Training for The Future The story so far. Catherine Kirkpatrick Consultant Sonographer United Lincolnshire Hospitals NHS Trust
Background 2013/2014 HEE established a sonographer workforce project group Representatives from LETB s, HEE, CASE, BMUS, RCR, SCoR, SVT amongst others Different HEI s developing varying education models
The Challenges Why is Change Needed Increasing demand for Ultrasound services Sonographers are on the Migration Advisory Committee s Shortage Occupation List (SOL) A range of local solutions to fill skills gaps, include using - and depleting - other skilled workforces
The Challenges Why is Change Needed? Undergraduate, postgraduate and postregistration training models but overwhelmingly post graduate/post registration Sonography/Sonographer not a statutorily regulated profession/title Challenge of clinical training capacity
Emerging Solutions Leeds 3+1 model taking the top radiographer graduates directly into US Cumbria developing direct entry into post grad with any undergrad degree Birmingham undergrad sonography course
How bad is the problem? 2015 HEE commissioned Centre for Workforce Intelligence (CfWI) to review current status of sonographers in the US This project supported the DH drive to ensure that the NHS has the right number of trained staff available to deliver current and future demand for diagnostic tests. It was also to help to improve HEE s understanding of the current ultrasound practitioner workforce
CfWI Findings 2016 ~ 33 % of Ultrasound workforce are aged 50 plus Agency, locum and bank (ALB) staff make up around 12 % of the total ultrasound workforce The total snapshot vacancy rate at the time of survey, was around 10 % This vacancy comprises post not filled by substantive or ALB staff
Progress to Date HEE and stakeholders have been exploring options for some time Broad stakeholder consensus that the present situation is not sustainable Degree apprenticeship Trailblazer in development
Integrated Career Pathway? HEE commissioned further work A Curricula Mapping Exercise to Develop NHS Sonography Eleanor D. Kennedy BSc PhD MBA E.D. Kennedy Consulting Ltd. Report Published March 2017 Integrated degree not viable

Skills for Health IWWWG Paper 5/2b Sonography Career Framework, Competence Framework and Transferable Role Templates (Interim) June 2017
Apprenticeship Trailblazer Expressions of Interest to develop apprenticeship 2016 Employer driven Apprenticeship levy income to employer Mid Yorks Team leading Apprenticeship standard drafted
HEE 2017 IIWWG CfWI Commissioned review for integrated degree Skills for health Still no outcome so. Task and Finish sub group developed to finalise sonography career development
Sonography Workforce Group This is a sub-group of HEE s Integrated Imaging Workforce Working Group (IIWWG), responsible for the sonography and diagnostic radiography workforces HEE-commissioned Career and Competence Framework (currently in draft) will for the first time articulate career pathway and opportunities for career development
Aims of the Group The Sonography Workforce Sub-Group (SWSG) is a task and finish group established to provide a national solution to sonography workforce shortages. It intends to develop a workable solution which provides a sustainable, new, supply of direct entrants into the sonography workforce. The work of this should contribute to ongoing discussions around the development of sonography into a recognised profession
Key Challenges Registration and Regulation How to advance the development of sonography as a profession? Impact of nonregulation on ability to work? Voluntary register options? Draft brief for CHPO Draft HCPC info on how/where regulated on register Issues of protection of title No plans for annotation for post grad sonography routes
Key Challenges How can we train this workforce? What are the best practice models we can utilise to train across geographies? What is the clinical, workforce and financial burden? How could we build on good practice within the Radiology Academies? What is the balance between a need for shortterm investment and a long-term sustainable model? What is the impact of doing nothing?
Key Considerations External Standards/expectations PHE, RCR, RCOG, BMUS, SCoR etc. New service delivery models e.g. Urology pathways, diagnostic centres, etc. Advanced Practice Framework Retention of current staff Implications of grand parenting if regulated
Strands of Work Regulation - advance our position on regulation of the profession. Scope of practice to clarify what direct entrants can do and onwards development needed. Training capacity understand how we can train direct entrants and how best to stimulate the market. How we support employers to receive graduate sonographers into useful service.
Sonography Workforce Sub-Group Summary of work undertaken by the Scope of Practice Workstream 28 September 2017 This pack shows the product of discussions to-date to support this aim. There are two additional workstreams underway to support the safety, education and training needs raised by this work. The regulation workstream is building a case for regulation which assesses the risks around the practice and delivery of sonography. The final workstream is looking at how we can create capacity within the system to support undergraduate clinical training places and the requirements of any preceptorship programme that may arise as a result of this work.

The Vision Being responsive to employer need Agreeing innovative solutions and pathways to achieve these Direct entry to undergraduate degree/ undergraduate degree apprenticeship Continued training, including preceptorship, to advanced clinical practice and beyond Addressing clinical placement capacity Registration/regulatory challenges mitigating the risks
Next Steps Regulation Subgroup Case to DoH by Dec 2017 If progressed scheduled parliamentary time April 2018 Stakeholder event HEI Education development CASE develop preceptor learning outcomes Learning outcomes for emerging undergrad education
