3 3 Overview: What is the 2019 APS Employee Wellness Incentive Program and how do I qualify? The goal of the 2019 APS Employee Wellness Incentive Program is to improve the health outcomes of APS employees and their families. Through the Employee Wellness Incentive Program, APS offers savings on monthly medical plan premiums to employees and spouses/domestic partners who complete the following steps: Step 1: Complete a preventive medical exam/visit a) Visit to download and print the Physician Engagement Form. b) Complete the employee/patient portion of the Physician Engagement Form
4 4 c) Complete a preventive medical exam/visit anytime between Oct. 1, 2017 and Sept. 30, Have your provider/provider s office complete the provider portion of your form and return the form to you. This year you will be responsible for uploading this form to the Wellness at Work online platform. Between July 1 and September 30, 2018, complete steps 2-5 below. Step 2: Log in to the Wellness at Work online platform (a) Visit the APS Employee Wellness Incentive webpage: (b) If this is your first time accessing Wellness at Work, click Step 1: Start Here for Access Instructions
5 5 (c) Select Employee or Spouse/Domestic Partner. In accordance with federal laws, if you are the spouse or domestic partner of an APS employee, and wish to participate in the Wellness Incentive Program, you will be directed to an authorization form to complete before being directed to the Personal Health Assessment for completion. (d) Follow the Wellness at Work registration instructions.
6 6 (e) Once registered and you are logged in to Wellness at Work, you will see the homepage for APS. Step 3: Complete the online Personal Health Assessment Click here to take your Personal Health Assessment. It generally takes between 20 and 30 minutes to complete.
7 7 Once you complete the Personal Health Assessment, the tracking bar on the Wellness Incentive Tracker near the bottom of the homepage will turn green and you will be awarded 25 points. Step 4: Submit your Physician Engagement Form a) Click here to upload your completed Physician Engagement Form (you ll also complete your Tobacco Affidavit). You ll need to upload a scanned version or picture of your Physician Engagement Form. This step generally takes minutes minutes.
8 8 b) Before uploading your Physician Engagement Form, be sure to complete the required fields. c) Upload a picture/ scanned version of your Physician Engagement Form. Save the original form for your records. Once uploaded, continue on to the Tobacco Affidavit, located below on the same page.
9 9 Step 5: Complete your Tobacco Affidavit
10 10 This is the screen you will see once you have successfully uploaded your Physician Engagement Form and completed your Tobacco Affidavit. Your completion of the Physician Engagement Form and Tobacco Affidavit will be reflected on the Wellness at Work homepage within 7 days of when you submit these forms. After the 7 day review period, the appropriate tracking bars on the Wellness at Work homepage will turn green. You will also receive the corresponding points (50 points for uploading the Physician Engagement Form and 25 points for completing the Tobacco Affidavit).* * If you attest to using tobacco products on your Tobacco Affidavit, you will need to complete a 12-Week Online Tobacco Cessation Workshop. Upon completing this course, you will be awarded 25 points. The 25 points awarded for this Tobacco Cessation Workshop are in lieu of the points awarded for the Tobacco Affidavit. If you attest to using tobacco products on your Tobacco Affidavit, you will need to complete a 12-Week Online Tobacco Cessation Workshop, worth 25 points. You will receive the 25 points for this workshop instead of the 25 points for completing the Tobacco Affidavit.
11 11 Who is eligible for the 2019 APS Employee Wellness Incentive Program? Employees, Spouses, and Domestic Partners currently enrolled in one of the APS medical plans are eligible to receive the 2019 Employee Wellness Incentive. Individuals not currently covered under one of the APS medical plans are not eligible to receive the 2019 Employee Wellness Incentive. If you are uncertain whether you are currently enrolled on one of the APS medical plan options, contact the Benefits Specialist assigned to your last name: Cruzita Lujan (A-D) Claudia Velasquez (E-K) Andréa Marie Biggs (L-Q) Sheila Jaramillo (R-Z) Employee Benefits Department general phone number: Once I complete all steps of the 2019 APS Employee Wellness Incentive Program, how much will I save on my monthly medical plan premiums? Individuals who complete all steps for the 2019 APS Employee Wellness Incentive Program will save $25 per month ($300/year) on their medical plan premiums. If both the APS Employee and their Spouse/Domestic Partner complete all steps, the medical plan savings will be $50 per month ($600/year). How long do you estimate it takes to complete each step of the 2019 Employee Wellness Incentive Program? We estimate completion times as follows: Registering for Wellness at Work & setting up your Wellness at Work profile: minutes on average Completing the Personal Health Assessment: minutes on average Uploading your completed Physician Engagement Form and completing your tobacco affidavit: minutes on average How can I verify that I ve completed the steps required to receive the 2019 APS Employee Wellness Incentive? On the Wellness at Work homepage, find the Wellness Incentive Tracker: In this example, the individual has completed the Personal Health Assessment. The Physician Engagement Form and Tobacco Affidavit remain to be completed. Note: Please allow 7 days after submitting your Physician Engagement Form and Tobacco Affidavit for your progress to be reflected on the Wellness at Work Incentive Tracker. In order to receive the 2019 Employee Wellness Incentive, you will need a total of 100 points.
