7 DEPARTMENT OF OTOLARYNGOLOGY HEAD AND NECK SURGERY Mission Statement The Mission of the Department of Otolaryngology Head and Neck Surgery, College of Medicine, Faculty of Health Sciences, University of Manitoba is to provide professional leadership in all aspects of Otolaryngology Head and Neck Surgery through its teaching, research, and by providing exemplary clinical care. Clinical Care The clinical mission of the Department: Provide Manitobans with compassionate, evidence based care with the best possible clinical outcomes in the clinical areas of general otolaryngology, neurotology, pediatric otolaryngology, rhinology, communicative disorders; sleep disordered breathing, facial plastic and reconstructive surgery, and head and neck oncology. Play an active role in health care management and advocacy as they pertain to the specialty. Provide care to remote communities in Northern Manitoba and Nunavut through the Northern Medical Unit. Education The general educational mission of the Department: - Provide fundamental skills and a basic understanding of otolaryngology to undergraduates, in-depth training of postgraduates, tailored education for allied health care professionals, and an environment of continuing medical education in Otolaryngology Head and Neck Surgery. The primary undergraduate educational goals: - Provide fundamental knowledge and skills to medical students such that they are prepared to enter postgraduate training in the field of their choice and to introduce them to Otolaryngology Head and Neck Surgery as a potential career. The primary postgraduate educational goals: - Provide residents with broadly based, in-depth training that will enable them to enter general Otolaryngologic practice, or enter advanced training in any of the Otolaryngologic subspecialties. The Department provides a safe, collegial, academically stimulating environment for staff and trainees. Educational goals will be accomplished within the framework of the College of Medicine, Faculty of Health Sciences, University of Manitoba mission statement, and the accreditation standards of the Royal College of Physicians and Surgeons of Canada. 6

8 ANNUAL REPORT 2017 Research The research mission of the Department is to produce scientific work that improves outcomes for our patients, improves the educational process for our trainees, and improves the administration and delivery of health care. 7

9 DEPARTMENT OF OTOLARYNGOLOGY HEAD AND NECK SURGERY Department Head Report Dr. P. Kerr I am very pleased to report on our department s activities for The Department of Otolaryngology-Head and Neck Surgery, University of Manitoba continues to provide expert care to Manitobans including hearing restoration through cochlear implantation; management of nasal and sinus disorders with image guided endoscopic surgery; treatment of tumours of the head and neck with minimally invasive techniques such as endoscopic skull base surgery & laser micro-surgery; correction of voice and swallowing disorders using the latest techniques and materials; and advanced multidisciplinary care of pediatric ear nose and throat disorders. We provide leadership to the Medical College, University of Manitoba in advancing medical student education, and continue to train surgeons at the highest levels in a fully approved Royal College of Physicians and Surgeons of Canada residency training program. Our areas of research activity include functional outcomes in head and neck cancer, management of hearing loss and vertigo, and advancement of clinical training through our Robotics and Simulation Laboratory. I d like to extend the warmest of thanks to all my colleagues and our support staff for their outstanding efforts over the past year. Some of the highlights for 2016 include: Clinical Program The Department created a full time administrative directorship and welcomed Mr. Rob Ranson into this position. Mr. Ranson comes with a wealth of experience in process engineering from national consultation work with large corporations, and from running a technology company of his own. Mr. Ranson will be helping the department with projects such as: implementation of electronic records, improving institutional and regional clinical processes, and improving processes pertaining to the academic mission of the Department. Dr. Darnbrough has further developed the voice and swallowing clinic at HSC to include early endoscopic vocal cord injection in the management of vocal cord immobility Dr. Darren Leitao has stepped down as Section Head, Otolaryngology-Head and Neck Surgery. Under Dr. Leitao s many years of leadership, Pediatric Otolaryngology- Head and Neck Surgery evolved to take on the full breadth of clinical activity for this subspecialty, establishing advanced programs such as cochlear implantation, and multidisciplinary care of complex airway pathology and neck tumours. The Department extends sincere thanks to Dr. Leitao as he moves into the portfolio of Undergraduate Medical Director, and extends a welcome to Dr. Jodi Jones who will take over as Pediatric Section Head. The group of Drs. Barker, Chan and Gall continue to provide the core of expert general otolaryngologic care in the academic setting, also including some subspecialty exposure the residents in cosmetics, rhinology and laryngology. 8

