Cost Effectiveness of a Statewide Falls Prevention Program: Healthy Steps for Older Adults, Pennsylvania. Am J Managed Care, 2 nd round. Synopsis of research, including reference to publications and grants in above lists My primary research passion involves improving the quality of life of older adults by investigating thoughts and actions that could decisively determine whether one is healthy or ill, regardless of clinical status. Over the past several years, I have focused on the questions of how patients cognitive perceptions and emotional reactions to the challenges of osteoporosis may influence their health behavior and ultimately their health-related quality of life (HRQoL). More specifically, I am interested in gender-based health-related variables. Near to nothing is known about this specific subject among osteoporosis patients. I am the first researcher (worldwide) to use the Self-Regulation Model (SRM) as a theoretical framework to investigate the ways in which osteoporotic patients perceive their illness (referenced article #1) and emotionally react to it (referenced articles #1, 4). Prior to this study, I focused on gaps in knowledge about osteoporosis among osteoporotic men and associations with health behaviors (referenced article #3). Osteoporosis is considered to be a women s disease; as such, data on men is scarce. My studies highlighted significant differences in the manners in which women and men cognitively perceive and emotionally react to osteoporosis, as well as suggested directions for gender-based interventions. I will soon submit an additional paper on an integrative model for predicting health-related quality of life among osteoporosis patients. Bridging the gaps in knowledge found among osteoporotic men (referenced article #3) is considered crucial to the promotion of adequate participation in health behaviors and will be addressed in future intervention plans. I believe that older adults' quality of life should be promoted by a multimultidisciplinary approach. In order to deepen and broaden my expertise in quality of life promotion, I joined an Australian research group and also pursued my post-doctoral studies at the University of Pittsburgh Graduate School of Public Health. Findings of the Australian study (referenced article #2) revealed that older adults were aware of the benefits of training on general and functional health, and that these benefits were employed in the motivation to train. The benefits to mental and social health, as well as the effects on aging and body image were also raised. As this study provides health professionals with valuable insights on physical activity motivators among older adults, I intend to replicate this study in Israel and to examine it using a cross-cultural perspective. My post-doctoral visit had three main purposes: a) to better understand the public health social workers' perspective on older adults' health promotion; b) to strengthen my analytical skills by using data originated from large sample studies; and c) to establish sources for future cooperation with highly-ranked researchers. Findings of this study revealed that older adults living in less urbanized areas are more likely to be prescribed psychotropic drugs. This difference may indicate a health disparity based on access to geriatric specialists or mental health care (referenced article #5). Moreover, the effectiveness of non-randomized pragmatic design for primary prevention of falls was acknowledged (referenced article #6). Health disparity based on access to geriatric specialists or mental health care also exists in Israel, but prescribing of psychotropic medications for the elderly has yet to be investigated based on geographical perspective. Given the well-known hazards of excessive psychotropic medication use in older adults, in
7 Offer E. Edelstein page 7 my opinion it is very important to evaluate this in Israel and compare it to Western world data. Use of pragmatic design for this purpose will suit well the challenges of the current Israeli and worldwide economic situation. My post-doctoral visit allowed me to establish contacts with potential partners for future collaborative research with highly ranked researchers in the USA. Lastly, upon my return to Israel, I joined a research group on burden of care among minorities in Israel. Using a quantitative approach, we were the first researchers who sought: a) to explore the profile of ultra-orthodox caregivers and their burden of care; and b) to evaluate associations between various caregivers' characteristics, care recipients' characteristics, and burden of care by care situation. In an additional study, we qualitatively addressed the meaning of burden of care among these populations and revealed the unique manners in which ultra-orthodox perceive the caregiving situation. Two papers have been submitted recently. (referenced article #1, 2 submitted section). Plans: I plan to devote the upcoming years to the following academic activities: Research related to chronic diseases, coping and health behaviors, cognitive perceptions and emotional regulation, HRQoL, and gender-based differences. Research related to trends of informal caregiving among minorities in Israel (ultra- Orthodox Jewish and Arab populations). Fall prevention among the elderly (the topic of my post-doctoral fellowship): Implementation and evaluation of a short-term, low-cost, and low-intensity educational intervention for falls prevention in Israel. Alternative therapies for dementia patients: reduction of psychotropic medications (grant writing is in progress, with Prof. Steven M. Albert, University of Pittsburgh). Pain assessment and reduction of agitation (grant writing is in progress, with Prof. Steven M. Albert, University of Pittsburgh). Art and caregivers of dementia patients: burden of care moderators (grant writing is in progress, with Prof. Efrat Huss, Ben-Gurion University of the Negev).
