1 November 2012 Highlights UNICEF Nigeria Monthly Humanitarian Situation Report A total of 17,684 new cases of children with Severe Acute Malnutrition (SAM) have been treated in October in the 414 Community Management of Acute Malnutrition (CMAM) sites in 8 Sahel States - Kebbi, Sokoto, Zamfara, Katsina, Kano, Jigawa, Yobe and Borno The cumulative number of children reached from January to October is at 179,452; of these 166,233 are from Sahelian states and 13,219 from non-sahel states - Bauchi, Gombe, Adamawa The funding gap for Nutrition Crisis is 60%. The funding shortfall for the sectors beyond nutrition places constraints on UNICEF integrated response (esp. WASH) to scale up plans to reach more vulnerable children. According to the National Emergency Management Agency (NEMA) the number of IDPs related to floods reduced, between Nov 15 th -30 th, from 387,153 to 84,679. The UNICEF Representative and emergency M&E person conducted a mission to some of the flood affected States from the11 th to the 16 th of November to review on-going humanitarian response, especially in sectors where UNICEF is the lead agency or co-leads. The Mission visited 5 states(delta, Bayelsa, Anambra, Benue & Kogi) states and observed that Camps were at the verge of official closure and the State governments in the process of commencing the recovery interventions as the IDPs return to the communities. 84% of Cholera cases until Nov 30 th are reported from non-flood affected States (Kaduna, Zamfara and Gombe) Situation Overview & Humanitarian Needs 1. More people are moving back to their community of origin. Between Nov 15 th -30 th, NEMA reported the number of Internally Displaced People (IDPs) reduced from 387,153 to 84,679. Emergency assistance is moving progressively from camps to the communities, while early recovery and medium term support for reconstruction is also needed, in order to enable affected population to rebuild their livelihoods. Additional to more than US$ 76 million of pledges have been raised by the Presidential Committee for Flood Relief and Rehabilitation, which is acting as a fundraising structure. 1

2 As of Nov, 30 th NEMA reported the South-south, South-East, South-West Region and Abuja camps managed by State Emergency Management Agencies (SEMAs) have been closed. Three camps in Kogi, one camp in Jigawa and few make-shift camps in Borno, Yobe, Nassarawa, Adamawa and Taraba States remain open. Most of the occupied schools were vacated in October and are free for children to prepare for the end of year primary school exams., except for locations where the schools were seriously damaged by the floods. UNICEF is working with partners in the worst affected states to map the conditions of the schools and support provision of temporary learning spaces during the new school year. Source: OCHA 3. Cholera A total of 444 suspected Cholera cases (with no laboratory confirmation) and 8 deaths (CFR 1.80%) were reported between the weeks 1-47 in 2012 (Nov, 30 th 2012). This is an increase of 6 cases and 2 deaths since last month. On the whole, 27 LGAs (11 states) have reported at least 1 case of suspected cholera since the beginning of % of Cholera cases until Nov 30 th are reported in non-flood affected States (Kaduna, Zamfara and Gombe) 4. Agency Collaboration and Partnerships In the context of recovery and rehabilitation of flood affected areas, the Government of Nigeria, with the support of the European Commission, the UN and the World Bank, are currently conducting a Post Disaster Needs Assessment to measure the damage and impact of the floods. The Post Disaster Needs Assessment (PDNA) is focusing on: WASH, Food & Livelihood, Education, Shelter, Health & Nutrition, Culture, Psychosocial Need & Protection, Transport & Communication, Environment, DRR, Emergency Needs, Governance and Conflicts resulting out of flood disaster The country office continues to collaborate with the IA EPR Working Group, comprising of the UN Agencies and International NGOS on the sector based response planning, providing support to the humanitarian Country team (HCT), taking responsibilities for sector lead roles and sub-sectoral coordination work in the various sectors where UNICEF is a stakeholder as well as in the overall humanitarian context where UNICEF acts as provider of last resort in line with the CCCs. 2

