PROFESSIONAL ACTIVITIES: Clinical Social Work Experience: Rehabilitation Institute of Chicago, an affiliate of Northwestern University Medical Center, Chicago, IL. During the eight year tenure at the Rehabilitation Institute of Chicago the following positions were held: Manager of Systems Case Management Services Designed and implemented a case management program for following discharge of the Brain Injured and Spinal Cord Injured Patients. Project Support Specialist Part of a three member re-engineering team. Coordinated and facilitated work groups and design teams to ensure timely completion of business process re-engineering efforts and provided ongoing assistance to leaders and staff in change management. Director of Social Work/Early Intervention Responsible for planning, developing, implementing and evaluating the Institute s social work clinical and administrative programs for the inpatient and outpatient population. Responsible for the organization and management of the early intervention program for 0-3 year olds. Clinical Manager of Social Work Managed the clinical operations of the Social Work Department consisting of 15 social workers and 2 supervisors. Responsible for the hiring of all new social workers, budgets, and policy and procedures. Clinical Supervisor of Social Work Supervised social workers on the Head Trauma and Spinal Cord Injury units and provided supervision and consultation to social workers on patient/family dynamics and patient/family therapy groups. Coordinated the Social Work Student Program. Clinical Social Worker 1991 Assessed the psycho-social needs of patients and families on the Head trauma unit. Provided discharge planning, individual and family counseling, and support groups for caregivers Michael Reese Hospital Outpatient Alzheimer s Clinic, Chicago, IL.

3 Clinical Social Worker Assessed families and provided ongoing counseling to caregivers of patients with dementia. Facilitated a support group for caregivers. Analyzed research data from the center s registry on dementia Rush Alzheimer s Disease Center at Rush Presbyterian-St. Luke s Medical Center, Chicago, IL. Clinical Social Worker and Researcher Provided psycho-social assessments and interventions for families caring for persons with Alzheimer s Disease and related disorders. Member of an interdisciplinary research team, collecting and analyzing data on functional and psycho-social issues of patients and caregivers The Johnston R.
