10 Community Health The Community Health Department s focus is health promotion/education and disease prevention. The department s goals are to reduce health disparities through partnerships, outreach, education, prevention and capacity-building. 11 Great Plains Good Health and Wellness (GPGHW) Program CDC created A Comprehensive Approach to Good Health and Wellness in Indian Country as a more coordinated and holistic approach to chronic disease prevention and health promotion in tribal populations Reduce the prevalence of obesity and reduce the rates of death and disability due to diabetes, heart disease and stroke In 2014, GPTCHB was one of eleven tribal organizations to be awarded this five year cooperative agreement

"I think they're all awesome I just, I I love, I love the staff that works there, they're like so easy to talk to, and, it seems like, any time I have a question, they they always have an answer, they always have, you know, if they don't know then they'll find out for me. So I really like that they're pretty easy to work with. And, at first I didn't use them that much, I was kind of reluctant but then, once I started talking to them more, then they were telling me the things that they can help me with. The resources that I can use, so, I started to use them more and now I'm so happy that, they've helped me out so much.

20 Diabetes Prevention Project 2016 supplemental funding from CDC s Good Health and Wellness in Indian Country Increase number of tribes offering CDC-recognized diabetes prevention program in Great Plains Region from 0 to 2 In partnership with CDC & GPTEC, provide funding, training, and technical assistance to two tribes to strengthen their existing diabetes programs by achieving full CDC recognition Increase knowledge and understanding of the process of establishing, scaling and sustaining the National DPP in Indian Country

21 Community-Clinical Linkages Project 2016 supplemental funding from CDC s Good Health and Wellness in Indian Country Provide funding and technical assistance to three tribes or tribal entities to design and implement a community engagement process Develop and implement strategies to improve or increase community-clinical linkages
