The Health professional will review the results with the individual patient. The patient will receive a counseling brochure at the screening with their results. 2
3 BLOOD PRESSURE/HYPERTENSION & PULSE RATE We provide blood pressure and pulse rate screenings for individuals. A nurse will review results with the patient and provide a counseling brochure with blood pressure and pulse rate guidelines after the screening. CHOLESTEROL PANELS Our cholesterol screenings are done by using a fingerstick. Results are available in less than 10 minutes and reviewed with the individual at the screening. Our cholesterol screening options include:! Total Cholesterol! Total Cholesterol and Glucose! Total Cholesterol and HDL! Total Cholesterol, HDL and Glucose! Lipid Profile (Fasting Comprehensive Cholesterol Test) This test requires a 12 hour fast! Total Cholesterol! HDL (high density lipoprotein)! LDL (low density lipoprotein)! Triglycerides! Ratio of Total Cholesterol to HDL! Lipid Profile and Glucose (Biometrics)! Mini Biometrics! Total Cholesterol! Glucose! BMI! Height/Weight! Blood Pressure! Small Site Biometrics DIABETES The diabetes screenings use a blood glucose monitoring system to conduct the test. Individuals receive a counseling brochure and results at the time of the screening. HEMOGLOBIN A1c This test measures the patient s blood sugar for the last 2 3 months. Results are reviewed at the end of the screening. BODY MASS INDEX (BMI) PAPER BASED The Body Mass Index screening is a paper based calculation performed by acquiring the patient s height and weight. The patient will receive a results sheet with their BMI and also a height and weight chart that indicates their target weight. 4 VISUAL ACUITY Vision Acuity Screenings measure the acuteness or clearness of your vision. Visual Acuity is a quantitative measure of the ability to identify black symbols on a white background at a standardized distance as the size of the symbols is varied. It is the most common clinical measurement of visual function. BODY COMPOSITION AND BODY MASS INDEX (BMI) The Body Composition screening determines the estimate of total body water which is used to calculate the fat free mass and body fat. The Body Mass Index screening is determined by using a Body Fat Analyzer. BONE DENSITY/OSTEOPOROSIS The Bone Density and Osteoporosis screening measures an individual s bone mass and indicates the risk (if any) for osteoporosis. Results are reviewed at the end of the screening and patients will receive a counseling brochure at the end of the test. SKIN CONDITIONING (DERMASCAN) The skin conditioning screening uses a non harmful ultra violet light to determine the sensitivity, dryness and conditions of an individual s facial skin. Sun, soaps and other pollutants can have a negative impact on skin, which could eventually result in skin cancer. This test is completely safe and allows both the health care staff and patient to personally view the condition of a individual s facial skin. Each person will receive a counseling brochure showing the condition of his or her facial skin. FATAL VISION Fatal Vision is a specially designed and manufactured pair of goggles that simulate the visual impairment caused by alcohol or other drugs. Viewing through the goggles is rather clear, but confusing to the mind. The wearer experiences a loss of equilibrium, which is one of the effects of intoxication. Both the wearer and those observing are convinced that such impairment makes activities with known risks, such as driving vehicles, very dangerous. HOME TEST EMPLOYEE ADMINISTERED The Home Test screens for Lipid Profile (Total Cholesterol, HDL, LDL, Triglycerides) and Glucose. It is mailed to the employee s home or office, where the employee self administers the test by placing blood on a specially treated paper the size of a credit card. The employee then mails the specimen in a self addressed and pre paid envelope directly to the lab for processing and reporting. The entire process is safe and easy, and the employee may call an 800 number at any time for more information and support. After processing the sample, the lab mails the test results and interpretation back to the employee. Everything is conveniently included in the Home Test kit, including forms and instructions, fingerstick supplies, and a pre addressed, pre paid mailing envelope for the blood sample. 4