10 ANNUAL REPORT 2017 Dr. Jodi Jones has commenced endoscopic salivary gland surgery in children and adults, making Winnipeg one of the earliest adopters of this technique in North America. Postgraduate Medical Education Drs. Sutherland & Osler have actively been participating in the development of the RCPSC Competency Based Medical Education model for education which will be rolled out in Our Department welcomed its second Head and Neck Surgical Oncology Fellow, Dr. Majed Aldosari, a McGill graduate. Dr. Gall has teamed up with Plastic Surgery to expand the annual Resident Sinus Course to include formal surgical anatomy sessions for Plastic Surgery and OtoHNS residents that follows the Sinus Course. Undergraduate Medical Education Dr. Adrian Gooi continued to provide leadership within the Department, the College of Medicine, and the Canadian Society of Otolaryngology - Head and Neck Surgery, advancing the education of medical students. He continues to be recognized by the students as one of the top educators in the College. Unfortunately, Dr. Gooi relocated to B.C. to be closer to family at the end of the academic year. We will sorely miss Dr. Gooi s always bright demeanor and outstanding ability to relate to students! Dr. Leitao has assumed the Undergraduate Directorship being vacated by Dr. Gooi. Research Dr. Brian Blakley continues to serve as our Director of Research. Our primary areas of focus remain hearing loss, functional outcomes after management of H&N cancer, and robotics and simulation. Dr. Jordan Hochman continues to publish and establish inter-institutional collaboration with his Robotics and Simulation Laboratory at the HSC. The Oncology team continues to publish on functional outcomes, and head and neck endocrinology, representing the department in the annual Poliquin Competition at the CSO-HNS. Finally, a special mention for Dr. G. Osler who began a campaign to become the CMA president elect in 2017 in the fall. At the time of this writing, again, heartfelt thanks to all of those who have helped advance the Department over the past year! 9

11 DEPARTMENT OF OTOLARYNGOLOGY HEAD AND NECK SURGERY Educational Reports Postgraduate Director Report Dr. D. Sutherland Otolaryngology continues to work towards being in the first cohort of Royal College programs to transition to Competency Based Medical Education. Meetings of the Program Directors along with the Nucleus Committee Members were held at The Royal College in Ottawa. Locally we have held Rounds and meetings to discuss implementation of the program. Otolaryngology Resident Research day was held on May 12/13, with Dr David Schuler from Ohio State University as Visiting Professor. A full day of local research was presented. Three residents wrote the Royal College Otolaryngology-Head and Neck Surgery exams this spring. Dr D. Khalil is pursuing a Head and Neck Surgery Fellowship in Ottawa, Dr A. Esmail is pursuing a Facial Plastics Fellowship in Ohio, and Dr D. Wong has joined a Community ENT practice in Nanaimo, British Columbia. Two new residents will join us this July Dr Shubhi Singh joins us from Dalhousie University, and Dr. Enzi Alenezi joins us from Ireland where he completed his medical training. 10

12 ANNUAL REPORT 2017 Undergraduate Director Report Dr. Adrian Gooi Education Transition to new UGME curriculum, now have had all 4 years in new curriculum Incorporation of ENT into Clerkship Academic Half-days with very positive feedback, however, this might be removed in the future with the new curriculum. ENT face-to-face time continues to be reduces, thus requiring different educational delivery methods Full use of flipped classroom and interactive learning in instructional planning. ENT continues to be a leader in innovation and online learning in UGME Team Lead for tracheostomy education for American Academy of Otolaryngology Head & Neck Surgery Innovation Examined student and faculty attitudes towards live-streaming Examined best modality for assigned studies Interprofessional initiatives with education sessions for Rural Family Medicine, Urban Family Medicine, Northern Family Medicine, Emergency Medicine, Dentistry, and internal Medicine. Very positive feedback on all sessions. Examined Imposter Syndrome in medical students and residents Scholarship Published and presented on e-learning, mobile audience response systems in CME, live-streaming, audience response systems in medical school 11

