3 04 / YEAR IN REVIEW YEAR IN REVIEW / 05 YEAR IN REVIEW Hands Annual Report reflects the activities of our organization to deliver on our Mission, as well as the commitments of the Board of Directors to our funders and communities. This is expressed in the report by both: n Data, which reflects our productivity; and, n Stories of our clients on their service journey with us and our partners. We are proud of this story and the hard, client-centered work of staff to make a difference. THE ENVIRONMENT The work and service delivery of our staff has occurred in a whirlwind of service re-design, change and uncertainty in our external environment. Our ability to support these directional changes results from: n Partnering actively at local, regional and provincial levels; n Flexibility by staff leadership and the Board of Directors to balance our service delivery focus with the changing environment; and, n The diligence of our Board of Directors, and their ability to fulfill the Board s stewardship role. STRATEGIC DIRECTIONS In the context of this transformational time, we have undertaken the development of a new Strategic Plan to assure our responsiveness and alignment with the service directions going forward. Key focuses include: n Strengthening relationships with our partners in the spirit of a strong, integrated system. This includes assuming the Lead Agency role for child and youth mental health services in the Nipissing Parry Sound Muskoka Service Area. n Building our internal capacity for greater efficiency, effectiveness and the ability to be nimble in response to a shifting service environment. We are undertaking a significant organizational design review that will help us to build on our strengths and maximize our resources. n Ensuring our clients have the respectful service environment they deserve and, that our staff have the space that allows for good work. In 2015, we completed our Parry Sound site renovations. In the coming year, we will be realizing the final step of our Space and Place initiative with the construction of our North East Ontario Family Help and Treatment Centre in North Bay, scheduled to begin in the summer of THANK YOU! 5 08 / AUTISM AUTISM / 09 HANDS IS A LIFESAVER! I AM SO INCREDIBLY THANKFUL TO HANDS FOR ALL THE SUPPORT THEY HAVE GIVEN ME OVER THE LAST 13 YEARS. PARENT, AUTISM AUTISM Hands offers services to support families who have a child or youth with Autism Spectrum Disorder (ASD) and live in the North East Region. Our programs are individualized to meet specific developmental needs to enable each child to reach his or her full potential. PROGRAMS INCLUDE: n The Autism Intervention Program (AIP), also known as Intensive Behavioural Intervention (IBI) n Behavior Communication Therapy (BCT) n The School Support Program (SSP) n Enhanced Respite Program 65 FAMILIES RECEIVED SUPPORT THROUGH BEHAVIOUR COMMUNICATION THERAPY KEY PERFORMANCE INDICATORS MILLION TREND INNOVATION 66 CHILDREN children helped through the School Support Program; 66 of those children within the Connections for Students Program. The Ministry of Child and Youth Services recently made a major announcement that will impact service delivery of Autism Services across the province with over 333 million dollars invested. The focus will be on reducing wait times for services; AIP services for children aged 2 to 4 years; expanding supports in the community; and, offering a more integrated delivery model for families. 116 parent training hours provided by our Autism Intervention Program, which surpassed our target two-fold as a result of a change in format and more flexible access. Initiated a new Autism and Technology Research Project, which allows us to integrate ipads for data recording and collection. Our goal is to increase efficiency, accuracy and effectiveness in the Autism Intervention Program, while reducing costs. SERVED IN THE AUTISM INTERVENTION PROGRAM

PARENT, DEVELOPMENTAL SUPPORT DEVELOPMENTAL Hands offers programs and services to support individuals with a developmental disability and their families. Developmental Services assist those in the community who have a developmental disability or dual diagnosis to access the services, treatment and resources they need to live a fuller life. DEVELOPMENTAL INCLUDE: n Developmental Services Ontario-North East Region n Passport Program in North East Ontario n Developmental Support Services in Muskoka-Parry Sound n North Community Network of Specialized Care, which covers the entire Northern Region of Ontario KEY PERFORMANCE INDICATORS 235 TREND 315 TREND 351 INNOVATION adults served through the Developmental Support Services, which provides direct services to clients. As a result of change in the Developmental Support Services program framework, the number of clients served has quadrupled over the last two years. individuals served by the North Community Network of Specialized Care (North Network) as the program saw an increase in agencies utilizing their services. The number of individuals served through the North Network has tripled over the last two years. clients served by the completion of an application package through Developmental Services Ontario- North East Region, which helps adults connect with services and supports in their community. Hands initiated a Skills System Analysis research project to develop evidence-informed practices to support individuals with developmental disabilities in achieving life goals.

