Medical Protection Society. Experience in Anaesthesia. I began training in Anaesthesia at The Royal London Hospital with excellent direct Consultant-led teaching, after which I joined the busy on-call rota, which gave me a thorough grounding in basic anaesthesia. I trained in Obstetric Anaesthesia at the Homerton Hospital which included the management of a large proportion of high risk obstetric cases, including pre-eclampsia, diabetes and obesity. I became proficient in 10
regional and peripheral nerve block techniques including femoral, sciatic, lumbar and brachial plexus blocks, during my attachments at St. Bartholomews and Homerton Hospitals. At Charing Cross Hospital I trained in ENT surgery, neurosurgery and vascular surgery, including the anaesthetic management of major emergency and elective cases such as intraabdominal and intracerebral aneurysm repair. I then went to The Royal Marsden Hospital where I was involved in major oncological surgery, including head and neck, gynaecology and plastic surgery. I became proficient in awake fibreoptic intubation and regularly anaesthetised children under 5 years old for oncological procedures. I was the most senior trainee and administration SpR at Hemel Hempstead Hospital which gave me excellent general experience in a busy DGH, including organisation of personnel and the management of major emergency and elective vascular surgery. Chelsea and Westminster Hospital gave me extra training in plastic surgery and paediatrics, including general and ENT surgery. I have gained further experience in cardiac anaesthesia and ENT anaesthesia as an SpR 5 at St. Marys Hospital. Since August 2004 I have been working as a Locum Consultant at St. Marys Hospital which I have found a very challenging and useful experience, that has stretched my abilities as an anaesthetic doctor, in particular managing complex, sick patients, paediatric surgery and working at a senior level within theatre and anaesthesia teams. 11
Experience in Pain Management. St. Marys Hospital (2 days per week) August 2004 present Royal Marsden Hospital (2 days per week for 8 months). May 2003 December
Chelsea and Westminster Hospital (3 month pain block). November April 2003 Hemel Hempstead Hospitals (1 to 2 days per week pain for 6 months). May October 2002 Royal Marsden Hospital, London (1 to 2 days per week pain for 6 months). November April 2002 Charing Cross Hospital, London (3 month pain block). November February 2001 Central Middlesex Hospital, Middlesex (Half day per week for 1 year). November November 2000 St. Bartholomews Hospital, London, EC1 (1-2 days per week for 3 months). August October 1998 Experience in Pain Management. I became interested in pain management during an SHO attachment to the pain clinic at St. Bartholomews Hospital in 1998 and was encouraged to attend the clinics and procedural sessions by Dr. Jane Gallagher and Dr. Jim Foster. This gave me a useful introduction to pain management and the practice of basic pain procedures, including epidurals, facet joint blocks, lumbar sympathetic blocks and intravenous regional 13
sympathetic blockade. As a Specialist Registrar I attended general pain clinics with Dr. Davies at Central Middlesex Hospital in In 2000, I was offered 3 months of pain training with Dr. Peter Evans at Charing Cross Hospital. I was allowed to see patients myself in the clinic and performed pain management procedures under direct supervision. I was also introduced to the practice of acupuncture in a variety of conditions, particularly painful musculoskeletal disorders. I attended weekly cancer pain clinics at The Royal Marsden Hospital in 2001 with Dr. John Williams, which gave me an insight into the management of pain in this particular patient group. I also worked under Dr. Jacqueline Filshie, an expert in acupuncture and cancer pain, in her clinics and co-wrote two publications with her. In 2002 I worked with Dr. Paul Hart at St. Albans and Hemel Hempstead Hospitals. This was particularly useful as there is a multidisciplinary pain management team, comprising: clinical psychologist, physiotherapist, occupational therapist, pain nurse and pain clinician. I was able to follow patients through a multidisciplinary outpatient pain management programme. I participated in a large audit project of postoperative pain of over 400 patients which was presented as posters at the Anaesthetic Research Society (2002) and The Pain Society Annual Scientific Meeting (2003). I completed a further 3 months of pain training with Dr. Glynn Towlerton at 14
Chelsea and Westminster Hospital in 2002, seeing patients in clinic and doing supervised procedures including being taught stellate ganglion and suprascapular blocks. I conducted 3 to 4 acute pain rounds per week and attended several palliative care clinics with Dr. Cox. I was also fortunate to be given an introduction to basic sciences research in pain by Dr. Andrew Rice, the senior lecturer in pain research at Imperial College. I then worked at The Royal Marsden Hospital where my clinical commitments included intensive care (1 in 5 rota) and general/ paediatric anaesthesia. I attended a weekly pain and palliative care meeting, followed by a cancer pain clinic with Dr. John Williams. I conducted acute pain rounds three times per week, with the pain sister, Anne Chandler. I was part sponsored by NAPP pharmaceutical company to review the role of controlled release oxycodone in postoperative pain and pre-emptive analgesia. I presented two posters at The Pain Society ASM 2004 in Manchester with Dr. John Williams. I was appointed Locum Consultant in Anaesthesia and Pain Management at St. Marys Hospital, from August This has given me the opportunity to work at consultant level in the multidisciplinary pain management team at St. Marys Hospital, where I have encountered some very challenging patients. Dr. Michael Platt and Dr. Jane Shepherd have allowed me to asses and manage patients myself, and are always available to discuss problems. They have welcomed me to the department and encouraged me to participate 15
in other, non-clinical activities, including audit and teaching. Experience in Intensive Care Medicine. I commenced training in Intensive Care Medicine as an SHO at the Homerton Hospital with a 3 month block. This is a 6 bed unit in a busy DGH which gave me a solid grounding in the assessment and management of a variety of critically ill patients. I was also on-call for Intensive Care, accepting and managing patients via the Accident and 16
Emergency department, with indirect consultant supervision. I then worked for 3 months at The Royal London Hospital which included the early assessment and management of critically injured patients, retrieved by the Helicopter Emergency Medical Service (HEMS). I became proficient in ATLS, including chest drain insertion. I then worked on a general ITU rota for a year at Central Middlesex Hospital, as an SpR 1, admitting acutely ill patients and managing a six bed unit. I completed another 3 month block as an SpR 2 at Charing Cross Hospital with Dr. Mark Palazzo, which included the management of major elective and emergency postop vascular and neurosurgery cases, including emergency intra-abdominal and intracerebral aneurysm repair.as an SpR 3 I was on the on-call rota at The Royal Marsden Hospital which encompassed the management of patients following major oncological surgery and critically ill medical oncology patients. Following this I did a 3 month block at Chelsea and Westminster with Dr. Neil Soni which incorporated a regional burns Intensive Care unit. I became proficient in tracheostomy insertion and the insertion of chest, pleural and abdominal drains. Management Experience. Elected Trainee representative for Anaesthesia, North West Thames Specialist Training Committee. February December 2005 Member of British Association of Medical Managers (BAMM.) June 2002 June

I taught SHO s at Central Middlesex and Hemel Hempstead Hospitals by organising multiple choice, OSCE and viva voce 19
sessions, for Primary FRCA candidates. At the Chelsea and Westminster Hospital I taught medical students basic skills in anaesthesia and did viva voce practice for two Primary FRCA candidates. In June 2002 I was an examiner in mock vivas on the St. Bartholomews/ Whittington Hospital Final FRCA course. I have recently been involved in mock vivas for Final FRCA candidates at St. Marys Hospital. Research. Jenner C.A.
