2 1 Palliative care in Ecuador: a research proposal Title: Development of an effective model of palliative care in Ecuador, based on a critical analysis of actual needs and means. Aim: This project intends to improve quality and efficiency of palliative care in Loja, Ecuador
3 2 Summary Critical analysis of local situation about PC - inclusive needs and efficiency of care - different models of good PC - both quantitative and qualitative research methods used Proposal of an effective model of PC (consensus methodology) Development and testing of some tools on their efficiency within the local context: an educational model, procedures, guidelines for palliative care and a palliative care pathway.

5 4 Aim 1. To study the actual situation of the care of palliative patients in Loja, Ecuador: - what is the actual situation of palliative care (supply and demand) in Loja, Ecuador? - which most frequent pathologies and symptoms need palliative attention and care? - what are influencing factors to the diagnosis of incurable diseases for the person, his/her family and the society?
6 5 Aim 2. To make a scientific analysis of different models intending to reach the WHOobjectives for good palliative care - what is the most effective model for implementation of palliative care, regarding the context of Ecuador? - is it possible to create a palliative care model/unit in Loja, Ecuador (feasibility)?
7 6 Aim 3. To create some tools helping best palliative care practices - how effective is an educational model on the competence of physicians concerning palliative care? - is it feasible to develop and implement procedures, guidelines and a palliative care pathway within the context of Loja,Ecuador?
8 7 Phases 1. Critical quantitative and qualitative scientific analysis of the actual situation: -- a. description of the needs of patients with incurable diseases, b. description of the actual possibilities for care of these patients, analysis of the impact on and the role of the (extended) family, of the cultural factors influencing care. c. descriptive study with quantitative registration and interviews with palliative patients, their family and health care providers.

Swot-analysis of strengths and weaknesses of the actual provisions and facilities for palliative care in Loja (here the role of important stakeholders as for example the management of hospitals, organisations for cancer and 8 Phases 2. This search should lead to the proposal of the most appropriate model(s) for good palliative care in Loja. By a nominal group method and/or Delphi procedure the elements for the palliative care model to be implemented in Loja are determined.

Design and analysis of an educational model: education is an extremely important aspect of palliative care (2): by means of a pre- and post-test the impact of such an educational model will be tested (3)...
