1 Appendix The following section describes tests that are available to help you get to the root cause of your health problems. These tests are recommended throughout the book for different health conditions. The tests covered in this section include: The Comprehensive Stool Analysis (CSA) from Doctor s Data Intestinal Permeability Assessment Food Sensitivity Test from EnteroLab Lactose Intolerance Breath Test CSA from Parasitology Center Lactulose Breath Test GI Effects Stool Profile Adrenocortex Stress Profile Hair Toxic Element Profile Neurotransmitter Profile Heidelberg ph Diagnostic test Of these tests, I would like to highlight the first two, the Comprehensive Stool Analysis (CSA) from Doctor s Data, and the Food Sensitivity Test from EnteroLab. These two tests are critical to the reversal or management of all the conditions in this book. The CSA is the test I use to determine what is going on in the gut. I would not hesitate to say this test would be helpful for anyone wishing to manage their health long-term, even if they have not exhibited symptoms of any kind. The CSA gives a good baseline picture of your health in a similar way as preventive blood work does. This test will show a gut imbalance even before symptoms develop. Because gut imbalance can lead to so many health conditions, correcting this imbalance is essential, even before health conditions arise. In cases where the person has already developed a health condition, this test is a must. This test will give you a picture of the health of your entire body. Each of the conditions in this book may begin with an imbalance in the gut. It is amazing the amount of people who go to doctor after doctor with an enormous amount of testing and money spent without ever finding out what is wrong. The important part of my message, and the message in this book, is about looking to the gut first for the solution to your health problems, as well as preventively to keep yourself healthy for life. 559
2 or sensitivity to gluten that has not yet progressed to celiac disease. A blood test is used to detect celiac disease, but the gluten reaction occurs in the intestine. Immune complexes will only be found in the blood after significant damage to the intestine has occured. Detecting food sensitivity before this damage occurs is key. That is why the stool sensitivity test is so important. This is a very hard concept to get across. Many people suffer from chronic health problems due to the fact that they regularly consume foods that create an immune response in the gut that they may not feel directly, but that occurs daily. Over time this food reaction slowly creates an imbalance in the ratio of good to bad bacteria in the gut, creating inflammation and then leaky gut. The leaky gut proteins (from gluten or dairy) enter the bloodstream and circulate throughout the body, creating more inflammation and even autoimmunity. Food sensitivities are an underlying factor in many health conditions. When I consult with people who have chronic health challenges, I always request they do the CSA, as it provides a direct way for me to help them. This test takes away the guess work! When you address gut health, the pathway to healing the body just happens. Here is the best advice I can give anyone: Stop looking for the solutions to symptoms look at your inner terrain. You will find the real source of health in your gut. Then you can heal from the inside out. Read on for more information about the the tests recommended throughout this book. The second test I highly suggest for people struggling to find the answer to ANY health problem is the EnteroLab food sensitivity test. If there is anyone to thank for his/her work in this field, it is Dr. Fine at EnteroLab. Dr. Fine s work is saving the lives of many people, myself included. You can also have your genes tested to see if you are genetically predisposed to these food sensitivities. With many people I work with, I insist they take this test, which is critical to getting to the bottom of many disease conditions. People often have chronic health problems due to the fact that the gut becomes out of balance. But what causes that imbalance? In many cases, food sensitivity. 3 Appendix The Comprehensive Stool Analysis or CSA from Doctor s Data (also available with Parasitology to detect parasites) The CSA is an invaluable noninvasive diagnostic assessment that permits practitioners to objectively evaluate the status of beneficial and imbalanced commensal microflora including Lactobacillus and Bifidobacterium species, Clostridium species, pathogenic bacteria, yeast/fungus and parasites. Inflammation can significantly increase intestinal permeability and compromise assimilation of nutrients. The CSA determines the extent of inflammation, whether caused by pathogens or inflammatory bowel disease (IBD), and can be assessed and monitored by examination of the levels of biomarkers such as lysozyme, lactoferrin, white blood cells and mucus. These markers can be used to differentiate between inflammation associated with potentially 561 life-threatening inflammatory bowel disease (IBD), which requires life-long treatment, and less severe inflammation that can be associated with the presence of invasive pathogens. Since the vast majority of secretory IgA (siga) is normally present in the GI tract where it prevents binding of pathogens and antigens to the mucosal membrane, it is essential to know the status of siga in the gut; siga is the only bona fide marker of humoral immune status in the GI tract. The CSA also supplies information regarding the efficiency of digestion and absorption from the measurement of the fecal levels of elastase (pancreatic exocrine sufficiency), muscle and vegetable fibers, carbohydrates, and steatocrit (percent total fat). The CSA is offered online at and results are mailed to your home.
