2 COMPASSION = A PASSION for PEOPLE OUR COMMUNITY IS PASSIONATE ABOUT THE HEALTH NEEDS OF PEOPLE IN OUR AREA The Cancer Center was built in 2007 with donations from throughout the communities we serve and was named after local people who spearheaded the fund drive. Today community groups regularly raise funds and donate gifts to support the people we serve.

4 OUR HOSPITAL IS PASSIONATE ABOUT BEING FOR OTHERS through Community Partnerships. Our Hospital Administration has always promoted growth in clinical and support services for cancer patients. And has expanded Screening,Wellness, Education, and Fitness Programs to assist the community in developing healthier lifestyles. MHHCC offers a variety of wellness services for area industries, community groups, and individuals.

5 Our LFCC TEAM s passionate style of continuous personal support, forms a CIRCLE of caring around each patient. When the doors opened to the cancer center in 2007, people were greeted with warm smiles, kindness, compassion, respect, an upbeat attitude, and a sense of humor. This nurturing approach let PEOPLE know at their first encounter they would be treated like friends and family. Regardless of their diagnosis or plan of care, patients had a sense of ease with each visit to the cancer center. The circle formation continues throughout every aspect of their journey during and after cancer treatment.

11 We Make every step of the journey Personal

12 Peeps work together for the good of the Whole Person Everyone on the LFCC team is aware that serious illness has brought someone to us, but we do not use this illness as an identifier. We recognize illness as part of the current life situation for the PERSON.

13 Passionate is PEEP STYLE Navigation at LFCC is not a person, but is a process that everyone on the TEAM shares to address the clinical, psychological, spiritual, lifestyle, and daily living needs of everyone who steps through our door.

14 PEEPS work to Meet every NEED We call new Patients before their 1 st appointment and let them know they can call us for any need. At every visit to see us we ask people to measure their level of psychosocial distress and we immediately review with them how we can help them reduce their stress.

15 Patients tells us what they want and need in support We ask and they let us know. From their feedback in 2008 we brought support services to the infusion room with Massage, Chaplains, and Volunteer visitors, Compassionate Healing Journaling, and a variety of creative expression materials.

16 We Gather Weekly and Bi-weekly for Group review We have a Weekly Team Huddle to discuss any issues related to clinical or psychosocial needs of patients in treatment. We have a Bi-weekly Interdisciplinary Team meeting to discuss new patients and others who have special social, emotional, or physical needs. The group includes social work, medical and radiation nursing, case management, dietary specialist, pharmacists, chaplains, palliative care and others. We plan for consultations, and how we will meet needs and assign responsibility for coordination.

17 Our Passion KEEPS US CONNECTED with our Patients and our Community Cancer Patients Survivorship Plans are shared with and include on-going wellness activities in concert with Primary care providers and other key health professionals. We formalized Survivorship planning in Survivor Support Programs and services are funded by community donations, led and assisted by Community volunteers (many are survivors).

18 Connecting Patients to Support Services and Programs At the patient s first visit our social worker gives them information about all the programs and services we have to help them during and after treatment. They are also introduced to all the services they can access through the American Cancer Society. Before they begin treatment they receive an Education Session that let s them know exactly what to expect and how we will help with any issues related to their treatment.

19 In 2008 and 2009 We began 4 evening programs Men s Discussion Group Women s Support Group Creating Hope Art Group CELEBRATE TODAY a quarterly dinner party off site Daytime groups ACS s LOOK GOOD FEEL BETTER PROGRAM Saturday FUN DAY for adults and children(in 2011) Annual winter week-end Women s Group Retreat (in 2013). Lymphedema Education Group(in 2016) After renovation in 2015 we added attractive new spaces for support services and group gatherings.

20 Our Art Group are patients giving back to other patients by creating bookmarks for all new incoming patients and a calendar for all active patients and for supporters in our community. These groups are lead by community volunteers.

21 CELEBRATE TODAY has the 4 F s Fellowship,Fun, Food, and Faith In April we had our 31 st program and served 230 people Balloons are our logo and we have a theme like country western night

23 Day and Evening Groups have a Special Place in the center. Patients chose a warm and comfortable décor.

27 What is NEXT for Patient Support? We have listened to our patients and their families. We want to find the best way to add: Caregiver Group Support Nutritional and Physical Fitness Group Support for cancer patients during and after Treatment
