4 The Department of Orthopaedics and Traumatology in Sarawak General Hospital, Kuching, Sarawak, Malaysia is an 200 bedded referral center for the state of Sarawak of about 2.2 million population. It also received patient from the nearby states of Sabah, Malaysia, Brunei and Kalimantan, Indonesia. A team of 8 Orthopaedics surgeons performs a wide range of procedure here. We have a 12 bedded ICU and also an HDU to support our patients through their recovery period. Majority of the patient treated here is due to trauma (about 80%). Joint replacement surgery, spine surgery, arthroscopic surgery, pediatric orthopaedic surgery as well as orthopaedic oncology make up the remaining workload. I am mainly involved in the care of spine patients. Scope of work includes spine trauma (cervical, thoracolumbar and sacral), infective spondylitis, neoplastic conditions, scoliosis correction and degenerative cervical/lumbar spine diseases. Both the adult and children spine conditions are treated by the same teams of surgeons in Sarawak General Hospital. I perform lumbar provocative discography, nerve root/epidural, facet injections and RF for spinal pain intervention. Both open and minimally invasive procedures are carried out here. Surgical procedures carried out include cervical anterior plating. Odontoid screw fixation, cervical lateral mass fixation, cervical pedicle screws fixation, C1-2 transarticular screws fixation, anterior decompression/plating of thoracic and lumbar spine ( thoracoscopic assisted surgeriesa0, pedicular screw fixation of thoracic and lumbar spine, posterior lumbar interbody fusion subtraction osteotomy of thoracic and lumbar spine, artificial cervical/lumbar disc replacement and percutaneous endoscopic lumbar discectomy/foraminoplasty. As the hospital serves mainly the whole of the state of Sarawak, the majority of spine cases seen here are traumatic conditions of the spine, and tuberculosis of spine. Traumatic injuries of the thoracolumbar junction and cervical spines that require surgical decompression and stabilization are frequently carried out. The surgical treatment of tuberculosis of spine involves mainly those active diseases that require debridement and fusion. However, some delayed cases with severe spinal deformity needing correction surgery are also performed here. EDUCATIONAL ACTIVITIES Key instructor for the Kuching Operative Spine Courses (KOSC), a small group hands-on course aims at improving surgical skills and confidence in spine surgery. The KOSC was first organized in The total number of participants trained is more than 100 surgeons and paramedics from within Malaysia, Hong Kong, China, Vietnam, Philippines, Myanmar, Singapore, Taiwan, Indonesia India and Pakistan. We also have fellow under the spine fellowship program attaching from one week to six months locally as well as from China, Hong Kong, Taiwan, Vietnam, Myanmar, Singapore, Philippines, India, Indonesia, Nepal and Pakistan.
