3 Northumbria Healthcare NHS Trust Northumbria Healthcare NHS Trust (the Trust) is an acute and community trust serving a population of 530,000 people, living throughout the areas of North Tyneside and Northumberland. The Trust employs over 7,000 staff, making it the largest single employer in the area. Northumbria Healthcare was formed in April 1998, from the merger of the following four organisations: North Tyneside Healthcare NHS Trust Cheviot and Wansbeck NHS Trust Northumberland Community NHS Trust Hexham General Hospital (formerly part of the RVI Trust) The trust manages a network of hospitals, including three district general hospitals (at sites in North Tyneside, Ashington and Hexham) and several smaller local hospitals (in Berwick, Alnwick, Rothbury, Haltwhistle, Blyth, Morpeth and Wallsend). In 2001 and 2002 some community and support services, previously provided by Northumbria Healthcare, were transferred to two new organisations, North Tyneside PCT and Northumberland Care Trust.

4 Facts about Wansbeck General Hospital 1. Wansbeck General Hospital, on the outskirts of Ashington, opened in June 1993 to begin the phased replacement of the town s original 1914 acute general hospital Ashington General. 2. Wansbeck General serves a population of almost 250,000 people in north and south east Northumberland from Berwick on the Scottish border through to Cramlington and Newbiggin on the edge of Tyneside. 3. Wansbeck General Hospital employs around 1,500 staff and deals with around 26,000 in-patients, 56,800 outpatients, 12,400 day cases and 41,600 emergency attendances every year. 4. Wansbeck General was one of two low energy hospitals built in the UK under a Department of Health pilot scheme and won a Green Building of the Year Award in Features of it s pioneering design and construction include high levels of insulation, extra windows and site-landscaping which help Wansbeck achieve much lower bills and emissions than traditionally built hospitals. 5. The hospital generates up to a tenth of the electricity it needs by harnessing the natural power of its site s strong local winds using a wind turbine in it s car park. Extra power is saved using a pioneering combined heat and power (CHP) system that reclaims heat from engine and exhaust gases, which would otherwise be wasted. 6. Wansbeck General Hospital has around 390 beds for specialities like general medicine, general and orthopaedic surgery, day surgery, maternity and elderly medicine. 7. A new 20 million wing opened in March 2003 incorporating maternity, gynaecology, outpatients, ante-natal, rehabilitation, day case surgery and dental departments. A separate Child Health Centre was also built as part of the same development, this is operated by Northumberland Care Trust and the Northumberland, North Tyneside and Newcastle Mental Health Trust. 8. Other ongoing developments on the site include an extra 2.1 million remodelling of the hospital s A & E department, a new MRI scanner suite and a new cardiology cath. lab. Training Regular in-service training is held in the Physiotherapy Department. Students are encouraged to attend. 12 Students can expect to treat first patient on: Day 3 Average number of patients students will be expected to treat each day. Students will be involved with up to 4 patient groups at the unit. The group size can vary from 4-15 patients. During the placement the student will be expected to lead part of the group sessions and regularly review the patients within the class. As well as group work students will also have an individual caseload of musculoskeletal patients (as in a normal outpatient department). Students will see approximately 3-6 new patients a week. WEEK 1: Day 1: Induction session and orientation period Work shadow Hartford Unit staff (class work and individual assessments) Day 2-5: In the first week a large proportion of the time will be spent shadowing the staff at the unit to get an overview of the different classes. Training will also be provided including class teaching and class criteria. There will be opportunity for students to review the individual patients within the classes as well as assess new patients which will be added to the students individual caseload. Students will also be expected to plan and take a specific component of one of the classes at the unit. Students should be prepared to discuss their learning outcomes by day 3. WEEK 2-6: Students will have the opportunity to plan and implement all parts of the treatment programme involved in the different classes at the unit. This will involve specific exercises, general and functional circuits, cardiovascular exercises and education. Students will continue to review patients and alter treatment plans as appropriate within the classes as well build on their individual caseload. Students will be continually assessed though will receive a formal assessment halfway and at the end of the placement. During this placement students will have ample time to prepare classes including aims and objectives, class plans, exercise content etc., and will be able to discuss all classes with senior staff before taking the class. ADDITIONAL LEARNING OPPORTUNITIES There may also be an opportunity to attend one of the consultant clinics held at the unit. Quarterly departmental I.S.T. (Various topics).

13 Progression and the degree of supervision will be adjusted according to the year group, ability of the individual student and prior clinical experience.

15 GENERAL INTRODUCTION TO THE HARTFORD UNIT The Hartford Unit is a musculoskeletal / functional rehabilitation unit providing assessment, diagnosis and treatment for a wide range of musculoskeletal conditions. Patients are referred to the unit from Orthopaedic Consultants, GP s, ESP s and A&E for an individual physiotherapy assessment. The patients may then continue to be seen on an individual basis (as in a normal out patient department) or may be transferred into one of the classes at the unit for intensive rehabilitation if and when appropriate. Patients may also be transferred directly to the classes at the unit by a musculoskeletal physiotherapist within the trust, following assessment, if they meet specific class criteria. All patients transferred to the classes must attend an induction. The aims of the classes are to educate, advise, support and provide the patient with the opportunity to achieve the level of fitness and functional confidence which would enable them to return to normal activities including ADL s work and/or sport. The classes include; Early Legs Early ankles Advanced legs Spinal education Functional backs Upper limbs Food for thought group. The patients who attend the classes at the unit still receive individual reviews on a regular basis, by the physiotherapist(s) responsible for the class, and are continually monitored and assessed functionally in the classes. Some of the orthopaedic consultants hold regular clinics at the unit to review their patients who are attending the unit with the physiotherapist treating them.

To gain experience in class teaching and group therapy for patients with an orthopaedic/musculoskeletal condition Objectives 1.1 To assess patients with orthopaedic/musculoskeletal problems and formulate an effective treatment/management plan 1.2 To formulate aims and objectives of treatment/management 1.3 To be able to evaluate the effectiveness of treatment 1.4 To plan and implement group exercise sessions, with consideration to safety, class criteria, individual injuries, stages of rehabilitation and abilities 1.5 To provide effective explanation and demonstration of exercises/activities to both individuals and groups of patients 1.6 To progress patients through their rehabilitation with respect to their injury, stage of rehabilitation and ability