12 12 Physician Engagement Form & Annual Preventive Medical Exam/Visit Questions What qualifies as an annual preventive medical exam/visit for the purposes of the 2019 APS Employee Wellness Incentive Program? The Primary Care Provider annual visits that are included in the 2019 Wellness Incentive Program are: Family Practice, General Practice, Internal Medicine, Doctor of Osteopathy, Nurse Practitioner, Physician Assistant Endocrinologist Cardiologist Oncologist Obstetrics, OBGYN, Gynecologists if identified as a primary care provider/practitioner How do I schedule an annual preventive medical exam/visit? Schedule an appointment with your Primary Care Provider/Practitioner. If you need a Primary Care Provider/Practitioner: Presbyterian Health Plan members can call: PRES DOC ( ) or BlueCross BlueShield of New Mexico members can visit the BCBS Provider Finder, or contact: DaVita Medical Group (505) Lovelace Care Concierge (505) True Health New Mexico members can contact: Note: Your preventive medical exam/visit must occur between October 1, 2017 and September 30, 2018 to qualify for the 2019 APS Employee Wellness Incentive Program.
13 13 Where can I find the Physician Engagement Form? You can find the Physician Engagement Form by visiting the APS Employee Wellness Incentive webpage: Then click Physician Engagement Form For Your Doctor To Fill Out. I completed a preventive medical exam/visit between October 1, 2017 and September 30, 2018, but I didn t have the Physician Engagement Form with me. Do I have to make another appointment? If you already completed a qualifying preventive medical exam/visit sometime between October 1, 2017 and September 30, 2018, you do not need to schedule another exam. Rather, complete the employee/patient portion of the Physician Engagement Form, then return to your provider s office and have the provider/provider s staff complete the physician portion of your form and return the form to you. This year you will be responsible for uploading your completed form to the Wellness at Work online platform. Wellness at Work Questions I m having trouble registering/logging on to Wellness at Work. 14 14 Personal Health Assessment Questions What is a Personal Health Assessment (PHA)? The Wellness at Work online portal provides a questionnaire called a Personal Health Assessment (PHA) that aids in identification of personal health risks, provides recommendations for improving those risks, and offers easy-to-use tools to help members make healthy lifestyle choices. The assessment is reviewed by clinicians who have extensive backgrounds in health promotion and primary care. The PHA uses multiple models to assess the participant s risk for both preventable and chronic diseases. PHA integrates the Framingham Heart Study, the Centers for Disease Control and Prevention, the United States Preventive Task Force (USPSTF), and the Prochaska and Clemente s Stages of Change. How is the information collected in the Personal Health Assessment used? The information you provide in your Personal Health Assessment is Protected Health Information (PHI) and protected by law. It will not be shared with Albuquerque Public Schools (APS) or any other unauthorized third party. Aggregated data will be shared with APS to inform employee wellness programming decisions. How long does it take to complete the Personal Health Assessment? We estimate it will take minutes on average to complete the Personal Health Assessment. How will I know if I ve completed my Personal Health Assessment? As soon as you complete the Personal Health Assessment (PHA), the PHA completion tracking bar located on the Wellness at Work homepage will turn green and you will be awarded 25 points. I m having trouble completing my Personal Health Assessment. How can I get additional help? Call the Wellness at Work help desk ( , Monday-Friday, 6 a.m.-6 p.m.) or wellnessatwork@phs.org. 15 15 12-Week Online Tobacco Cessation Workshop Questions Do I have to complete the 12-Week Online Tobacco Cessation Workshop? If you attest to using tobacco products on your tobacco affidavit, you will need to complete this 12-Week Online Tobacco Cessation Workshop in order to qualify for medical plan savings under the 2019 APS Employee Wellness Incentive Program. If you attest to NOT using tobacco on your tobacco affidavit, you DO NOT need to complete this 12-week online course. In this example, the individual will need to complete the 12-week online Tobacco Cessation Workshop since he/she attested to using tobacco products on the Affidavit. What is the 12-Week Online Tobacco Cessation Workshop? The 12-Week Online Tobacco Cessation Workshop is a self-paced online course, available on the Wellness at Work online platform. This 12-week workshop is designed to help you become tobacco free by the conclusion of the course. In this online workshop, you will be assigned a series of tasks each week. You cannot continue to the next week s tasks until you have finished the tasks assigned to you for the current week. If you finish the current week s tasks early, you will not be able to begin the following week s tasks until they are released each week. It takes a minimum of 12 weeks to complete this workshop. When is the deadline to sign up for and finish the 12-Week Online Tobacco Cessation Workshop? The deadline to sign up for the 12-Week Online Tobacco Cessation Workshop is September 30, In order to qualify for medical plan savings under the 2019 APS Employee Wellness Incentive Program, you must complete the course by December 31, 2018.
16 16 Because I indicated that I use tobacco on my Tobacco Affidavit, I must complete the 12-Week Online Tobacco Cessation Workshop to qualify for the 2019 APS Employee Wellness Incentive. How do I sign up for this course? a) Log on to Wellness at Work and click here: b) Under Add click 12-Week Tobacco Cessation- Take the First Step! Then click Signup.
17 17 b) Once you have signed up, the workshop will appear under the Active tab. Click this link to open the workshop.
18 18 I ve already signed up for the 12-Week Online Tobacco Cessation Workshop. How do I access it again and/or check my progress? On the Wellness at Work homepage, locate the To-Do List icon. Each week you will be assigned a number of tasks. Click each task to open and complete it. In this example, the individual is in week one of the 12-Week Online Tobacco Cessation Course. Once you complete a task it will automatically appear under Completed.
19 19 How can I verify that I ve completed the 12-Week Online Tobacco Cessation Workshop? As soon as you complete this workshop, the Tobacco Cessation Workshop tracking bar located on the Wellness at Work homepage will turn green and you will be awarded 25 points. In this example, the individual s tracking bar is not green. This means they have not yet completed the Tobacco Cessation Workshop. The tracking bar will immediately turn green when the workshop is completed. When the bar turns green, 25 points will also be awarded.
