Vadakin, P.A., Moderator of the Curia/Vicar General, 3424 Wilshire Boulevard, Los Angeles, CA , by May 30, A copy of this report should be kept on file in the parish office. If you have any questions, please call the Archdiocesan Office at (213) Who is the Chairperson of your Safeguard the Children Parish Committee? The committee membership roster should be on file. NAME OF CHAIRPERSON: MANDATED MINUTES: At least two sets of Safeguard the Children Parish Committee Meetings must be included with this report. 2. Does the parish publicize to its parishioners the existence of the Archdiocesan policies and procedures relating to the Charter? Are copies of the 2011 Working Together to Prevent Child Sexual Abuse brochures permanently displayed in the vestibule of the church?: If no explain: Did you distribute the 2011 Working Together to Prevent Child Sexual Abuse brochures at your parish during Child Abuse Prevention Month in April?: How often do Did You Know? sexual abuse prevention bulletin announcements appear in the parish bulletin? Does the Parish/School/Religious Education/Youth Ministry provide safe environment training for each of the various groups set forth in Article 12 of the Charter *? * Article 12. Dioceses/eparchies are to maintain safe environment programs which the diocesan/eparchial bishop deems to be in accord with Catholic moral principals. They are to be conducted cooperatively with parents, civil authorities, educators, and community organizations to provide education and training for children, youth, parents, ministers, educators, volunteers, and others about ways to make and maintain a safe environment for children and young people. 3
4 ADULT TRAINNING Please verify all adults, including clergy, paid staff and volunteers who work or volunteer with or around children have taken the VIRTUS Protecting God s Children / Keeping the Promise Alive Adult Awareness Session. Where are the VIRTUS certificates of proof of attendance kept for the: Parish: CHILDREN S TRAINING What Archdiocesan-approved program is being used for your children and youth? (e.g., Good-Touch/Bad-Touch, VIRTUS Teaching Touching Safety, Archdiocese of Los Angeles Safe Environment Program): a. How many children/youth did you have this year in / b. How many children/young people were trained? / c. Have the above children s training(s) been reported on VIRTUS Online? If No, provide explanation: The Auditors will ask to review documentation that the adult and children s trainings have been provided, as well as review curricula and materials designed for use in the programs. 7. What is the Parish/School/Office of Religious Education/Youth Ministry policy for those parents who choose not to have their child participate in the Archdiocesan safe environment children s training? a. List number of Opt-Outs and where the signed Opt-Out Forms are filed? b. If parents refuse to sign any form, has a record been maintained by the parish? Where is the documentation filed? 4
5 c. Did your Parish/School offer safe environment training materials have been offered to parents who opted out? What materials were offered?: The Auditors will ask to review the documentation regarding non-participation in safe environment training and ask to see any Opt-Out forms signed by the parents. 8. Verify the Pastor/Principal ensures that Fingerprinting through the Archdiocese of Los Angeles is conducted on all Archdiocesan paid personnel and volunteers to include priests and deacons as well as Parish/School/Office of Religious Education or other paid personnel and volunteers whose duties include ongoing, unsupervised contact with minors? Has the Pastor/Principal verified that the Fingerprinting information documented is on VPIN (Volunteer and Personnel Information Networks) for the Archdiocese of Los Angeles? If No, provide explanation: As verification of fingerprinting the auditors may ask for a VPIN printout for the staff and volunteers at your parish/ school site. 9. Does the Pastor ensure that visiting or non-permanent clergy (e.g., weekend assistants, priests who witness marriages, perform Baptisms, conduct funerals or otherwise engage in visiting ministry) are clergy in good standing? If No, provide explanation. 10. The following questions are for those parishes directly affected by allegations of sexual abuse of children by clergy which have been brought forward since September 2005 the current audit period (these do not include cases of pre-2005 misconduct being investigated in the clergy abuse litigation): a. Has the Archdiocese kept the parish informed? b. Has the Archdiocese provided outreach to affected persons or groups? c. Has the Archdiocese supported reinstatement of the accused when allegations are determined to be unfounded? If Yes, describe. If No, provide explanation. 5