3 Program response Nutrition Overview Sahel In 8 Sahel States in the month of October, 17,684 children were admitted to a total of 414 sites in the 8 Sahel States. The cumulative number of children reached between January and October is at 166,233 In the total of 11 States where UNICEF Nutrition is implementing their activities, in the month of October, 19,328 children were admitted to a total of 468 sites. The cumulative number of children reached in all 11 States of Nigeria between January and October is 179,452. The statistic in Nov SitRep covers only 8 Sahel states while in Oct it covered the total of 11 Northern States in which Nutrition Section implements CMAM programing. UNICEF and partners programming Jan Feb Mar Apr May June July Aug Sept Oct 0 Jan Feb Mar Apr May June July Aug Sept Oct Figure 1: Admissions Sahel States = 166,233 Figure 2: Admissions Non-Sahel States = 13,219 The reporting rate for the month of October in 11 States stands at 83.5% while the reporting rate from 54 OTP sites in the Non-Sahel at 81.5%. Defaulter rate for the month of October is (for 8 States) 24.2%, and for 11 states 19.6%. The death rate is within the recommended SPHERE standards at 1.7% (11 states 1.9%). A study on community perception and possible reasons for high default-rates has just been concluded and results will be used to review interventions in 2013 with a view to lowering the rates in the worst affected states. Nutrition surveys were carried out in the 8 Sahel States between August and September. The prevalence of global acute malnutrition (GAM) was found to be above 10 percent in all surveyed states except Kano. The highest and lowest prevalence of GAM according to WHZ-score were reported in Sokoto and Kano states at 16.2% and 9.2% respectively. The highest and lowest prevalence of severe acute malnutrition according to WHZ-score were reported in Sokoto at 4.4% and in Katsina at 1.6%. Compared to results of the nutrition survey conducted in the same period last year, higher prevalence of severe acute malnutrition were reported 3

4 in Kano, Kebbi and Sokoto states whereas the prevalence in Katsina state has decreased when compared to the same period last year. Katsina shows good results because of scaling up CMAMs, malnutrition awareness prevention work, while in other states various factors contribute to increase of SAM such as access to markets, purchase power, availability of basic market goods, etc. These findings will be used in planning and strategies in Acute Malnutrition Using WHZ Global Acute Malnutrition States Borno 10.6% ( ) 11.5% Jigawa ( ) 9.2% Kano ( ) 12.8% Katsina ( ) 11% Kebbi ( ) 16.2% Sokoto ( ) 10.2% Yobe ( ) 11.1% Zamfara ( ) Severe Acute Malnutrition 2.7% ( ) 2.6% ( ) 2.3% ( ) 1.6% ( ) 2.0% ( ) 4.4% ( ) 2.2% ( ) 1.7% ( ) Integration of Polio and CMAM is now fully operational in Kano. 557 Volunteer Community Mobilization Network (VCMs) and 57 supervisors were trained and teams are conducting case-finding and defaulter tracing within CMAM catchment areas. Plans are in place to scale up in Yobe, Borno, Jigawa and Bauchi Katsina and Kebbi States are providing LGAs with funding for CMAM program monitoring and implementation on a monthly basis (N ) per implementing LGA. CMAM taskforce/partners meetings are now taking place in Kebbi, Katsina and Zamfara during which data is collected and challenges in the program are discussed Facility-based infant and young child feeding counseling is now fully integrated with counseling activities currently ongoing into CMAM services in three LGAs in Jigawa state. Training was completed for 150 community volunteers and 30 health workers. Plans are in place for scaling up into other LGAs and states (which ones?) Short term - LGAs in Jigawa, longer term (2013) expansion to Katsina and Kebbi States. 150 community members (male, female, youths) were reached with key messages on prevention of malnutrition, diarrheal diseases and other childhood diseases in Kebbi State. In Jigawa, plans have been advanced to scale up the use of zinc supplementation for the treatment of children with diarrhea Recruitment of Third party monitoring consultants for Nutrition has advanced with candidates identified for deployment to Borno and Gombe States. The presence of the third party monitoring consultants is expected to augment state capacity for the Nutrition program and supporting monitoring and on-job mentoring of the CMAM program A capacity mapping exercise on CMAM and IYCF was carried out for all states in North East Nigeria. The number and location of health workers that received training will be used to guide plans for scale up of interventions and identify training needs Overview Floods A total of 348,198 under five children have been screened for malnutrition in the most affected states (Oyo, Kogi, Benue, Plateau, Adamawa, Delta, Bayelsa and Anambra). Of this number, 3,261 (or 1%) were severely malnourished, admitted and given therapeutic treatment. They were also administered with vitamin A supplements as per treatment guideline A total of 793 children with diarrhoea have been identified and treated with Zinc and low Osmolarity oral rehydration salts. This represents 15% of total estimated caseload of diarrhoea among the displaced children 6-59 months 4