13 DEPARTMENT OF OTOLARYNGOLOGY HEAD AND NECK SURGERY Transition of UGME Director role to Dr. Darren Leitao This year, we said farewell to Dr. Adrian Gooi, our previous UGME ENT director, and announced Dr. Darren Leitao as the new UGME ENT Director. We wish Dr. Gooi best wishes in his bright future in academic medicine. This year, the ENT department has continued the medical education advances that were introduced over the past few years. Didactic lectures are now streamlined and online. Instructors continue to utilize Top Hat audience response systems to maximize student interaction and engagement in the materials. We have continued the small group MCQ development sessions, encouraging students to delve deeper into the material. The ENT component of undergraduate medical education includes clinical simulation learning opportunities, and takes a hands-on approach. Students interact with computer simulations of the ear, nose and throat, and are exposed to various general and specialized equipment, from nasal packing instruments, to Otoscopes, flexible endoscopy, and tracheostomy. This year, the stations were modified and updated, to provide more comprehensive skill development and checklists for the students. We continue to see a small, steady group of medical students rotate through the ENT department on electives, including both local and out-of-province medical students. Through feedback received from these students, we are actively improving the electives to provide the most diverse experience, while still providing consistency required for evaluations. 12

14 ANNUAL REPORT 2017 Research Director Report Dr. Brian Blakley The Department of Otolaryngology, has initiated some changes to the research program intended to improve research output. The main purpose of the residency training program is to train highly competent clinicians, but part of this training requires the development of sophistication about research and interpretation of new findings on behalf of patients. Changes to the Research program has focused on residents. These include increased; 1. expectations for faculty to be involved and more actively supervise research, 2. allowable block time for residents to perform research up to 3 months, 3. requirement for resident presentations four times during the residency, 4. potential funding for projects from Departmental sources, 5. didactic presentations about research. Ongoing research collaborations with the Engineering Faculty are stronger with several projects ongoing. Otolaryngology involvement in graduate student supervision is increasing with participation in the Biomedical Engineering (BME) program of the University. 13

15 DEPARTMENT OF OTOLARYNGOLOGY HEAD AND NECK SURGERY Educational Reports Director of Continuing Professional Development Dr. Jodi Jones After taking a break for the Annual CSO meeting in Winnipeg in 2015, we held the 3 rd Prairie Otolaryngology Weekend (POW) at Elkhorn Resort, August 19-21, Once again, this conference was fully accredited by the Royal College of Surgeons of Canada. The meeting featured great talks from several of our colleagues in Saskatchewan as well as Dr. Chrystyna Kalicinsky, local allergist and Dr. Paul Kerr, local Head & Neck Surgeon. The meeting was well attended and was a great success. As the next CSO Annual Meeting will be held in Saskatoon, we will not be holding the POW in Plans are underway to host 2018 POW in Saskatchewan. The Department of CPD in the Rady Faculty of Health Sciences underwent accreditation review in As a member of the Advisory Committee, I participated in the accreditation process. We are happy to announce that the Department received full accreditation by the reviewers with several exemplary areas. In 2017, new University of Manitoba policies for interactions with industry will be introduced. Some significant changes include that CPD events supported by industry can no longer be held in restaurants. In addition, physicians are to assume costs for food and beverage being provided at non-accredited events. Registration fees must be charged for accredited events which are supported by industry at a rate of $25.00 per hour of accredited learning. 14

16 ANNUAL REPORT 2017 St. Boniface Hospital Section Head Report Dr. F. Gigi Osler The Section of Otolaryngology-Head and Neck Surgery (Oto- HNS) at St. Boniface Hospital (SBH) provides care to patients in the emergency department, outpatient clinics, and inpatient units. There were over 4,000 patient visits to the Oto-HNS outpatient clinics and over 1,100 patients treated in the main operating rooms for Oto-HNS procedures. SBH has the province s only CT-navigated sinus surgery system and the rotation continues to provide outstanding Rhinology teaching to the residents under the expert guidance of Drs. Richard Gall, Eric Meen and Gigi Osler. Dr. Mark Barker continues to provide the residency program with Laryngology teaching and clinical expertise. Drs. Kerr, Sutherland and Viallet continue to provide excellent head and neck surgical oncology teaching with many of Manitoba s head and neck surgical oncology procedures performed at SBH. The hospital was first established in 1871 and has a long tradition of excellence and caring. SBH strives to provide Manitobans with the highest quality health care possible by anticipating and responding to community needs. As a leader in patient care, research, and education, the hospital maintains the highest ethical standards, respects life and human dignity, and is committed to the populations it serves. St. Boniface Hospital is a tertiary care facility, a teaching hospital, and the second busiest hospital in Winnipeg. The hospital campus is comprised of more than 180 departments and services, which are located in seven separate buildings and situated on 20 acres of land. It also operates several satellite facilities and maintains close relationships with other health facilities as well as the health agencies of the Grey Nuns network. 15

