4 For 37 years I knew my diagnosis, but I did not know me. I was afraid if I gave up my diagnosis, I would not know who I was. I can work against my diagnosis and become myself again I can relate to this because there are people out there who want to know what my diagnosis is rather than who I am. By keeping reminders of who I am I can keep myself in check! You have to sacrifice to change on a daily basis and that it is something you have to do for yourself Diagnosis: -the act of identifying a disease, illness, or problem by examining someone or something GROW Model WHAT S NEXT DECIDE NEXT STEPS GOALS LONG TERM GOALS PERFORMANCE GOALS SESSION GOALS ASSES CURRENT REALITY REALITY Goals - What do you want? Reality What is happening? Options What might you do? What s next What will you do? 5 Questions to Help Set a Goal What is something you have wanted to do for a long time for yourself, but just haven't gotten around to it? What seems to get in the way? What is preventing you? What might you do to overcome these obstacles? What is your plan of action? By when? What resources do you need? Commitment level?
