3 Harborview The only Level I adult and pediatric trauma center and regional burn center serving Washington, Alaska, Montana and Idaho. Owned by King County, governed by a county-appointed board of trustees and managed by the University of Washington (UW). One of the two primary teaching sites for the University of Washington School of Medicine and other UW Health Sciences programs. Safety Net Provider - Our mission is to serve patients from all walks of life. In 2005, Harborview provided $98 million in charity care. For 2006, it has grown to $112 million. 3
4 Mission Harborview Medical Center is a comprehensive health-care facility dedicated to the control of illness and the promotion and restoration of health. Its primary mission is to provide and teach exemplary patient care and to provide health care for those patients King County is obligated to serve. 4

6 Stats Licensed beds: 413 Employees: 4,027 Physicians: 413 active medical staff Highest occupancy rate of any hospital in the region. 97 percent in More than 81,000 Emergency Department visits in More than 18,750 inpatient admissions in More than 358,950 outpatient visits in 2006 (including clinics, mental health and emergency department). 6
7 Division of Gerontology and Geriatric Medicine Formally established in 1977 Program Includes: Long-Term Care Service Medicine G / Senior Care (Inpatient) SeniorCare Clinic Research Geriatric Medicine Fellowship Program 7
8 Long-Term Care Service Managed by both UWMC and HMC MDs and ARNPs work in pairs to provide primary care and medical case management for patients discharged from UWMC and HMC to selected community nursing homes Provide service to 650 patients in 15 skilled nursing and several assisted living facilities Referrals for house call visits to homebound patients are also accepted 8
9 Long-Term Care Service Coordinate follow up appointments and/or readmissions Assist in controlling hospital length of stay for those patients who are no longer in need of acute care, but have needs for skilled care Coordinate end-of-life care with community Hospice agencies 9
10 Medicine G / Senior Care (Inpatient) Geriatric Medicine team will consult on all patients 65 years and older that are admitted to the Orthopedic and Urology Services Consultation is available to all units (by request) for any patient 65 years or older Consultation service includes: Diagnosis and management of medical problems Medical care of surgical patients, including pre-operative evaluation and post-operative management Comprehensive assessment of functional status and appropriate levels of care Discharge planning to optimize use of community and outpatient services available 10
11 Medicine G / Senior Care (Inpatient) Patients are admitted or transferred to the Senior Care inpatient unit if the patient has potential for rehabilitation or stabilization at an improved level of function through intensive and coordinated evaluation and therapy Program Includes: Full-time medical fellow Geriatrics oriented nursing staff Full-time social worker Recreational therapist Occupational therapist Physical Therapist Pharmacist Medical Resident rotations Nutrition support services 11
