1 PROGRAMME BUSINESS CASE (PBC) SUBMITTED TO WELSH GOVERNMENT The PBC was submitted to Welsh Government at the beginning of July. It sets out how we intend to develop our community infrastructure over the coming years to support the implementation of the SOFW Strategy and promote the physical, mental and social wellbeing of our residents. The PBC sets out the capital projects required and an indicative timeline for implementation. Each project will need to be supported by an appropriate business case to be approved by WG prior to the release of capital funds to construct the facility. In some cases, the size and complexity of the project will require a series of business cases which demonstrate, at each stage of the development, the decision making process, the costs, benefits and risks of the project and how these will be managed. This will provide assurance that the development offers the best value for money. Our plans include the development of a network of community based infrastructure including:- a Health and Wellbeing Centre (H&WC) for each locality Vale, North and West Cardiff, South and East Cardiff a (WH) for smaller clusters of GPs, colocated with Council wellbeing facilities where possible fit for purpose primary care premises potential to use non-health facilities where appropriate Community facilities rationalised where appropriate Wellbeing HUB

2 How will the community facilities differ? Health and Wellbeing Centres (Locality): a Health and Wellbeing Centre is proposed for each of the three Localities (1 in the Vale and 2 in Cardiff). s (Cluster): locations will take into account: deprivation within the cluster/local community; travel access; and opportunity to deliver collaborative services with the Local Authority and third sector. Unsuitable community facilities may be closed where necessary. WHAT WILL LOCALITY AND CLUSTER FACILITIES DELIVER? While the services offered at each facility may vary, depending on the needs of the local population, there will be many similarities:- Health and Wellbeing Centres will include: Diagnostic and investigation facilities; Clinics to support shift of services from hospital to community; and Some services for a wider population, where critical mass is required. Both Health and Wellbeing Centres and s will: Deliver local access to health and wellbeing education, information and advice; Promote a social model of health (physical, mental and social wellbeing) in collaboration with our partners; Deliver local services which meet the specific health and wellbeing needs of local residents; Provide fit for purpose facilities that are flexible and multi-functional; Support services to improve utilisation, efficiency and sustainability.

4 The First Tranche of Projects Primary Care Pipeline Fund Existing Llanedeyrn HC site Maelfa Powerhouse Community Hub Proposal: adjacent to the Powerhouse Community Hyb, primarily to serve residents of Llanedeyrn and Pentwyn. Service Scope: Replacement of Llanedeyrn Health Centre. Services to include Llanedeyrn GP Practice; wellbeing information, advice and education delivered in partnership with third sector groups; range of clinics e.g. counsellors, podiatry, dietetics, addictions, child health stop smoking, midwifery, audiology, heart failure, physiotherapy; team bases. Progress: Over the last 12 months we have engaged with local residents, clinical services and partner organisations to develop the vision and service scope for the wellbeing hub. A schedule of accommodation has been prepared which will be shared with the recently appointed design team to inform the development of plans. Timescale: An Outline Business Case will be submitted to Welsh Government in Spring We will also be required to complete a Full Business Case for approval by Welsh Government prior to starting building work with a view to opening the wellbeing hub by the end of Penarth Proposal: adjacent to Penarth Leisure Centre developed in partnership with Vale of Glamorgan Council and the third sector to serve Eastern Vale residents. Draft Service Scope (to be confirmed): Relocation of Redlands and Albert Road GP Practices; wellbeing information, advice and education delivered in partnership with third sector groups; consolidation of health clinics and teams who serve the Eastern Vale; e.g. district nursing, child health, sexual health, pain management, family services and adult social care; heart failure, midwifery, podiatry, mental health, dietetics, physiotherapy and others. Progress: The draft service scope is currently being tested to determine if it can be delivered within the available capital. We will shortly be talking to local residents to find out what will make this facility successful and a good place to visit. This will inform the plans to be developed by the recently appointed design team. Timescale: An Outline Business Case will be submitted to Welsh Government in Spring We will also be required to complete a Full Business Case for approval by Welsh Government prior to starting building work with a view to opening the wellbeing hub by the end of 2021.

5 The First Tranche of Projects All Wales Capital ParkView Community Hyb Proposed Proposal: Wellbeing hub adjacent to the Ely/Caerau Community Hyb to serve the local population. Service Scope: Replacement of Park View Health Centre which was recently forced to close following severe flooding; relocation of Westway GP Practice; information, advice and education delivered in partnership with third sector groups; range of clinics e.g. sexual health, child health, district nursing, stop smoking, dietetics, podiatry, dental, learning disability, cardiology, midwifery, falls prevention; team bases. Progress: Following discussion with the local community and staff, we have agreed the scope of services to be delivered along with the associated schedule of accommodation. A meeting with Welsh Government is planned shortly to agree the appropriate business case route and funding to appoint a design team. Timescale: Implementation timescales to be identified once the business case route is agreed. Health & Wellbeing CRI RELOCATION OF SARC AND ENABLING WORKS Proposal: SARC to move to a more appropriate location on the CRI site, temporary modular accommodation for the Community Addictions Unit and Locality CMHT. Progress: Outline plans for the new facilities have been prepared. SOC/OBC/FBC route agreed with Welsh Government. The SOC is in development for submission to Welsh Government in September Once approved, a design team will be appointed to progress plans to the next stage of development. Timescale: Following the appointment of the design team, timescales for submission of the OBC and FBC will be determined. CHAPEL DEVELOPMENT Proposal: Development of a community facility incorporating a café, library, IT and meeting rooms. Progress: Plans developed. Detailed capital costs in development to inform the Business Justification Case. Timescale: Submission of the BJC to Welsh Government December Once approved, implementation timescales will be determined.

6 EQUALITY HEALTH IMPACT ASSESSMENT (EHIA) The proposals within the programme have been assessed to determine their impact on residents and service users and actions required to strengthen positive impacts and mitigate negative impacts. The assessment found that the programme offered significant opportunities to make positive changes in the way we deliver services and the facilities in which they are provided. This will have a beneficial impact for people, especially those with protected characteristics and those with greatest health need. The EHIA will inform key stages in the programme development to ensure that the proposals promote equality and positive health outcomes for all.
