You will not be penalized for not answering every question; however, we ask that you provide as much information as possible for our research. This survey has been designed so that you can save the answers you input and return to the survey link at another time to submit additional information or edit your previous responses. We will not retrieve any of the data you input until the deadline of 3/2/2018 at 5pm (EST). The information you include in this survey will be automatically submitted on the above stated deadline.