5 ASTHMA/COPD/LUNG AGE ASSESSMENT Spirometer is an instrument used for measuring lung capacity. Such a device is extremely important to the medical community. Spirometery is valuable in assessing the risk of common diseases such as lung cancer, heart attacks and strokes. Measurements from the screening include Forced Vital Capacity, Expiratory Flow Rate, Chronic Obstructive Pulmonary Disease (COPD) Risk Assessment and Lung Age Assessment. A Health professional will review the results with the patient. Each patient will receive a printout of 15 indices derived from the test. THERAPEUTIC MASSAGE Licensed massage therapists come to your place of work and conduct chair massages for employees. An on site chair massage relieves tension, relaxes the mind and helps get employees through their day. FLU, PNEUMONIA AND OTHER IMMUNIZATIONS! Flu Shot! Pneumonia Shot! Tetanus! Tdap! TB test! Hepatitis A! Hepatitis B! MMR! Travel vaccines Wellworks for you has a network of over 300,000 nurses available all across the united states. 5
6 FITNESS ASSESSMENT PACKAGE The fitness assessment package includes the following:! Blood Pressure and Pulse Rate Screening! Height and Weight Measurements! Body Composition and Body Mass Index (BMI)! 3 Minute Step Test! Sit and Reach Test! Handgrip Test! *Push Up Test! Consultation (2 to 3 minutes to review results and answer questions) *Sit Up Test may be substituted in place of the Push Up Test at the client s request. The participant will receive their results at the fitness assessment. The Health fitness professional will review the results with the individual participant. The screening takes approximately 20 minutes for each individual participant. The participant will receive a counseling brochure at the screening with their results. 3 MINUTE STEP TEST The 3 Minute Step Test is a commonly used test to measure an individual s cardiovascular endurance. Aerobic capacity reflects the functional capabilities of the heart, blood vessels, blood and lungs. A prediction of the participant s aerobic capacity will be calculated by measuring the participant s one minute recovery heart rate. The quicker the heart recovers, the more fit the individual. The participant s results will be compared to national guidelines to determine the individual s cardiovascular endurance compared to general population. SIT AND REACH TEST The sit and reach test measures the participant s flexibility of hamstrings, lower back and shoulders. This test is administered using a sit and reach box. The participant will sit in front of the box and reach forward as far as possible. The participant s results will be compared to the age and gender norms. HANDGRIP TEST Muscular strength will be measured using a handgrip dynamometer. Both the left and right hand will be measured (in kilograms) and the combined score will be recorded. The results are compared to normative data by age and gender. PUSH UP TEST The push up test is easy and quick to administer. The push up test measures a participant s muscular endurance. The total number to push ups completed consecutively is counted and recorded. These results are compared to norms to summarize the participant s overall muscular endurance. *Note: A sit up test may be substituted in place of the push up test upon request. 6
7 HEIGHT/WEIGHT & HIP/WAIST RATIO The height and weight of an individual is used to calculate their Body Mass Index. The hip/waist ratio is the ratio of the circumference of the waist to that of the hips. It is calculated by measuring the smaller circumference of the natural waist, usually just above the belly button, and dividing by the hip circumference at its widest part of the buttocks or hip. It is used as an indicator or measure of the health of a person, and the risk of developing serious health conditions. COTININE SALIVA TEST Cotinine (a metabolite of nicotine) can be detected in saliva to determine if a person uses or has been exposed to any form of tobacco, including nicotine replacement therapy (patch, gum, or inhaler). The examiner collects a sample of saliva by inserting the swab between the patient s cheek and gum for 2 5 minutes. The pad is placed in a vial that contains preservatives to stabilize the sample during transport to the clinical lab. After processing, the lab electronically uploads the Positive or Negative result for subsequent reporting. The saliva swab is a fast, easy, and accurate alternative to cotinine measurement by venous blood draw or urine sample. Oral conditions, disease and medications, will not impact the accuracy of the oral specimen collection device or its processing. Ask us about our additional screening Offerings! 7
