1 In response to the needs of the South Carolina Initiative External Leadership Team, the Initiative Inventory was designed to identify existing health-related initiatives in South Carolina in order to highlight opportunities for collaboration and detect gaps in coverage. At the local (county) level, the Inventory focuses on those counties within the state demonstrating the greatest need for improvement in population health. Utilizing the 2013 County Health Rankings, the 10 lowest ranking counties in both health outcomes and health factors were identified to reflect both current health status and predicted future health outcomes. For the resulting 13 counties (discounting overlap), measures for adult smoking, adult obesity and adult physical inactivity were selected as the factors emphasized as they most closely align with the goals of the South Carolina Initiative. The county-specific tables have been populated using the South Carolina Initiative year 1 and 2 letters of intent, brief proposals and budget summaries and full proposal applications. Information was also provided by Eat Smart, Move More South Carolina, South Carolina Tobacco- Collaborative, the Outpatient Quality Improvement Network, South Carolina Office of Health, Blue Cross Blue Shield of South Carolina and South Carolina Hospital Association. Website searches were conducted to identify additional activities that fall into the scope of the South Carolina Initiative. The entries demonstrate where groups are mobilized for health improvement activities in each of the least healthy counties in the state. In addition, the tables indicate where the South Carolina Initiative has awarded funding to improve the health of the county.
