7 Near Miss Accidents in Construction - Bob Boyd Key Points Investigate all near miss incidents. A near miss not reported is the next accident. Document separately all near misses; they are an opportunity to learn from and to prevent an incident/accident. Bob outlined a near miss incident involving a MEWP, which had occurred on one of their sites Primark, Birmingham Pavilions. Although a method statement had been produced, the ensuing incident which could have had devastating consequences - could not have been foreseen. Despite safety measures being put in place, the main contractor was carrying out remedial works at height and had earthed their welder against the steel girders but left a number of trailing cables. The MEWP operator working beneath believed he had moved all of them out of the way. However, as the MEWP ascended, a trailing cable he had not spotted became severed, rendering the MEWP live which then made contact with overhead lighting track chains causing sparks and flames beneath the platform. The quick-thinking MEWP operator managed to leap 5 feet to safety. Link to website presentation slides

9 Drugs & Alcohol - Mark Rowley, Hampton Knight Key Points Alcohol misuse costs society 21bn per year an estimated 8.5bn to UK industry alone. Absenteeism/presenteeism costs 2.2bn per year. A survey found 60% of employers are reporting alcohol-related problems. An NHS survey found 25% of men had consumed over 8 units in one day during the previous week. Recommended weekly alcohol limit -men & women - now 14 units. Drug misuse costs an estimated 1.7bn to UK industry per year. It is now cheaper to consume drugs than alcohol. Uses on average between the ages of 15 to 35. Prescription drugs are an increasing problem, significantly affecting work performance, concentration and alertness. Their use is illegal if used incorrectly or when prescribed for someone else. 5.3% of the working population use illegal drugs. Alcohol misuse can be triggered by relationship difficulties, stress, depression, low self-esteem. Work-based predictors can be job insecurity, tight deadlines, long working hours, shift patterns etc. Alcohol-related crime costs UK taxpayers 11bn per year. It makes up a substantial portion of violent offences and is implicated in domestic violence, child abuse and sexual assault. Alcohol dulls and slows the brain s reaction times and thoughts and affects co-ordination. For every unit of alcohol consumed (eg half a pint of beer/small glass of wine), the body requires one hour to metabolise it. So, at 7am the next morning a person could still be under the influence. Over the last 3 years, 550,000 people have been admitted to hospital A&E departments due to drink and drugs 25% of them were in their 40s. Alcohol and Drug Policies are a good idea but should not moralise. Drug testing should be fair, transparent, stand up to scrutiny and use kits which are legally defensible. Be aware that some medical conditions can affect the outcome. Be clear on the consequences of drug test failure. Link to website presentation slides

11 Behaviour, Barriers & Leadership In very many cases: Unsafe behaviour is well intentioned. The unsafe behaviour usually occurs many times before an accident or occupational ill health. This means: There are many opportunities to observe & correct unsafe behaviour before an accident or occupational ill health. The lack of good practice in engineering / equipment & / or systems / procedures often provides a barrier to safe behaviour. Behavioural observation, two way dialogue, coaching & helping to remove barriers to safe behaviour (and good quality etc) is a fundamental role of leadership. Continuous improvement holds the key But, people need to know what good and not good look like. Single page learning
