1 UNICEF Senegal Situation Report 25 September 2012 Highlights At the government s request, this month UNICEF and its partners have scaled up their technical support of the nutritional response to cover three more regions: Thies, Louga and St. Louis. 32 additional Inpatient Facilities (IPF) and 335 Outpatient Therapeutic Programs (OTPs) are being supported under the expanded program. This is in addition to ongoing activities in 7 IPF and 216 OTPs in Diourbel, Matam, Kolda and Tamba. UNICEF support to the Cellule de Lutte Contre la Malnutrition (governmental cell for malnutrition) begins to bear fruit, with an increase in the number of severely malnourished children screened at community level and their arrival at health centers for treatment. Over the past month, a community screening campaign in Diourbel has reached 201,137 under-five children; up from 126,711 in the last campaign in June. Screening in Matam is currently ongoing and results will be shared in upcoming reports. More than 300,000 people have been affected by flooding due to heavy rains during August and September throughout the country: outskirts of Dakar, Kolda, Sédhiou, Ziguinchor, Diourbel (Bambey, Mbacké, Touba), Fatick and Thiès (Joal). UNICEF is currently providing support to 150 affected schools and 15,000 households. 1

More importantly, the last national nutritional survey with SMART methods (June 2012) has highlighted new hotspot regions. Based on the results below, UNICEF and government have agreed on a scale up plan to support five additional Regions to the five already covered by UNICEF. Based on needs highlighted in the table below, the new regions are Fatick, Kaffrine, Tamba, Kedougou and Kolda. This direct support would be is in addition to UNICEF s ongoing provision of therapeutic foods for Severe Acute Malnutrition (SAM) treatment in all health facilities countrywide. 3 Flooding August-September More than 300,000 people have been affected by flooding due to heavy rains during August and September. Most affected areas are: outskirts of Dakar, Kolda, Sédhiou, Ziguinchor, Diourbel (Bambey, Mbacké, Touba), Fatick and Thiès (Joal). Two UN rapid joint needs assessment missions have taken place on the first and third weeks of September to analyse the resulting damage and needs. In addition to the destruction of homes and disruption of livelihoods, over 150 schools have been flooded, occupied by displaced families or otherwise affected. Some health facilities have been flooded as well, with total loss of medicine and equipment. Immediate action is also required to mitigate public health threats from observed stagnant waters and overflowing latrines. Special attention is focused on border areas where local health authorities are vigilant to the threat of cholera. Local authorities have responded to immediate needs, with UNICEF assistance where needed (Dakar suburb, Saint Louis and Ziguichor). Prepositioned WASH and Education in Emergencies supplies have been used. UNICEF is ready to further respond to any outstanding needs based on ongoing detailed assessments. The figures quoted in the chart above are based on reports received only. 3

