2 INVITATION Dear Colleagues: It is with great enthusiasm and pleasure that I invite you to attend the Second BMP Symposium on the Extended Clinical Applications of Recombinant Human Bone Morphogenetic Protein-2 (rhbmp-2) in Philadelphia. The strong attendance and overwhelming success of the First BMP Symposium in San Diego last February underscores the interest and expanding applications of this remarkable biotechnology. By now, most oral and maxillofacial surgeons, periodontists, and craniofacial surgeons are aware of rhbmp-2 and its potential. This symposium will present both clinical outcome data and a how to technique-oriented approach to the extended applications of rhbmp-2, such as horizontal and vertical ridge augmentations, sinus augmentations, periodontal and peri-implant bone regeneration, alveolar cleft grafts, ridge split grafts, orthognathic surgery, continuity defect grafts, and rescue grafts, among others. This symposium is designed to provide you with specifi c indications, doses, use of co-materials, and placement principles, as well as an understanding of the pitfalls to avoid, drawbacks, and a discussion of complications so that the best results in bone regeneration can be achieved. The faculty for this symposium consists of the clinicians and researchers with the most thorough scientifi c and clinical experience using rhbmp-2 so that everyone will receive the most up-to-date information and see case-specifi c examples to assess their own patients as to the applications and limitations of rhbmp-2 for their own practice. I am confi dent that this meeting will deliver the same extraordinary learning opportunities that participants experienced during the First BMP Symposium last year. Marx, DDS Program Chairman ABOUT THIS COURSE After numerous animal and human research studies that fi rst began in 1965, the Food and Drug Administration (FDA) cleared recombinant human Bone Morphogenetic Protein-2/ACS (rhbmp-2/acs) for two dental and oral and maxillofacial surgery indications (socket grafting and sinus augmentation) in March of In just three short years, it has met and even exceeded its clinical promise of bone regeneration without using the patient s own bone. Extended applications into horizontal and vertical ridge augmentations, ridge splitting, implant salvage, alveolar clefts, orthognathic surgery, continuity defects, and salvaging inadequate grafts have shown that rhbmp-2/acs can replace autogenous grafting in many other applications thereby providing a reduced complication potential and morbidity for such patients. This course is designed to present the specifi cs of indications, techniques to be used, doses, co-materials, costs, and outcomes for these extended applications, by the researchers and clinicians with the most thorough scientifi c and clinical experience using rhbmp-2/acs. This course should provide the necessary information and examples so that attendees can assess each of their own patients as to the potential use and limitations in using rhbmp-2/acs.

For cancellations received on or after April 1, 2011, but before the start of the meeting, a cancellation fee of US $50 will be charged. No refunds will be made after the symposium begins on April 16, LOST BADGES Your badge represents your ticket of admittance to all lectures. If you lose or misplace your badge, you will be charged a replacement fee of US $400; there are no exceptions to this policy. You must wear your badge each time you wish to enter the lecture halls. CONTINUING EDUCATION CREDITS One hour of continuing education (CE) credit is offered for every hour of program attendance. CE credit awarded for participation in a course may not apply toward licensing renewal in all states. It is the responsibility of each participant to verify the requirements of his or her state licensing board. Meeting attendees are responsible for ensuring that their education credits are current and on fi le with their respective state board or other licensing/regulatory agency. 6 SPEAKERS Joel S. Berger, DDS, MD, is chief of surgery at Sharp Memorial Hospital in San Diego, California, where he supervises all surgical subspecialties as well as general surgery, neurosurgery, and orthopedic surgery. He has served as a member of the San Diego Cleft and Craniofacial Group; chief of dental surgery at Rady Children s Hospital; chief of oral surgery at the University of California, San Diego, in the Department of Plastic Surgery; and a senior staff member at Scripps Mercy Hospital. He also maintains a private practice in oral and maxillofacial surgery in San Diego. David L. Cochran, DDS, PhD, is currently chairman of the Department of Periodontics at the University of Texas Health Science Center at San Antonio s Dental School. Prior to his appointment at San Antonio, he was director of postgraduate periodontics at the Medical College of Virginia, where he received his DDS and PhD in biochemistry before being trained in periodontology at the Harvard School of Dental Medicine. Dr Cochran is the former president of the American Academy of Periodontology. He has published numerous scientific articles on various periodontal biochemistry and implant topics and has received awards for his research work at both the national and international levels. Alan S. Herford, DDS, MD, is the chairman and residency program director of the Department of Oral and Maxillofacial Surgery at Loma Linda University School of Dentistry. He is the author of more than 20 peer-reviewed scientific publications on topics ranging from reconstructing skin cancer