chief Medical Officer, Silk Road Medical, Sunnyvale, California ACST2 April 2016

Lecture Plan: Perils of trialing a rapidly evolving technology within an RCT construct Stent design considerations Impact of stent design parameters on outcomes Procedural stroke hazard Surrogates (DWMRI new white lesions) Current membrane mesh stent platforms

Perils of trialing a rapidly evolving technology within an RCT construct Leaving no questions unanswered Criticisms of the use of suboptimal/obsolete technology Selection bias Anatomy Mind share

The Impact of Stent Design Parameters On Outcomes: Surrogate Markers (DWMRI Brain & TCD)

32 studies: 1363 CAS & 754 CEA Ipsilateral DWI lesions: 51% open cell stents 31% closed cell stents p < 0.01

14 asymptomatic patients 1:1 RCT eptfe covered membrane stent (symbiot) vs. Wallstent Microembolisation (TCD) and DWI Schillinger et al JEVT 2006;13:

Outcomes Stratifed By Stent Design:

Available Membrane Mesh Stent Platforms

GORE SCAFFOLD

500 micron pore size

Modified Chandler loop with recirculating blood Heparin bonded devices thrombus free after one hour exposure to human blood Untreated control contained adherent thrombus Coagulation biomarker prothrombin fragment 1 & 2 (F1 & 2) revealed significantly lower levels with heparin bonded stent

CONCLUSIONS: Closed cell stents are associated with fewer DWI lesions than open cell stents Membrane mesh stents are associated with fewer TCD MES & DWMRI lesions than closed cell stents Closed cell stents are consistently associated with lower stroke & death than open cell stents IN SYMPTOMATIC PATIENTS with no meaningful difference in asymptomatic patients

CONCLUSIONS: Likely too small a final number of cases with membrane mesh stents in ACST2 for a valid sub-set analysis Is a membrane mesh stent a relevant consideration for a purely asymptomatic patient population?