Hygiene promotion was accompanied by distribution of soap by State Government for a population of 200 families as part of the campaign Training of 20 WASH health facility staff on integrated ANC, HIV/AIDS and WASH response was organized in 9 CMAM centers in Jigawa State Caregivers in over 60 communities utilize 15 new compartments of sanitary latrines with hand washing facilities in 5 CMAMs in Zamfara state Overview Floods A weekly meeting of the WASH Emergency Sector Group, led by the Federal Ministry of Water Resources and co-led by UNICEF, was held on Nov 19 th, with the attendance of the Minister and Permanent Secretary 3W Matrix capacity mapping of Sector members on-going and expected to be completed by the end of November A Workshop for 36 States on WASH Emergency Response and Recovery is prepared for the first week of December in Abuja. The Permanent Secretary of State Ministry of Water Resources, the Director SEMA and MD/PM RUWASSA from all Nigerian states are expected to participate The WASH Sector Response Plan including the fund requirement for Flood response was developed and shared with Presidential Committee through UNHC/UNRC. The review of the plan is on-going and taking in consideration new figures in the IDP camps/host/returnee communities State- led, Post- Flood assessment of affected flooded areas was carried out by a joint assessment team in Jigawa, Adamawa and Taraba States. Jigawa State has facilitated the resettlement of flood victims estimated at 5000 along the River Hadeja to a new location at higher ground Adamawa and Taraba States have conducted orientation on personal hygiene and water disinfection reaching a population of 8,000 people affected by the flood disaster Health* Estimated #/% coverage * Data above is from Nigeria Oct SitRep. To consolidate on the gains and opportunity provided by the National Council of Health, a high level consultative process was undertaken between NPopC and the health care delivery systems (from all 36 States plus the Federal Capital Territory) in October 2012 to structure a framework for improved birth and death registration coverage The meeting identified strategies for improved routine birth registration processes within the health sector, established decentralized networks for service delivery, agreed on a road map for changes in programming and improved registration coverage of 0-59 months children accessing all CMAM /health centres With the structured framework with health care delivery actors - for improved birth registration coverage - currently, birth registration of 316,418 children are on-going in 67 LGAs, in 363 CMAM centres and in the 11 Sahelian states The number of children being born and/or accessing treatments in these CMAM centres (including children born within the birth registrar s coverage area) continues to increase, with an additional 10,277 registered in September and 16,207 in October Children under-five with birth registration and recorded in CMAM centres continue to be far beyond the set targets because numbers include new born and children born outside the facilities/cmam centres but within the registrar s coverage areas. Some efficient registrars have existing relationships with traditional/religious institutions, and other notifiers all assisting with ensuring births of children in the remote and hard to reach areas are captured/registered. The registration booklet from where the figures are computed does not indicate whether the child is malnourished or not, but only reflects children registered in the CMAM centres/lgas and the locality Overview Floods In order to prepare a coordinated response to the on-going Flooding emergency, a Child Protection Sub-sector Working Group has been established and the first meeting was held on Oct 31 st. At the meeting it was agreed that due to the lack of protection indicators in the Joint Rapid Assessment coordinated by OCHA, a separate Child Protection Rapid Assessment should be carried out in the 8 most affected states (Anambra, Bayelsa, Benue, Edo, Kogi, Niger, Plateau, and Rivers). It was agreed that the assessments should be carried out by Child Protection Networks which had recently been trained in child protection in emergencies (Gombe, Kaduna, Delta, Rivers and Bayelsa), with the support of "local" CPNs in the selected states which had not been trained (Benue, Edo, Anambra, Niger, Plateau and Kogi). Technical assistance for CPNs will be provided through UNICEF consultants (under existing SSAs) and Save the Children Child Protection team. A second meeting of the Child Protection Sub-sector Working Group was held on 2 November and Adamawa was added to the list of states to be assessed Rapid assessments have now been completed in Benue and Kogi (but some reports are yet to be received), while assessments are ongoing in Bayelsa, Niger, Delta and Plateau Initial Results show: sexual and domestic violence on increases because of pressure on families, child prostitution in exchange for food, teenage boys pressure to join the armed groups and ritualistic cults, child substance abuse, violence at NFI, food distribution, lack of organised camp management 6

