1 Nevada County Mental Health Board Minutes Date: November 3, 2017 Time: 9:30 a.m. 12:00 p.m. Place Behavioral Health Department 500 Crown Point Circle, Grass Valley STANDING ORDERS 1. Call to Order Self-introductions were made and a sign in sheet was passed around. 2. Public Comment. Members of the public may address the board on items not appearing on the agenda. Please understand that no action shall be taken on items not appearing on the agenda. A visitor is trying to get help for her mentally ill son. Rebecca Slade will get her contact information and contact her by phone. 3. Announcements. There was a recent Mental Health Services Act Meeting on October 30 th. There will be a Cultural Competence Training on Transgender Care on November 30 th. See the flyer included in today s handouts for more information. Registration is required, contact Annette LeFrancois to register. Additional CalFresh and CalWORKS assistance is available to recent fire victims. See the attached flyer for more information. 4. Change December Mental Health Board Meeting to Friday, December 8 th. Under the new Mental Health Services Act (MHSA) requirements, Behavioral Health should submit the MHSA Plan by December 31 st. Our 30 day public comment period begins today November 3 rd and ends on December 3 rd. The public hearing must take place after the public comment period ends, so we need to change the Mental Health Board Meeting date to the following week on Friday, December 8 th. There is also only one Board of Supervisor Meeting in December on the 12 th. A decision was made to move the December Meeting to December 8 th, Minutes from October 6, There is a correction to the October minutes. The Gun Violence Restraining Order Law on page four, under the fourth paragraph add: It is modeled after Connecticut Law where it is used most for suicide prevention. Amanda Wilcox made a motion to approve the minutes with the correction. The motion was seconded by Shera Banbury. 6. Mental Health Services Act 3-Year Plan Review Michele Violett. Handouts. Behavioral Health is hoping to get the Mental Health Services Act (MHSA) Plan approved by the Board of Supervisors on December 12 th in order to submit it to the Mental Health Services Oversight and Accountability Commission and the Department of Health Care Services by December 30 th. Michele Violett briefly reviewed the plan sections. Exhibit A is the MHSA County Compliance Certification to be signed by the Behavioral Health Director. Exhibit B, the County Fiscal Accountability Certification is signed by the Behavioral Health Director and the Auditor Controller. 1

2 Exhibit C, the first page is a funding summary that lists funding for each year by category. There was a discussion on unspent funds. MHSA funding is volatile and it is recommended to have reserve and unspent funds to be able to sustain programs. Behavioral Health has less than ½ year s funding. Exhibit D is The Community Stakeholder Planning Process. There have been several MHSA meetings throughout the year. Michele Violett also visits agencies (SPIRIT, Hospitality House, FREED) and attends community organization meetings to ask about the mental health needs of their clients. On page 23 is a summary of any substantive recommendations received during the public comment period with a description of substantive changes made to the plan. Due to the feedback Michele has received from the public, if there are no substantive comments, she will change the wording to: No changes to the plan were made based on the comments received. Exhibit E is the MHSA Plan. If a program or item we want to fund is not in the plan, we cannot spend MHSA funds on it. Exhibit E refers to Exhibit F: Recommendations of Needed Mental Health Services. Michele Violett has included housing services in community services and supports (CSS) and prevention and early intervention (PEI). Beginning on page 29 is General System Development. We deliberately use generic language to allow us the flexibility to make changes as needed. Exhibit F was re-done this year for the new 3-year plan. Truckee s needed mental health services were re-written after input from their community planning process. The last section of the plan is Exhibit G, the Annual Progress Report for Fiscal Year 2015/16. Each program is listed in the report and includes a client story along with outcomes. Michele Violett asked the Mental Health Board for their approval to post the plan for 30 day public comment and hold a public hearing on December 8 th. Shera Banbury made a motion to move forward with the MHSA Plan. The motion was seconded by Ann Kelley. All members present were in favor. 