2 Improving Health Outcomes Statewide With support from the United Health Foundation (UHF), ASTHO worked with five state teams to improve health outcomes around diabetes, obesity, infant mortality, and/or smoking through systems-level changes in an effort to improve their America s Health Ranking. 8 Integration of Primary Care and Public Health Problem: Micro-practices need easy access to affordable smoking cessation and weight loss resources in communities where patients live and work Quality Improvement: Identifies root causes of problems Tests solutions Improves outcomes Solution to be tested: Community Health Network Centralized Referral System linking clinical practices with community resources Primary Care Micro-practices: One or two doctors High tech solutions to office management patients Close relationships with patients

12 Georgia Telehealth Highlights: Targeted telemedicine efforts based on health outcomes (barriers to care & rural access gaps) Telehealth capability in all 159 counties USDA funded (network / infrastructure) Significant increase in utilization (training/education) Strong statewide coordination (partnerships with private/public) Eight telemedicine carts successfully deployed On average, approximately 160 patients are currently being served, per month via Telemedicine, statewide Nine additional carts to be deployed by Dec Mobile equipment for home health visits or disaster response Over $1 million secured in grant funding Strong grant applications which show collaboration (Teledentistry, HRSA, Georgia Technology Authority, ASTHO, Public Health Preparedness, State Programmatic funding)

14 Kansas Learning Collaborative Improving Obesity and Diabetes UHF Rankings through a Physical Activity Champions Network Objective: recruit and support volunteer champions in every Kansas county and sovereign nation to: Promote physical activity in their respective community; Serve as a liaison to the Governor s Council on Fitness in implementing a statewide Get Active Kansas campaign; Serve as a local spokesperson on behalf of the GCOF; Connect community leaders committed to increasing opportunities for PA in community settings; Assist the GCOF in collecting information.

15 Top Successes Successes: 116 PA Champions in 63 counties and one tribe PA Champion Toolkit developed and disseminated Active Transport and Shared Use Surveys collected from 29% of represented counties (18 out of 63) within 3 weeks High level of social media use-387 likes, 110 posts on Facebook Grant award received from the Kansas Health Foundation to sustain and grow the project over the next two years with a focus on active school transport