CLIENT, COUNSELLING CHILD AND YOUTH MENTAL HEALTH Hands offers a system of mental health services to children and youth in the Districts of Nipissing and Parry Sound. Highly trained professionals provide support through Child and Youth Mental Health Core Services. CORE INCLUDE: n Counselling n Brief n Intensive Treatment (Home-Based, Day Treatment Classrooms and the Child and Youth Mental Health Treatment Centre) n Family/Caregiver Skills Building and Support n Targeted Intervention n Service Coordination n Specialized Consultation and Assessment n Crisis Services 106 HOME-BASED CLIENTS SERVED MORE THAN 800 PEOPLE PARTICIPATED IN HANDS CHILD AND YOUTH MENTAL HEALTH WEEK EVENTS KEY PERFORMANCE INDICATORS TREND INNOVATION children and youth served through Counselling/ Therapy Services. individuals received help through Brief Services (short-term counselling). children and youth served through Crisis Services. An increase in children and youth served through Brief Services has relieved the demand for long-term counselling. A parenting program is now offered online to allow some families to complete sessions in the comfort of their homes, at a time convenient for them. Planning is underway to host additional group sessions by videoconference to make our groups more accessible and frequent throughout our catchment area. INNOVATION Hands created a course using itunesu for the Bounce Back Program to better support youth at our Child and Youth Mental Health Treatment Centre. Hands has been identified as Lead Agency for child and youth mental health services in Nipissing, Parry Sound and Muskoka as part of Ontario s Moving On Mental Health strategy.

PARENT, INTENSIVE SERVICE COORDINATION COMMUNITY AND JUSTICE Hands Community and Justice Services strive to improve the lives of children, youth and adults, some with complex special needs, through strong collaboration with our service partners and communities. COMMUNITY INCLUDE: n Access and Intake n Intensive Service Coordination n Tele-Mental Health n Youth-In-Transition JUSTICE INCLUDE: n Assertive Community Response Team n Direct Accountability Program n Intensive Support and Supervision Program n Supervised Access Program n Youth Justice Committee n Youth Mental Health Court Worker 87 % OF IN THE PAST 6 MONTHS, MORE THAN KEY PERFORMANCE INDICATORS SITUATIONS TREND INNOVATION families served through our Supervised Access Program, which provides a safe, supervised environment for child visits and exchanges in Sundridge and Parry Sound. Hands was the lead/assisting agency in 67 situations at the Gateway Hub, which facilitates an integrated approach to community safety in North Bay. Outreach efforts in Tele-Mental Health Coordination have resulted in 65 different organizations/agencies making referrals, a number that continues to grow. The Assertive Community Response Team is a federally funded pilot program now in its 3rd year of operation. The program includes formal evaluation in partnership with Nipissing University. 144 INDIVIDUALS CLINICAL INTAKES WERE COMPLETED WITHIN 2 WEEKS AFTER CONTACT WAS MADE WITH THE AGENCY. SUPPORTED THROUGH HANDS JUSTICE IN PARRY SOUND-MUSKOKA
9 16 / EARLY INTERVENTION EARLY INTERVENTION / 17 WE HAVE BEEN EXTREMELY PLEASED WITH THE WE HAVE RECEIVED. OUR WORKER HAS BEEN WONDERFUL WORKING WITH OUR DAUGHTER. PARENT, INFANT AND CHILD DEVELOPMENT 265 CHILDREN SERVED THROUGH EARLY LITERACY PROGRAMS EARLY INTERVENTION Early Intervention Services promote maternal and infant health, early identification and healthy development to give children the best start in life. There is also an emphasis on increasing parent knowledge of community resources. At Hands, we help guide children across the Muskoka and Parry Sound Districts during this critical developmental stage in their life, with specialized programs that support infants, children and parents or guardians. INCLUDE: n