2 Massachusetts General Hospital, Internal Medicine Associates, Boston, MA Investigated resident and patient perspectives on the efficacy and safety behind patient care transitions in resident provider clinics with the goal of modifying the current handoff structure to a safer and patient-centered process. Identified both effective practices as well as areas for improvement from both residents, via survey, as well as patients, via phone interviews. Inpatient Delirium Management Initiative Resident respresentative ( ) Massachusetts General Hospital, Boston, MA Multidisciplinary team consisting of attendings, nurses, and residents across multiple disciplines (medicine, surgery, psychiatry, neurology) tasked to develop a cohesive and consistent approach to initial management of delirium in inpatient setting. Launched CAM initiatives on medical service floors, created guidelines for initial basic pharmacological and non-pharmacological approach to delirium for residents, and launched algorithm for appropriate use of consultants in delirium management. Chart review and analysis of current MOLST form data. Strategizing for initiatives to improve completion rates with geriatric providers. Conducted literature searches and subsequently created study metrics for evaluating degree of socialization. Developed a cognitive health coaching module for older adults geared at increasing socialization by incorporating new technology (Skype and webcams) into older adult participants homes. Presented pilot study results at the 2009 Oregon Geriatrics Society meeting. Coordinated Center s outreach and recruitment efforts, coordinated educational initiatives for lay and professional audiences, and supported developmental research and evaluation projects. Coordinated the PAIRS Program, an educational initiative that matches first-year medical students with early-stage Alzheimer s disease patients; prepared program curriculum and organized launch of program s inaugural year; applied for and secured foundation support for program s operational expenses. Coordinated a cardiac and brain MRI study on the relations between heart and brain aging among older adults with mild cognitive impairment. Assisted a COPD study investigating genetic and environmental risk factors influencing COPD exacerbations.
