SEMINAR DIARY FROM FELLOW Diary of Marjana Burmuzoska, MD Clinic of Anesthesiology Skopje, Republic of Macedonia June 2018, Sunday I started this journey at 2 am; I was traveling to the airport in fear of boarding my flight (I hate flying), but very enthusiastic about the final destination. I arrived in Salzburg and I was thrilled by the city while traveling with the bus to the Schloss. I found the place very easily and at the reception desk a young lady warmly welcomed me and my colleagues. I had time until 7 pm so I opened Google maps and it guided me to a lovely park called Volksgarten, with a beautiful fountain and even a rainbow. And it was time to meet my colleagues and the faculty. I was so nervous, because a few days before I was looking at the schedule and a couple of names were familiar to me, apparently from the list of literature books for an EDAIC test. My reaction was like WOW I am meeting the professors who actually wrote those books. The reception dinner was great, 30 people from 28 different countries, selected from more than 200 applicants, and conversations about anesthesiology on every corner. However, the rain surprised us and it was time to rest, so I went to bed full of positive vibes and impressions, completely ignoring the storm outside my window. 18. June 2018, Monday It was already Monday morning. After breakfast, it was time for the lectures. We all gathered in a big wonderful authentic Austrian hall and there was professor Hugh Hemmings, more excited than us I think. Just imagine, it is his 15 th time here. All the lectures were so good, many discussions, many opinions, many questions, many experiences I really liked the mixture of approaches, both American and Austrian, that professor Kane Pryor and professor Klaus Klein presented to us through their topics about neuroanesthesiology. The thing I was excited the most about was the basic cardiac echocardiography lecture by

professor Shanna Hill, and it was great. After lunch, we had the opportunity to listen to some of the fellows case presentations and they were excellent. Finally, in the evening, we had some free time, and we (the Balkan group) went sightseeing in the city. Lots of history on every corner and imagine us taking photos every minute. We sat at a bar to have some beer and it was great hanging out with my colleagues. Time for bed and some rest, to charge with energy for the next day. 19. June 2018, Tuesday It was day 3 and a similar busy schedule as the previous day; lots of lectures and many different topics. It started with a hot one from professor Hemmings about anesthetics and their possible toxicity to the brain. And then with the excellent professor Jennifer Hunter, who I admired a lot for her incredible and convincing way of passing on her great knowledge about patients with kidney disease. This lady, who is actually a fan of The Beatles, was my surprise of the day. After receiving some theoretical knowledge from professor Peter Marhofer, it was time for the ultrasound workshops, which I think we all impatiently awaited. More than 3 hours of practicing, guided by our professors. We were so excited that we even practiced on ourselves. I don t know where I was finding my energy, but afterwards we went to the city for a walk. I loved the river and the Love Locks Bridge. When we got back, we all gathered in the wonderful garden in front of the Schloss, talked and smiled for a while in the night. 20. June 2018, Wednesday Wednesday was the toughest day for me. It was time for my case presentation. Luckily, we had some very interesting lectures in the morning to keep my mind off of the presentations. Professor Pryor was excellent with his views about postoperative delirium. The best part was the video presentation, which taught us how to evaluate our patients. I think we mastered anaphylaxis and neuromuscular blockage in professor Hunter s lectures. And of course we received some interesting facts about anesthesia in orthopedics and trauma by professor Klein. At the end of the lecture it was time for our presentations. I was nervous, I thought my heart was beating like 130/min, but everything went well, I even got some compliments by professor Hunter that meant a lot to me. At 8:30 pm, it was concert time. Three eminent musicians Wonji Kim-Ozim, Matthias Michael Beckmann and Elena Braslavsky, enriched our night with their passionate performance. We continued with a little wine and talks about Vienna and the Balkan with professor Klein and of course about the Royal family with professor Hunter.

21. June 2018, Thursday To Reverse or Not to Reverse started off the day, by the one and only professor Hunter. A little history, lots of study facts and so much of her own touch for the theme, and as always the take home message, to use neuromuscular monitoring every time we apply nondepolarizing NMBA! Next was professor Hill with lot to say about transfusion and as always too little time for a topic like this. I always enjoyed professor Klein s lectures, because he became so close and attached to the group and I think he was trying to help and teach us as much as the time allowed. Finally, an afternoon all for ourselves. We went to Hohensalzburg Festung and the view from up there was breathtaking. When we got back, everyone had a different story to share, but the best one was the visit and swimming in Fuschlsee Lake by Vladimir, Smbat, Rein and Temur. We were very excited to go to the Pub Steinlechner, it was professor Hemmings s favorite place. We had a wonderful time, many stories, lots of beer, laughing and many selfies, as it was International Selfie Day. Certainly, it was a night to remember. 22 June 2018, Friday We all knew that it was our last day of lectures and it was a bit hard. We had a good one by David Baron about patient blood management, with some good take-home messages. Then some obstetrics with Dr. Klaus Klein. I think my favorite lecture during the seminar was professor Pryor s summary of major trials in anesthesiology. I enjoyed his enthusiasm during the presentation. He motivated me to keep myself updated about new research in anesthesiology. After the lunch break we had the Publishing Workshop by the previous and current BJA editors and I think it would be hard to please any of them as an editor but I will keep dreaming that someday I will accomplish it. In the evening it was time for the Farewell Reception and Graduation Dinner. We all enjoyed it so much, I was happy for all my colleagues. We stayed very late in the night talking and talking about everything, even the teachers didn t want this night to end. 23 June 2018, Saturday It was time to end this journey. I was so sad that we were separating and going home. I wanted to stop the time and stay a little bit longer in Scholoss Arenberg. I was on my plane to Vienna looking down on this amazing place and rethinking about every single detail from this week. I was very happy that I got this opportunity to meet all these wonderful people and they for sure motivated me to study more, to explore more, to research more, to keep improving myself and to keep working hard. Until next time, Salzburg, because I m planning to come back Marjana Burmuzoska, MD (Republic of Macedonia)