defects to distraction osteogenesis. He has also written book chapters on treatment of mandibular fractures, facial flaps, and treatment of soft tissue injuries. Dr Herford has presented his original research at both national and international meetings. His clinical interests are reconstructive surgery, maxillofacial trauma, and esthetic surgery. Ole T. Jensen, DDS, MS, has a private practice in oral and maxillofacial surgery that focuses on implants and bone graft reconstruction in Greenwood Village, Colorado. He is the author and/or editor of many publications, including the new multiauthored book The Osteoperiosteal Flap: A Simplifi ed Approach to Alveolar Bone Reconstruction (Quintessence, 2010) and lectures widely throughout the world. He was president of the Academy of Osseointegration Foundation in Sascha A. Jovanovic, DDS, MS, is a course director in implant dentistry at the UCLA School of Dentistry and is chairman and founder of the gide Insititute, an online dental education company. He was formally trained in periodontics, implant dentistry, and prosthodontics at the UCLA School of Dentistry, Loma Linda University, and University of Aachen, Germany, respectively, and he holds an MS degree in oral biology from UCLA. Dr Jovanovic is the former president of the European Association for Osseointegration (EAO). He has published 63 articles and book chapters and was a contributing author to Implantology: Color Atlas of Dental Medicine (Thieme, 1995). His research focuses on bone regeneration, rhbmp-2 growth factors and barrier membranes, and the biology of soft tissue around teeth and implants. Jay P. Malmquist, DMD, is an associate professor of oral and general pathology and oral and maxillofacial surgery at Oregon Health Sciences University in Portland, Oregon, where he also maintains a private practice limited to oral and maxillofacial surgery. He is a diplomate of the American Board of Oral and Maxillofacial Surgery, the National Dental Board of Anesthesiology, and the International Congress of Oral Implantologists.
7 Robert E. Marx, DDS, is professor of surgery and chief of the Division of Oral and Maxillofacial Surgery at the University of Miami Miller School of Medicine. His areas of research include the use of hyperbaric oxygen following radiation therapy, the development of platelet-rich plasma, and the relationship between smoking and carcinogenesis. Dr Marx has received numerous awards, including the Harry S. Archer Award, the William J. Gies Award, the Paul Bert Award, and the Donald B. Osbon Award. His fi rst textbook, Oral and Maxillofacial Pathology: A Rationale for Diagnosis and Treatment (Quintessence, 2002), won the American Medical Writers Association s Medical Book of the Year Award for Craig M. Misch, DDS, MDS, received postgraduate certificates in prosthodontics and oral implantology and an MS degree from the University of Pittsburgh. He completed specialty training in oral and maxillofacial surgery at St Francis Medical Center in Pittsburgh and is a diplomate of the American Board of Oral and Maxillofacial Surgery. He currently practices as a dual specialist in Sarasota, Florida. Dr Misch is a clinical associate professor at New York University in the Department of Implant Dentistry and at the University of Florida in the Department of Prosthodontics. He serves on the Board of Directors of the Academy of Osseointegration and on the editorial board of The International Journal of Oral & Maxillofacial Implants. Peter K. Moy, DMD, is an associate professor of clinical dentistry in the Department of Oral and Maxillofacial Surgery as well as the director of the Nobel Biocare Surgical Fellowship Training Program in Implant Dentistry at the University of California Los Angeles. He also maintains a private practice limited to oral and maxillofacial surgery, primarily in the areas of osseointegrated dental implants and the reconstruction of severely atrophic ridges. Dr Moy has lectured nationally and internationally, has written numerous articles on implant dentistry and osseointegration, and serves as an associate editor for The International Journal of Oral & Maxillofacial Implants. Daniel B. Spagnoli, DDS, PhD, works in private practice as an oral and maxillofacial surgeon with offi ces in Concord, Denver, and Charlotte, North Carolina. He recently completed four research projects, three of which concerned the use of rhbmp-2 in grafts for maxillofacial bone defects. His interests include all aspects of temporomandibular joint treatment and surgery as well as reconstructive procedures of the oral and maxillofacial region, including bone grafting, distraction osteogenesis, dental implants, and orthognathic surgery. Dr Spagnoli has published numerous book chapters and research articles related to oral and maxillofacial surgery and basic science. He has also been a section editor of the American Association of Oral and Maxillofacial Surgeons Knowledge Update textbook since 1992 and editor of the oral and maxillofacial surgery section of the Cleft Palate Journal since William K. Tom, DDS, MS, is an oral and maxillofacial surgeon dedicated to treating patients with congenital and facial deformities. In addition to his private practice, he is a member of the craniofacial team at Children s Hospital and Research Center in Oakland, California. Currently, he is involved in the study of new tissue engineering materials, which he believes may reinvent surgical techniques used worldwide to treat congenital defects. Dr Tom is a diplomate of the American Board of Oral and Maxillofacial Surgery.
