Personal Statement I am a physician scientist and hepatologist who has dedicated her career to the development of better care for patients with liver disease. As the Associate Chief of Medicine, Specialty Care and Access, and director of the VA SCAN-ECHO program at the VA Ann Arbor Healthcare System, I am acutely aware of the need for efficient access to specialty care, particularly for rural and underserved Veterans. My research interest in analytic moprhomics, an innovative high-throughput, anatomically indexed methodology for assessing body composition and organ measurements from the data files of CT scans offers an unique opportunity to provide risk assessment in patients with liver disease which can be leveraged for developing novel approaches to specialty consultation. As the single largest medical system in the United States with one of the oldest integrated electronic medical record system and networked storage of radiological imaging, the VA provides a powerful and unique environment for this research proposal. As a Professor of Medicine at the University of Michigan and associate director of the Morphomics Analysis Group, I have had the privilege of working with a multidisciplinary group of scientists from multiple medical specialties and engineers to develop novel applications of analytic morphomics to clinical diseases. I believe that my expertise in translational medicine and in the area of developing analytic morphomic tools makes me ideally qualified to carry out the technical aspects of this proposal relating to this unique technology. To implement this from a health services and research perspective, I am eager to work closely with my -PD/PI, Dr. Sameer Saini, who has extensive experience in health services research. I have worked collaboratively with Sameer for nearly 15 years on ongoing federally-funded research, co-mentoring of trainees and early career faculty, and programmatic initiatives. We plan to meet on a weekly basis during the study period, and our collaboration will be facilitated by a clear delineation of our complementary roles and responsibilities, as detailed in our Multiple PD/PI Leadership Plan. 1. Contribution to Science Development of Analytic Morphomics for the Diagnosis and Prognosis of Liver Disease Using analytic morphomics to examine body composition and organ features in patients with chronic liver disease, I have developed prediction models to assess diagnosis of cirrhosis which outperform standard laboratory based models. Quantitative Detection of Cirrhosis: Towards the Development of Computer Assisted Detection Method, Journal of Dig Imaging 27: , PMCID: PMC Evaluating models of care which leverage technology to provide subspecialty care to remote sites Due to the geographic clustering subspecialists in tertiary care center, access to subspecialty care is a significant problem in many parts of the United States, particularly in rural and underserved areas. To address this, problem, we were funded to implement and evaluate the SCAN-ECHO demonstration project in the VA Ann Arbor Healthcare System. The goal of this project was to create highly innovative approach towards subspecialty care which leveraged technology to virtually deliver care to remote regions. Unlike prior telemedicine initiatives, this project is directed at training primary care providers in distant locations to better care for patients with complex disease through a combination of case based distance learning and patient comanagement. In order to evaluate this program, we developed a liver disease cohort for our VISN using administrative data retrieved through the central data warehouse. By examining this large VA based dataset we found that access to subspecialty care improved mortality in patients with chronic liver disease. In addition, we found that with appropriate risk stratification, a SCAN-ECHO visit was equivalent to a face to face visit (publication submitted). Specialty Care Access Network- Extension of Community Healthcare Outcomes Model Program for Liver Disease Improves Specialty Care Access, Dig D Sci (in press) Identifying factors which determine prognosis of patients with hepatocellular carcinoma (HCC) I have had long standing interest in understanding the factors which determine prognosis in patients with HCC. We were the earliest groups to identify non-alcoholic fatty liver disease (NAFLD) as a risk factor for the development of HCC but it is not yet clear whether body features associated with NAFLD are prognostic in patients with HCC. HCC is different from other cancers in that it develops predominantly in patients with chronic liver disease. Prognosis is dependent not only on cancer features but also on patient features such as functional status and liver function. These latter 2 features are part of the most widely accepted staging system in the Western world, the Barcelona Clinic Liver Clinic (BCLC) Staging system. Our group showed that of all the staging system tested, BCLC was the most prognostic in our population. However, despite it being a relatively good staging system, the accuracy for prognosis was still lacking. In our recent studies, we found that BCLC was not predictive of outcome in patients undergoing transarterial chemoembolization. Rather, our recent studies show that objective measurements using analytic morphomics performed better than BCLC in predicting prognosis of patients with HCC and may potentially be an useful adjunct to assess patients with HCC. We have shown in multiple clinical circumstances that serum based markers may be used to predict presence of disease such as liver fibrosis or the presence of primary liver cancer (hepatocellular carcinoma). These non-invasive approaches are clinically relevant have evolved over time as more standard approaches for clinical care. I was a co-investigator on these studies and my laboratory provided technical support to carry out these assays.