SEMINAR DIARY FROM FELLOW Jasmina Kovcic, MD, MSc University Clinical Center Tuzla Tuzla, Bosnia and Herzegovina June, Sunday: Sunday morning...i started my journey from Tuzla-Sarajevo by car, and then an easy and nice plane ride to Salzburg through Vienna. The taxi drive from Salzburg airport to Arenberg Schloss showed me a bit of the amazing city Salzburg with amazing parks and green trees, beautiful river and the stunning view of Hochen Salzburg Fortress looking like a guard over the city. I got to the Arenberg Schloss that put me in fairy tale times with a beautiful enterance and park around it. After the reception procedure and room settlement, I took a short and nice walk around the castle and nearby streets with beautiful houses, flowers in the gardens. Salzburg has already amazed me. I had some rest after my walk and was so inpatient to meet new friends and faculty. After the welcome and introduction procedure - the athmosphere was so calm and friendly - few of us decided to have a walk down the streets to have some fresh air and enjoy the Salzburg spirit. After that it was time to sleep to get strength and rest for coming week. 18 June, Monday: Monday morning, raining, a well-done, typical pesimistic Monday morning. I hardly woke up at 6 o'clock. But here we go, so many reasons to be here. After breakfast and chats with collegues, we finally reached the lecture hall, walked up the stairs with a view of the ceiling crystall chandelier, and entered the gorgeous lecture hall with wooden ceilings and photos of musicians, real Austrian spirit all around. The pre-seminar test was very controversial: too many questions that we were usually speaking about that gave multiple answers, but now we need to put only one!!! Well, that is why we are here to learn. There was a great introduction of the faculty members by Dr. Hugh Hemmings Jr. Then he started the first lecture about synaptic pharmacology of general anesthesia with so many details and lab findings through so many studies and was so interesting from that point of view, like NASAlevel expertise in anesthesiology. Another lecture was by Dr. Kane Pryor about neurophysiology and applied neuroprotection, followed by practical advice and advanced

science points. Then we had a presentation by Dr. Klaus Klein about Traumatic Brain injury with useful facts and new up-to-date data that was a pleasure to listen to and easy to understand. Then we had lunch and the following lecture by Dr. Hill, which for me as a cardiac anetshesiologist was very interesting and usefull, was about basics in cardiac echo. It was a really simple and interesting way to explain the basics and important things related to the topic. Then we had 12 great case presentations from different countries, voluntary ones. So many interesting cases and new things to think about in regards to the after effects of anesthesia. Later that day, we (balkan ex yu-group) decided to go for a walk to the historical part of Salzburg. We had such a remarkable walk along the river and trees and beautiful houses, wondering who about the life living inside them. Of course we took so many photos at every corner, every bridge, every monument. We passed by shops with Mozart sweets (famous Mozart kugeln), Mozart statue, and then finally the house where the great - and one of my favorite composers - Mozart was born. Well, when in Salzburg, it is all about Mozart. The old city of Salzburg is so beatuful with narrow, cobblestoned streets, restaurants, people walking, everything was so perfect, with ancient spirit. We met some friends and had a great night out watching the Football World Cup and drinking some drinks in a cheerful atmosphere. We were all a little bit tired and came back to the hotel to get a dose of good sleep to be ready for the day. 19 June, Tuesday: morning, what nice and sunny weather. I even woke up before my alarm, ready to have a great day full of obtaining new knowledge and fun. Breakfast was great, the first lecture was again by Dr. Hugh Hemmings, an interesting one about neurotoxicity of anesthetics, which was very nice and useful and brought me facts that are controversial but worthy of thinking about, yet very important for daily routine and futher aspects of science. Next lecture was by Dr. Marhofer about FAST ultrasound diagnostics, which was really interesting and showed very important facts in daily and emergeny cases that could be very useful to me after I return to my hospital. We continued with ultrasound guided regional anesthesia aspects and prospectives. Lunch outside the castle in a beautiful garden and sounds of many birds... Back to lecture with such an interesting and amazingly interactive lecture by Dr. Jennifer Hunter, spiced with such powerful energy. The theme was very interesting, Anesthesia for Patients with Kidney Disease, which I found very useful since I often face so many kidney disease patients undergoing cardiac surgery in my hospital, so this information will be useful in dealing with difficult cases. And then the most interesting and the most useful part of the day, ultrasound workshops. For me (as a cardiac anesthesiologist) the most interesting part was cardiac echo performed by Dr. Hill, who performed such techniques to obtain and assess the heart with ease. Other workshops were interesting and useful with practical issues.