7. Mental Health Services Act Innovation Plan Discussion Michele Violett. The Crisis Continuum of Care Innovation Plan was not approved by the Mental Health Services Act Oversight and Accountability Commission (MHSOAC). Behavioral Health needs to come up with a plan that is new and innovative. There were some topics brought up: homelessness, peer support, mentoring and youth, but they must be innovative. You can any ideas you have to Michele Violett. There is approximately one million dollars that can be used for an innovation project. Ideally there would be one plan. Behavioral Health is already overwhelmed with managing the programs it already has. Iden suggested developing a plan that would engage mentally ill homeless people. It is very difficult to get them to want to accept help and treatment. Shera Banbury mentioned there are some students attending Sierra College who are homeless. Another suggestion that was mentioned at the MHSA Community Meeting were alternative treatment services for adults (something like what Mountain Gateway Center does with youth). There are a lot of adults who do not want to come in for treatment. What can we do to outreach to these individuals. Homelessness is one of the largest needs we have in the community. Substance use is another concern in the community. Programs that help Transition Age Youth are also needed in our community. Behavioral Health will contract with Nancy Callahan to write our Innovation Plan. Behavioral Health will contact the MHSOAC for their guidance early on in the process. There was a suggestion that we look at other County Innovation Plans that have been approved and adapt one to fit our needs. Another suggestion was to meet with Sierra Roots and Divine Sparks. 2

3 8. Truckee Michele Violett. Michele Violett, Phebe Bell and Nancy Callahan are meeting to brainstorm and come up with a solution in getting and retaining a Behavioral Health case manager for Truckee. 9. Peer Support Maureen Gerecke. Maureen Gerecke and Julie Lang will be working on WRAP with a group of female inmates. There are about 15 people in the next group. SPIRIT Peers for Independence and Recovery Center has applied for a $10,000 grant to connect with individuals on probation and individuals released from jail for WRAP, peer support and services. SPIRIT will be holding a WRAP Facilitator Training in December. Michele Violett announced all of the WRAP Books have now been turned over to SPIRIT. 10. Forensic Task Force Iden Rogers and Helen Williamson. The Stepping Up Program is moving forward slowly. There is no grant funding for this program. Mental Health Court is near capacity of people. The more people in Mental Health Court the larger the case management costs are. Behavioral Health does not get reimbursed for these costs. Vivitrol (30 day alcohol anti-abuse medication) shots are sometimes painful and individuals may decide against continuing with it. Lynn Demartini a doctoral candidate is researching individuals with severe mental illness who have been in jail or prison and how incarceration has affected their mental illness. Interviewees who agree to the minute interview are given $25. Iden Rogers if you know someone who is willing to be interviewed. PC 1368 Competency to Stand Trial The Public Defender has been researching this topic and may present information at an upcoming Forensic Task Force Meeting. There was discussion on potentially using one of the Juvenile Hall Pods for PC The recent Crisis Intervention Training for Law Enforcement, dispatchers were not able to attend, but should be invited to any future training. A resource guide is handed out to those released from jail. There has been interest in mindfulness and yoga groups at the jail. Placer and Sierra Counties have been using beds in the Crisis Stabilization Unit. The Emergency Department at the Hospital has had some bad experiences with disturbed individuals that have staff concerned. AB 1299 shifts funding from where a foster youth entered care to the county where the foster youth reside. Due to the number of group homes in Nevada County this will be a financial burden to Behavioral Health. 3

4 11. Mental Health Board Questions Rebecca Slade. How does NCBH define a client s support system? How are support systems engaged and optimized by NCBH staff and contractors? Rebecca Slade reviewed the Nevada County Behavioral Health Assessment Follow-up form. This form is completed at the first session and given to the client. It contains basic information such as who was the assessment therapist and when the client can expect to hear from the therapist regarding eligibility. A list of recommended community resources is also handed out. There is space for the client to indicate a supportive person and whether or not the therapist can speak to them. Behavioral Health does not define a client s support system; the client defines their support system. When a client turns 18, they decide who Behavioral Health can speak with. This is difficult for families who often feel they