17 DEPARTMENT OF OTOLARYNGOLOGY HEAD AND NECK SURGERY Surgical Hearing Implant Program Medical Co-Director s Dr. D. Leitao Dr. Jordon Hochman The Pediatric Cochlear Implant Program continues to provide high quality, timely care to children with deafness. The number of children receiving cochlear implants continues to increase. In 2016, 10 cochlear implantations were performed in children. The Cochlear Implant Program held a full day symposium for audiologists on cochlear implant technology. Dr. Darren Leitao led the symposium, and presented on cochlear implant surgery, and pediatric cochlear implant candidacy. Attendees were provided with hands on opportunities with cochlear implant devices, and the event was well attended. Manitoba introduced Universal Newborn Hearing Screening in September Our program is excited about what this means for improving the hearing health for children. We anticipate that this program will enable us to identify children who may benefit from cochlear implants at an even earlier age, which will improve the hearing and speech outcomes for these children long term. We expect to see further increases in children receiving cochlear implantation in the next 2-3 years. Hearing Implant Program Summary Dr, Jordan Hochman 2016 has been a highly progressive year for the Surgical Hearing Implant Program. We continue to address the needs of patients who can benefit from implantable hearing solutions. We continue our mandate of furthering hearing related research with a substantial impact in simulation. Most significantly the WRHA and Health Sciences Centre are supporting increased implant activity in attempt to address patient wait times. We are thrilled and thankful for the endorsement. The Cochlear Implant Program completed 31 surgical procedures on 22 adult and 10 pediatric patients in 2016 implanting a total of 33 devices. There are currently 48 patients who have passed audiological candidacy criteria for surgery and are awaiting consultation for surgery, consistent with the previous year. The program continues to generate an approximate average of 3 new surgical candidates per month. The average waiting time for adult patients is now 19.8 months, which is an increase of 4.6 months from the previous year. The Cochlear Implant program has a caseload of approximately 177 active participants currently implanted and approximately 48 patients awaiting surgery. The program has a 44% binaural implant rate for pediatric patients and <1% for adult patients. There is no pediatric waiting list; however average pre-operative processing time (from diagnosis to surgery) is currently 7.1 months. 16

20 ANNUAL REPORT 2017 Children s Hospital Section Head, Pediatric Otolaryngology Dr. D. Leitao Dr. J. Jones This year marked a transition in leadership within the Pediatric Otolaryngology section. After 12 years in the position, Dr. Darren Leitao has stepped down, and we are pleased to announce Dr. Jodi Jones as the new Section Head. Dr. Jones completed her fellowship training in 2009, and has been an active member of the department both clinically and academically, for the past 8 years. Key highlights this year have been the departure of Dr. Adrian Gooi, which reduced our active otolaryngologists within the pediatric ENT section to six surgeons. The reduction in surgeons this year has resulted in a spike in wait times for general Peds ENT consultations and surgery. Wait times for specialized Peds ENT consultations, such as deafness, airway concerns, or neck masses, continue to be expedited. The Pediatric ENT section has secured new equipment for our clinical and surgical programs. New microscopes with high definition camera technology and video monitors have been acquired for the operating room and clinical environments. Specialized otologic surgical equipment has been obtained to advance our department s experience and expertise in endoscopic ear surgery. New laryngoscopes have been obtained to allow advanced endoscopic airway surgery in children. The Pediatric ENT clinic at Children s Hospital expects to see an increase in consultation volume resulting from the recent introduction of the Universal Newborn Hearing Screening Program. In December 2016, Dr. Darren Leitao and Dr. Jodi Jones coordinated Manitoba s first EXIT procedure (ex-utero intra-partum treatment), along with Dr. Carol Sanders, (obstetrics and gynecology). This highly complex procedure was performed for management of critical fetal airway compromise, and involved collaboration with teams from Peds ENT, Obstetrics, Neonatology, and Anesthesia. The procedure was successful, and has laid the groundwork for further collaborations with these services for highly complex cases. 19