4 EnteroLab has developed a unique screening test for gluten sensitivity, and dietary yeast, cow s milk, and chicken egg sensitivity. The test is more sensitive and specific than tests in current use (U.S. patent issued; International patents pending). Their method utilizes stool rather than blood as the testing substrate. The rationale of using stool rather than blood for testing food sensitivity is that immunologic reactions to proteins in the diet that cause these reactions are centered within the intestinal tract and not in the blood. 5 Appendix Comprehensive Stool Analysis from Parasitology Center For cases in which parasites are specifically suspected, the best stool analysis comes from Dr. Amin at the Parasitology Center. The analysis tests fecal, blood, tissues and skin specimens to diagnose and taxonomically identify human and animal-borne parasitic organisms and agents of medical and public health importance. These include over 40 species of protozoan (single-celled organisms), cysts and trophozoites, eggs and other stages of metazoans (multiple-celled parasites like roundworms, tapeworms, flukes, and other parasitic organisms) that may be found in biological specimens, e.g., fecal, blood, tissues, and skin. DNA assessment is specific and accurate, avoids the pitfalls of sample transport, reports results as specific numbers, and is more sensitive than classic laboratory methods. In addition to much more comprehensive bacteriology, mycology, and parasitology assessments, GI Effects Profiles report drug resistance genes, antibiotic and botanical sensitivities, gliadin-specific siga, Elastase1, plus other inflammation, digestion, and absorption markers. The test does not measure live microorganisms, however, which is one major difference between this stool test and the Comprehensive Stool Analysis. Hair Toxic Element Exposure Profile from Doctor s Data Hair is an excretory tissue for potentially toxic elements and essential and nonessential elements. In general, the 563
6 amount of an element that is irreversibly incorporated into growing hair is proportional to the level of the element in other body tissues. Therefore, the Hair Toxic Element Profile provides an indirect screening test for physiological excess of elements in the body. Clinical research indicates that hair levels of specific elements, particularly potentially toxic elements such as cadmium, mercury, lead and arsenic, are highly correlated with pathological disorders. For such elements, levels in hair may be more indicative of body stores than the levels in blood and urine. Hair element analysis provides important information which, in conjunction with symptoms and other laboratory values, can assist the physician with an early diagnosis of physiological disorders associated with aberrations in essential and toxic element metabolism. As protein is synthesized in the hair follicle, elements are incorporated permanently into the hair with no further exchange or equilibration with other tissues. Scalp hair is easy to sample, and because it grows an average of one to two cm per month, it contains a temporal record of element metabolism and exposure to toxic elements. Toxic elements may be 200 to 300 times more highly concentrated in hair than in blood or urine. Therefore, hair is the tissue of choice for detection of recent exposure to elements such as arsenic, aluminum, cadmium, lead, antimony and mercury. The CDC acknowledges the value of hair mercury levels as a maternal and infant marker for exposure to neurotoxic methylmercury from fish. Intestinal Permeability Assessment from Genova Diagnostics The Intestinal Permeability Assessment is a noninvasive assessment of small intestinal absorption and barrier function in the bowel. The small intestine uniquely functions as a digestive/absorptive organ for nutrients as well as a powerful immune and mechanical barrier against excessive absorption of bacteria, food antigens, and other macromolecules. Both malabsorption and increased intestinal permeability (leaky gut) are associated with chronic gastrointestinal imbalances as well as many systemic disorders. Increased permeability of the small intestine can: Increase the number of foreign compounds entering the bloodstream. Allow bacterial antigens capable of cross-reacting with host tissue to enter the bloodstream, leading to auto-immune processes. Enhance the uptake of toxic compounds that can overwhelm the hepatic detoxification system and lead to an overly sensitized immune system.