lose their ability to be part of the treatment team. The Mental Health Board asked for phone call statistics. Rebecca Slade will bring them to a future meeting. This topic is part of the Quality Improvement Committee. The next meeting is on Monday, December 18 th at 1:00 p.m. at the Behavioral Health Department, Crown Point Conference Room. How and to what extent, does NCBH and contractors do mental illness and medication education for families? Behavioral Health does not have formal training (i.e. classes), but if a client s support system is present at an appointment they are included. There was a suggestion to have a medication support group. There are some online chat groups, but they may not be well suited to families and consumers who are new to mental illness and medications. Psycho- Babble Medication by Dr. Bob: There was a suggestion to the nurse contact the client s support when there is a transition in medications. 12. Behavioral Health Director s Report Rebecca Slade. Handouts. The State has received a SAMHSA FEMA Grant to put crisis counseling in communities that have been affected by fires. Nevada County will be getting the equivalent of 2 FTE s and a part time supervisor for 30 to 60 days. SPIRIT staff will be trained and under the guidance of the Office of Emergency Services. Behavioral Health is placing two Turning Point case managers at Hospitality House with a contract that was recently approved by the Board of Supervisors. Rebecca Slade has been looking at properties for No Place Like Home that look promising. The Crisis Stabilization Unit has been busy. The State is coming to look at Bost Avenue House and will potentially give us a license. We hope to open Bost Avenue House by mid-december. The contract goes to the Board of Supervisors at their next meeting. The Steinberg Institute is visiting Nevada County on Monday. Behavioral Health has been getting hit pretty hard by social media. The Crisis Intervention Team has Behavioral Health, Probation, and Law Enforcement outreach to those who are homeless. They have been successful in getting 3 clients housed. 4

5 13. Mental Health Services Act (MHSA) Report Michele Violett. Handout. The handout is a copy of the Mental Health Services Act (MHSA) Report. The fiscal unit is working on past Revenue and Expenditure Reports to determine our revenue balance and any revenue in danger of reverting. AB 114 provides that all unspent MHSA funds subject to reversion as of July 2017 are deemed to have been reverted and reallocated to the county of origin. Behavioral Health received a letter from the Department of Health Care Services (DHCS) listing MHSA funds from our county that were subject to reversion. The fiscal unit must first finish all of the revenue and expenditure reports and review to make sure they are correct in order to determine if the State s report is correct. If we have any funds that reverted we must develop a plan to expend the funds by July 1 st MHSOAC Letter from September 2017 per new guidelines Behavioral Health is required to report data for Prevention and Early Intervention (PEI) and Innovation programs from July 2016 to June 2017 by December 30 th Behavioral Health has never been able to collect data this quickly. In order to get a waiver we must report on data we were able to collect. Provide an explanation of the obstacles we face in meeting the requirements. Obstacles: Data must be collected, analyzed, assembled into a report, posted for 30 day public comment period, hold a public hearing, and obtain approval from the Board of Supervisors by December. We must also provide a summary of the county s implementation plan and timeline for fully complying with future Innovation and PEI Reports. MHSOAC Letter from October 2017 is about excluding Protected Health Information (PHI) from the Annual Progress Reports and submitting a Supplemental Report with the excluded PHI through a secure File Transfer Portal. A soft launch of Coordinated Entry for the homeless population in Placer and Nevada County will occur on or around December 11 th. There will be training for service providers on November 16 th. All individuals that utilize Hospitality House Emergency Shelter, Hospitality House Rapid Rehousing and Nevada County s three Continuum of Care Programs will have to be in the Coordinated Entry System to be a program participant. Connecting Point (211) will be the screening point of entry for both Placer and Nevada County. Joyce Peterman from Nevada County Housing Development Corporation (NCHDC) has retired. The Alliance for Mentally Ill Housing (AMIH) CEO, Jennifer Price is now CEO of both organizations. Jennifer is doing the administrative part of the HUD housing activities. Jeffrey Cowen duties are the day to day operations. Michele Violett will invite Jennifer to a future Mental Health Board Meeting. 14.