And after such a meaningful day, the female part of the Balkan group decided to go for a walk, coffee, and ice cream time in some cafe downtown. The weather was amazing, real summertime, we tried to find some shops for presents. After that we returned to the Arenberg Schloss to take a walk to the forest for an amazing view and photo, and hung out in the front of the Schloss with the rest of the group, exchanging experiences in daily routines in OR, conference visits, and getting to know each other better by talking about the countries we are coming from. Then I went to sleep to prepare for tomorrows case presentation. 20 June, Wednesday: morning, the middle of the day, such a beautiful and sunny morning, so easy to wake up, fresh and ready for the new lectures and case presentations from the fellows and myself. As usual, I enjoyed breakfast and coffee outside. The day started with such an interesting and amazing lecture by Dr Kane Pryor about post opeative delirum, which I really liked since I often face it in my ICU. New facts are on the way to being applied. Another one with such great spirit from Dr. Jennifer Hunter about anaphylaxis during anesthesia, something that all anesthesiologists can face everyday and any moment. And we started with the case presentations, all of them were so interesting and international, such amazing cases and such competent management of the collegues during the event, and great discussion. In the afternoon we went to see the marvelious Mirabell Palace and garden with such beautiful flowers and parks, I had a great walk and coffee afterwards. And maybe one of the best entertaiment parts was the chamber concert with a great trio playing Mozart and Haydn music, that took me back in time to the composers' time. The night was beautiful outside hanging out with the faculty and fellows until the late night. 21. June, Thursday: started out sunny and warm with lectures, but all of the fellows were looking forward to the free afternoon, we had so many plans and checked the weather forecast. Lectures started with Dr. Jennifer Hunter about reversing or not reversing the muscule relaxants, a really interesting lecture. Dr. Hill continued with transfusion issues, another really great lecture, I particulary liked it as I often deal with coagulopathy disorders in my daily work routine. Then we had emergency echocardiography followed by more case presentations. We used our free afternoon to do some shopping for presents for our loved ones, visited Hohensalzburg Schloss, such an amazing and beautiful experience. After we returned to Arenberg, had dinner and exchanged experiences with the other fellows about how they spent their free afternoon. Some of them went to a nearby lake, I felt a little bit sorry for not going with them. But maybe next time. Thanks to dear Tatiana from Moldova, we have a Whatsapp group to keep in touch after we leave Salzburg and share the experiences. I was in the spirit for the World cup, since Croatia was playing against Argentina and won 3-0. It nightfall outside of Arenberg Schloss and we counitued with joining the faculty in a local pub. We were having such a great time in a great and relaxed atmosphere, laughing and taking selfies (Today is International selfie day as we were informed by Rein from Estonia ) We returned to Arenberg Schloss after midnight and we all got wet from the rain.

22 June, Friday: Rainy morning, last day, kind of sad emotions. Great lectures from Dr. Hill and Dr. Baron about topics I am very interested in - blood management and emergency transfusion - including experiences and protocols that I will definiteley use when I return to Bosnia. Dr. Klein coinutinued with anesthesia for labor and delivery, what a really great lecture like yesterday's lecture about pospartum haemorrhages. An interesting lecture and workshop was given by Dr. Hunter and Dr. Hemmings about publishing and editor-pleasing. We all waited to see how we will do on the test and evauluated the attained knowledge during the seminar. The test was great and results too. Finally, time to have the farewell reception and graduation dinner. It was great despite the solemnity and awards and awareness that we have come to the end of the seminar. It was a little bit sad and emotional. For this short period we became attached to each other. Wolfgang Aulitzky held his final speech, congratulated us on our successful and fruitful training, which was confirmed by the post-seminar test results. He handed out the certificates, then we took photos with the faculty and had a great time with the whole group, spending our last night in Salzburg in an amazing and fun atmosphere. 23. June, Saturday: The day of departures. We said goodbye to each other and hoped to see each other again somewhere, sometime. I looked back at the last week and felt mixed feelings, I was very happy because I found new friends, I have learnt a lot, and yet at the same time I was very sad because the seminar was over. But I hope, in the future, I ll have the opportunity to attend many new interesting and useful seminars like this one. In conclusion, I want to express a great gratitude to everyone involved in the organization of this perfect seminar, it was a pleasure to be a part of it. Thank you so much!
