2 Each group is asked to bring 1) snacks/drinks for their group during break times, 2) canned goods - primarily fruit - for a food pantry donation, and Gal. 5:22-23 What: ArchLou CYC (Catholic Youth Conference) is a 1-day event for high school youth from throughout the Archdiocese including a nationally known keynote speaker, relevant break-out sessions, Mass with Fr. Adam Carrico, youth-led prayer, large-group fun, a service project, a visit from Archbishop Kurtz, and more! When: Saturday, March 2, 2019, 9:00am-9:00pm Where: Flaget Center (1935 Lewiston Dr., Louisville) Who: All High School Students! Cost: $40/PERSON (includes lunch and dinner) Late Registration: $45/person Group Registration Deadline: Monday, February 11, 2019 Further information: - Groups must have one adult leader for every 10 youth participants. - Adult and Youth Registration Forms are due at event check-in. - Groups traveling from longer distances who would like overnight accommodations on Fri, Mar 1, and/or Sat, Mar 2, at the Flaget Center ($25/person/night) or at a local parish, please contact the Office of Youth & Young Adults for assistance with arrangements. Violation of any of these rules are grounds for a participant s immediate dismissal from the conference and the calling of the youth s parents/guardians to pick them up from the conference. Guidelines for Group Leaders/Adult Leaders/Chaperones: All adults are responsible for helping to enforce the Code of Conduct. Please review the Code of Conduct with all youth and adult participants with your group. Each adult is responsible for the youth from their parish/school group throughout the event. All youth and adults are expected to attend all scheduled activities. Each group leader must provide their own first aid kit. Medical assistance will be available during the event. Adult s are to refrain from drinking alcoholic beverages and using tobacco products for the duration of the event. Adult leaders/chaperones do not have the authority to adjust schedules or change the Code of Conduct. Adults should sit with their parish/school group during general sessions. Adults should not hesitate to give appropriate guidance to any youth who may be disrupting/distracting others. The group leader should pick up registration packets for their entire group by 9:30am, Sat, Mar 25. As an adult leader/chaperone attending the ArchLou CYC, I have read the Code of Conduct and agree to enforce
5 Gal. 5:22-23 Youth Permission & Registration Form I,, parent/guardian request that my child,, be allowed to participate in ArchLou CYC (Catholic Youth Conference), a day-long assembly including keynote presenter, workshops, prayer, Mass, and music for high school youth on Saturday, March 2, 2019, at the Flaget Center (1935 Lewiston Dr., Louisville, KY). I further give my permission for my child to ride in any vehicle designated by the adult in whose care my child has been entrusted while participating in this activity. In consideration of permitting my child to attend and/or participate, I do hereby, for myself and my child waive and release any and all claims that I might have against the Archdiocese of Louisville, the Office of Youth and Young Adults, the Flaget Center, or their respective staff members, and any designated driver of a van, bus, car or vehicle, for any and all injuries or losses suffered by said child. I further waive and release any and all claims against the owners, leasers or operators of any facilities or homes used for the meetings for any and all injuries or losses suffered by said child. I also give my permission for my child to appear in photographs/video of this event for use in Archdiocesan print publications and digital media. In case of medical emergency, I understand that every effort will be made to contact the parents or guardians of the ArchLou CYC participant. In the event I cannot be reached, I hereby give permission to the physician selected by the adult in whose care my child has been entrusted to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery for my child, as named herein.
