2 COURTYARD MANOR OF WIXOM 2017 MISSION Courtyard Manor of Wixom provides compassionate care that empowers our residents and facilitates recovery. VISION Courtyard Manor of Wixom will be recognized as the premier provider for specialized residential services. Through collaboration with residents, guardians/family members, and community providers, our residents will achieve independence, self-awareness, and improved quality of life. Courtyard Manor will be the statewide leader and advocate in doing what is right with, and, for residents and families. VALUES People are interdependent and equal citizens, regardless of ability. Our residents are treated in a highly integrated and holistic manner. We will provide the highest level of care and services. Those who seek our services are welcomed and treated with dignity and respect. Participation of stakeholders in all levels of decision making is welcomed and must be fostered. Community partnership, collaboration, and appropriate information sharing at all levels are essential to effective utilization of supports and services and to help our residents achieve community integration. All interventions are based on best practice when psychiatrically or medically appropriate. It is important to support and serve each resident through mission-based, person/family-centered practice, and community-based inclusive services. Residents are afforded opportunities and experiences for developing relationships with individuals and places in the community. We are part of a larger community; we are here to serve our community in such a manner that the community achieves valued benefits as a result of our existence. All members of our organization, including those agencies with whom we contract, must be committed to values, demonstrate knowledge, and actively engage in relevant practices that promote the values and principles just described. 3/15/17 2
3 COURTYARD MANOR OF WIXOM 2017 STRATEGIC PLAN CYM of Wixom completed our 2017 strategic plan. The plan identifies major initiatives and goals that are designed to improve Care & Treatment, Services, and the Environment where services are delivered. The Strategic Plan is divided into two sections. Section I focuses on Clinical Services and Section II focuses on Administrative Services. CLINICAL GOALS FOR 2017: CYM s Strategic Plan includes the following goals for 2017 to further enhance services to our residents: 1. Expand evidence-based, outcomes-oriented clinical programming. 2. Engage in initiatives to reduce the Stigma of Mental Illness 3. Increase Resident and other Stakeholders level of satisfaction with CYM Program/ Services 4. Incorporate technology into clinical programming to increase supports and enhance services ADMINISTRATIVE GOALS 2017: 1. Ensure continued implementation and compliance with CARF, HIPAA, and other regulatory body standards 2. Improve the organization capabilities 3. Improve the physical plant 4. Retain competent staff 5. Maintain full occupancy 6. Improve the use of technology and information systems 7. Decrease reliance on the paper record 3
4 Mental Health Awareness Month Did you know?... National Mental Health Month raises awareness about mental illness and related issues in the United States. In recent times, attitudes towards mental health issues appear to be changing. Negative attitudes and stigma associated with mental health have reduced and there has been growing acceptance towards mental health issues and support for people with them. Despite this shift in attitude, the idea of a mental health awareness campaign is not a recent one. In the late 1940's, the first National Mental Health Awareness Week was launched in the United States. During the 1960's, this annual, weekly campaign was upgraded to a monthly one with May the designated month. During this month, National Health America, the main organization which sponsors this event, run a number of activities which are often based on a theme. In 2010, the theme was 'Live Your Life Well'. 'Live Your Life Well' encouraged people to take responsibility for the prevention of mental health issues during times of personal challenge and stress. Many mental health problems can be avoided by taking positive lifestyle choices in how we act and think before they can manifest. To coincide with Mental Health Awareness month, Other mental health campaigns & activities also run during this month. National Children's Mental Health Awareness Day is one such campaign. This event is sponsored by the American Psychological Association (APA). Other activities have included 'Blogging for Mental Health' and 'Help For People Seeking Psychological Services'. 5 STIGMA Stigma refers to the extreme disapproval of a person who differs from their cultural norms; it distinguishes a person from other members of society and single s them out to be ridiculed. Stigma is one of the most challenging aspects of living with a mental health condition and causes people to feel ashamed for something that is out of their control. Stigma also prevents many people from seeking the healthcare treatment they need and from speaking out. Three out of four people with a mental illness report that they ve experienced stigma or a mark of disgrace that sets them apart from everyone else. When a person is labelled by their illness they may experience feelings of shame, anger and hopelessness that may prevent them from seeking social support, employment, or treatment for their mental illness. We can work together to reduce and change the way people perceive mental illness by: 1. Knowing the facts; educate yourself and others about mental illness. 2. Talking openly and non-judgmentally about mental health issues. 3. Be aware of your own prejudices and working to eradicate negative opinions toward others. 4. Choose your words carefully and be mindful not to use words that may be offensive to others. 5. Focus on the positive and see the strengths and qualities a person has rather than an illness. 6. Support people and their efforts toward recovery. 7. With compassion in your heart, talk about mental illness with your family and friends anytime the opportunity arises. 6 Recovery In the past, treatment interventions approached a person with a mental illness as an outcast needing to be institutionalized and segregated from society; this person was thought to be someone unable to achieve a higher education, unable to secure stable employment, unable to manage a long-term relationship or raise a loving family. Out with the old and in with the new. Recovery is a 21 st century treatment approach encompassing principles of selfdetermination and personalized care for all persons with a mental illness. Recovery-oriented practice emphasizes hope, social inclusion, community participation, personal goal setting and self-management. Recovery is a treatment approach that encourages and supports self-efficacy, self-management and self-mastery. Personal Development, Empowerment, Supportive Relationships, Dignity and Respect: these are the foundations that guide our current treatment programs. Recovery is a personal journey rather than a set outcome that focuses on a person and his or her goals rather than an illness with unmanageable symptoms. At Courtyard Manor of Wixom, we believe recovery is a person first approach, a collaborative effort, a goal to strive for; a new way to build our communities and ensure that everyone s allowed in the club. 7 There is No Safe Amount of Smoking Smoking cigarettes is the number one leading cause of preventable disease and death in the United State responsible for 480,000 deaths each year. Tobacco smoke contains a deadly mix of more than 7,000 chemicals causing cancer, heart disease, stroke, lung disease, diabetes and COPD. In 2014, the tobacco companies spent $9 billion a year in promotional advertising; that s nearly $25 million dollars a day or about $1 million dollars every single hour! People who stop smoking greatly reduce their risk for disease and early death: in fact 20 minutes after you quit smoking your blood pressure and pulse decrease and the temperature of your hands and feet increases. 24 hours after quitting oxygen levels in your blood increase and your risk of heart disease decreases. 2-3 weeks after quitting your circulation improves as does your exercise tolerance. 1-9 months after quitting coughing, sinus congestion, fatigue and shortness of breath decreases years after quitting your risk of having a stroke is reduced to that of people who have never smoked.