4 UNICEF support to the CLM 1 has enabled it to increase the frequency (every 2 months instead of 3), density of its coverage and quality of its community-level interventions. This support includes funding of screening equipment, transport for referral of SAM cases, home follow-up and care for recovering SAM children, systemic treatment such as Iron and Vitamin A, and nutritional education of mothers using locally available foods. The resulting CLM activity begins to bear its fruit, with an increase in the number of severely malnourished children screened at community level that arrive at health centers for treatment. Over the past month, the number of reported SAM cases has gone up from 2,736 cases to 4, The CLM has conducted a malnutrition screening campaign in Diourbel this month targeting 228,079 under-five children. Using MUAC, the CLM has screened 201,137 under-five children, of which 9,289 were found to suffer from MAM and 1,223 were cases of SAM for referral. The CLM coverage shows an increase from the June campaign, when 108,025 children were targeted and 126,711 screened. Screening by the CLM is currently ongoing in Matam and results will be shared in upcoming reports. At the government s request, this month UNICEF has scaled up its technical support of the nutritional response to cover three more regions (Thies, Louga and St. Louis). UNICEF has extended its partnership with Action Contre la Faim, who will accompany health personnel on the ground to bolster the quality of nutritional services offered. Under the expanded program, 32 additional Inpatient Facilities (IPF) and 335 Outpatient Therapeutic Programs (OTPs) are being further supported with equipment, training, on-the-job capacity building for treatment, management and monitoring. This is in addition to ongoing activities in 7 IPTs and 216 OTPs in Diourbel, Matam, Kolda and Tamba and to provision of therapeutic feeding inputs. To date Senegal has 34 Hospitals, 107 Health centres and 1299 Health Posts. The Hospitals and Health centres host the IPFs, and the health posts host the OTPs. These health facilities treat the severe acute malnutrition, but there are no data to assess the coverage of this program, due the strike of health worker resulting to the data retention. UNICEF supports treatment of Severe Acute Malnutrition in under-five children nationwide by providing therapeutic foods to government health services. 5 All 7 IPTs in Matam and Diourbel currently provide the WASH minimum package, with scale up to cover the regions of Thies, Louga and St. Louis almost finalised. Scale up to cover the 30 IPFs in the new areas of intervention (Thies, Louga, St. Louis) should be complete shortly. UNICEF is providing hygiene kits to 15,000 households affected by the recent floods, as well communications materials for the prevention of water-borne diseases in all affected Regions. Education With the academic year beginning on October 4 th, 22,500 students in ten regions affected by the floods are being supported by UNICEF. 150 schools will receive supplies such as disinfection materials, school-in-a-box, recreation kits and ECD kits, as well as iron supplements for the students. HIV/AIDS Children not responding to treatment at Inpatient Facility level are tested for HIV/AIDS. There have only been two cases of malnourished children testing HIV positive this month. This is consistent with the country s low HIV prevalence rate. Communications A UNICEF-sponsored series of documentary emissions on malnutrition has launched this week on national television. The program will cover different aspects of malnutrition such as identification, treatment and prevention, and will air nationally and internationally on a weekly basis. Wolof language is used alongside French to target female caretakers. A UNICEF communications consultant has assisted the regions of Matam, Diourbel, Thies, St. Louis and Louga to revise their emergency response plans in order to incorporate elements of sensitisation and communication for behavioral change. A 2-hour radio show in a Q&A with experts format also currently airs every other Saturday to improve the population s understanding of malnutrition and good nutrition and hygiene practices. 5

6 Interagency Collaboration and Partnerships The emergency response plan with government is an interagency joint plan. Fortnightly inter-agency coordination meetings are held between WFP-UNICEF- WHO to monitor progress against their joint response plan with MoH. Implementing partners such as ACF and the French Red Cross, and donors (ECHO), have been invited to participate for improved coordination and information sharing. The next meeting to be held on October 2 nd. Two UN joint rapid assessment missions have taken place in response to flooding (first and third week of September). The joint report outlined agreed-upon needs, available contingency supplies and gaps. WFP has launched its project to support 11 Regions with supplementary feeding for moderately malnourished children, and pregnant and lactating women. The project, which is to provide supplementary feeding to 73,455 under-five malnourished children and 19,105 malnourished lactating and pregnant women, had reached 8,513 children and 3,203 women by mid-september. Coordination National coordination meetings take place every two weeks under the leadership of the Ministry of Women Affairs and Social Action, designated by the Prime Minister to monitor progress of the response to the nutritional crisis. A monitoring committee visited Diourbel, Tamba and Kaffrine on September 11 th -14 th. Regional coordination meetings currently take place in Diourbel and Matam regions under leadership of the Regional Governor. All stakeholders are invited to these meetings with the latest taking place in Matam on September 21st. UNICEF has presented the results of the last National Nutritional Survey with SMART methods (June 2012) to the Thematic Group on Food Security and Nutrition, to raise awareness on the situation and current needs. Constraints Although UNICEF has successfully advocated for the release of nutrition response data in Diourbel and Matam, local capacity to report (skills and human resources) is still low, with only about 50% of reports being submitted, and often not on time. In all other regions, the data retention strike by health workers continues to considerably limit access to nutrition data. NGOs specialized in nutrition are limited in number and geographic presence. UNICEF continues to seek partners to support the ongoing response in Fatick, Kaffrine, Tamba, Kedougou and Kolda. Additional funding is needed for scale up of interventions in all crisis-affected regions. 6