7 Education The Education in emergencies working group was officially established during a two day workshop (29-30 Nov) under the leadership of the Ministry of education and with support from UNICEF and partners. In order to prepare coordinated response to the on-going Flooding emergency, UNICEF, Save the Children and UNESCO are supporting the Ministry of Education in the development of an integrated Education Emergency Response plan (currently under revision), identifying the specific objectives of immediate restoration of Education for flood-affected children in safe and protective learning environments Furthermore, UNICEF, the Ministry of Education and Save the Children held several sectoral meetings in order to prepare for Post Disaster Needs Assessment (PDNA), recovery and reconstruction.training for PDNA Education sector was conducted by World Bank, European Community, UNDP and Ministry of Education with UNICEF and UNESCO participation. UNICEF is supporting the Ministry of Education, which is leading the education sector for the PDNA, with the collection and analysis of the data from 12 states (aim to present draft report to the Government before end of December). Education emergency supplies for Kaduna Field Office flood response included 18 Tents (24m 2, rectangular, double roof), 30 Replacement kits (school in a box), 32 Recreation kits. Supplies awaited from off-shore include further 50 Tent (24m 2 rectangular Class in a tent), 50 Replenishment kits (School-In-a Box), 50 Recreational Kits and 30 boxes of ECD Kits HIV/AIDS The Bauchi Field Office is developing a CMAM monitoring and supervision visits Plan to ensure that services are being provided and documented on regular basis. Specific focus is given to ensure the conduct of HIV Counselling and Testing (HCT) among complicated cases of SAM in highly populated sites and sites in LGAs with high HIV prevalence The Kaduna Field Office data analysis of the assessment of integration of HIV services in CMAM sites is still on-going. The office has completed plans to support the training of health care workers from 30 sites in Zamfara state on HCT, starting on Dec 6 rd. UNICEF has mobilized Society of Family Health (SFH), Family Health International (FHI) and Zamfara State Agency for the Control of AIDS (SACA) to support this SFH, FHI to provide the trainers, while the Zamfara State Agency for the Control of AIDS shall provide test kits. As part of the World AIDS Day celebration, the Kaduna office is working with Nigerian Television Authority to create more awareness and to mobilize support through a TV discussion program focusing on HIV and nutrition response in the Sahelian crises A 2013 work plan and budget for the children and AIDS response has been developed for implementation in the 8 Sahelian states Communications for Development (C4D) Following advocacy and sensitization on the Sahelian food and nutrition crisis for a large number of media representatives from 10 states in the North East, newspapers including Nations, Trust, New Nigerian, Leadership and the Guardian have begun reporting on related issues. Several media representatives, including representatives of State-owned radio and TV channels gave a commitment to set up separate Nutrition Desks Supply and Logistics Between January and 30 th October 2012, a total of 119,929 RUTF cartons have been delivered to 8 States with CMAM centers in Northern Nigeria and UNICEF Kaduna Field Office, keeping a small amount as emergency stock. Over 160,000 Children have so far been treated as at October Tracking of deliveries between States and the LGAs & utilization rates will be further monitored under a new arrangement by engaging an NGO in a process that is at an advanced stage. Despite the huge impact of the floods, logistics for delivery of materials to the states where CMAM activities are on-going are unaffected as they are mostly in the Northern part of the country. 7

8 KATSINA KEBBI SOKOTO ZAMFA UNICEF BORNO JIGAWA KANO YOBE Qty Delivered Jan - Oct As part of the Flood response, UNICEF deployed the following emergency pre-positioned supplies: RUTF (500 cartons), F-75 (50 cartons), F-100 (25 cartons), Resomal (10 cartons), Vitamin A capsules (10 bottles), Albendazole (25 bottles), Zinc tablets (300 packs) and low Osmolarity oral rehydration salts (10 cartons) targeting 10% of the affected children U5 population. Challenges faced The exhaustion of EPR pre-positioned emergency supplies for 2012 as a result of the emergency flood response has created a need to replenish supplies in preparation for the recovery and rehabilitation phase of the response. Sections and the Field offices are working to prepare detailed replenishment plans while some of the Field offices have supplies in the pipeline. Funding needs earlier identified during the emergency response phase are now being modified to reflect needs in the early recovery phase. Human Resources Total surge capacity is 21 people, 11 were mobilized and are currently in the country, 9 are under recruitment and 1 is not yet funded. 9 and September 2012). Given the current situation of the floods, the most critical needs are USD million for 2012 with only USD 1million received so far.