Infant and Child Development Services n Great Beginnings n Ontario Early Years System n Early Literacy Programming KEY PERFORMANCE INDICATORS: 1, INNOVATION: children aged 0-6 years served in Early Years Centres across Muskoka-Parry Sound. parents attended our Great Beginnings programs for a total of 1, 922 visits. children served through Infant and Child Development Services, with 57 of them being new clients. The Ministry of Education announced upcoming changes to the Ontario Early Years System that includes a shift of coordination from Hands to the Municipal system in the Muskoka and Parry Sound Districts. We continue to work with our partners in this sector to ensure a smooth transition while enhancing individual and family development. Our Early Intervention Programs have partnered with Infant Mental Health (IMH) at the Hospital for Sick Children to raise awareness of infant mental health. The Muskoka-Parry Sound IMH Committee, comprised of ten community agencies, is collaborating to enhance coordinated interventions for at-risk children (0-6 years) and their families. 2,567 PARENTS AND CAREGIVERS PARTICIPATED IN EARLY INTERVENTION WORKSHOPS
10 WHAT IS THE NORTH EAST ONTARIO FAMILY HELP AND TREATMENT CENTRE? HOW CAN I HELP? Please support us in helping to make this new Centre a reality. Together, we can help even more children and youth your children, your family, your neighbours and friends to face their social, emotional, behavioural and mental health challenges. We ask you to make a donation in support of Hands Family Matters Campaign, a capital campaign to raise $2 million in support of family mental health and this $7.7 million project. FUNDING FOR THE CENTRE WILL INCLUDE: $1 million to go! Towards our goal of $2 million $1 million already raised For the new North East Ontario Family Help and Treatment Centre to become a reality, we need your support and the support of our community. $ $2 million financing 100% OF FUNDS RAISED WILL GO DIRECTLY TOWARDS THIS PROJECT AND REMAIN IN OUR COMMUNITY $ Other sources $0.5 million Families are increasingly seeking assistance as they work to ensure that their loved-ones receive the help and support they need to face their social, emotional, behavioural, developmental and mental health challenges. In , Hands provided over 12,000 services to children, youth, families and adults with developmental disabilities in our communities, and we expect to provide even more services in A larger, accessible space is required to meet the growing demand for assistance. To be able to provide these families with the supports they need, we are building the North East Ontario Family Help and Treatment Centre in North Bay. The Centre will be a clinical facility that will provide the team at Hands TheFamilyHelpNetwork.ca with an accessible and compassionate treatment environment for children, youth, developmentallychallenged adults and their families. At 18,000 ft 2, the Centre will feature space to support the growing need and demand for mental health services and will: n Serve as the base for over 35 dedicated clinical professionals; n Operate as a hub to address needs across Northeastern Ontario, using technology to improve connectivity, resulting in better and quicker access to services; n Be fully-integrated with the Ontario Telemedicine Network (OTN) to allow accessibility for clients and families in remote locations; and, n Be a training centre for clients, families and other partner organizations. IN , HANDS HELPED MORE THAN 12,000 INDIVIDUALS AND FAMILIES IN OUR COMMUNITIES The facility will be conveniently located in downtown North Bay, on property already owned by Hands. The Centre will feature ample parking and will also be on a bus route. As a signature investment in our downtown core, this new Centre will help us to ensure that our region continues to attract outstanding health care professionals.
