Ovulational migraine is a common cause of lower work productivity and sickness absence. It also lowers the quality of life for young, productive women and thus has a significant influence on economical and social status and economy in general. Connection between hormonal status and blood vessel dynamics during ovulational migraine could open a pathway to new therapies and better prevention. Migraine affects up to 12 percent of the general population [1]. Prior to puberty, migraine occurs in 2.5 to 4 percent of children, with a slightly higher prevalence among boys [2]. The prevalence changes with the onset of puberty, at which time migraine becomes markedly more prevalent in females than males [3], with attacks occurring in up to 17 percent of women and 6 percent of men each year [4,5]. This change has been attributed to hormonal fluctuations associated with pubertal maturation that precede the characteristic hormonal pattern observed in ovulatory women. The observation that migraine is predominantly a female disorder and that several reproductive milestones correlate with a change in migraine frequency or type, implicates sex hormones in the pathogenesis of migraine. Estrogens may interfere with cellular excitability of cerebral vessels. Ovarian steroids cross the blood brain barrier by passive diffusion, with brain levels mirroring blood levels [6] and are also produced within the central nervous system [7]. Estrogen and progesterone can influence the pain-processing networks and the endothelium involved in the pathophysiology of migraine. Interrelationships between estrogens and brain neurotransmitters have been confirmed, including serotonin, norepinephrine, dopamine, and endorphins [8,9]. Numerous studies have shown that the level of sex hormones has a significant impact on the incidence of migraine headaches. Headaches are more common in women, especially at the time of ovulation. It is known that female sex hormones have impact on vessel dilatation, but is so far not determined if they influence only the arterial or venal system. Goals of the study The aim of the study is to determine connectivity of the changes in serum concentrations of female sex hormones with blood flow and vessel wall dynamics in regard to ovulational migraine headaches. No study so far determined if female sex hormones influence only the arterial or venal system. Since it is mostly the influence of estrogen on dilatation of blood vessels, we were specifically interested in a flow through basilar artery at elevated concentrations of estrogen at the time of ovulation. Another interest of the study is how the concentrations of estrogen influence the cross-section and the blood flow of basal artery during the headache. The hypothesis is that the hormone state has influence on the blood flow
7 through basilar artery and thus among other factors causes ovulational migraine. Furthermore, in women with no migraine the change in the blood flow throuh basilar artery should be significantly less affected then in women who are suffering from migraine. The potential impact of the study Modern neuroscience advanced our understanding of ovulational migraines. Consequently, an improved understanding leads to improved therapeutic approches. Treatment of ovulation migraine will give women a higher quality of life. This means less sickness absence from the workplace, greater efficiency and fewer visits to the family doctor. This would lower the impact of the disease on the economy and on the health care sistem. It will also increase the productivity of the population. It will enable the development of new therapeutic approaches and pharmacological agents. On the other hand the greatest opportunity to advance in the field of ovulational migraine pharmacology will be in prevention. This offers many challenges in the future and new hope for women. With these advances, ovulational migraine will become more effectively managed. The results could also influence treatment of other forms of migraine and other types of headache and hopefully even other disorders, including vascular and neurovascular disorders. Of great importance is the development of clinical pathways for women with migraines. Adequate assessment will reduce the impact of this disease, which mainly affects women in family and career enviroment. It will also allow an effective clinical protocol that would lower the burden of the numerous patients on other levels of healthcare system. Work methods We already gained the approval of the ethics comitee regarding this study. All women included in the study must have a signed informed consent statement. The imaging techniques used posess no risk regarding ionising radiation. We will study a sample of 100 healthy women. Women included in the study must be years old with regular menstrual cycle. Exclusion criteria would be hormonal contraceptives, unregular menstrual cycle, alergy to gadolinium and endocrinological or gynecological disorders, vertebral disorders or vascular disorders. The control group of women are women who do not suffer from ovulatonal migraine with the same exclusion and inclusion cryteria. We will develop a clinical pathway to connect both neurological and gynecological outpatient clinics. Women suffering from ovulational migraine would then be appointed into a specialized department for further diagnostics. To measure the blood flow velocity and vascular diameter in a. basilaris we will use an MRA. Our protocol is to conduct three MRA imagings. First MRA will be conducted 10 days after the first day of the last period. The second MRA will be conducted during the ovulational migraine headache and the third MRA will be conducted three days after the cessation of the migrain headache. The blood flow velocities and the vascular diamether will then be compaired in order to asess any significant differences. Ovulation will be confirmed by the specific changes in the blood levesl of FSH, LH, estrogen and progesterone. The levels will be controlled every day to determine a specific phase in a menstrual cycle. The results will then be compaired with the measurements of women in the control group, who will undergo the same protocol. However, since there will be no ovulational migraine headache, the second MRA will be conducted at the time of ovulation as observed from the laboratory findings.
