D epartm ent of O rthopaedics, Southw est Hospital, the Third M ilitary M edical U niversity, Chongqing , China Abstract O bjective To investigate the clinical effect of anterior decomp ression and bone graft with titanic p late fixation on thoracolumbar fractures with sp inal cord injuries. M ethods Sixty2eight cases of thoracolumbar fractures with sp inal cord injuries were treated by anterior decomp ression and structural bone graft with titanic p late fixation from M arch 1999 to May There were 45 males and 23 females aged from 14 to 61 years with an average of years. The fractures were located at T 11 in 1 case, T 12 in 20, L 1 in 33, L 2 in 12, L 1, 2 in 1 and L 2, 3 in 1. The states of p reoperative neurological function were comp lete parap legia in 10 cases and incomp lete parap legia in 58. The fixators were titanic Z2PLATE in 43 cases, PROF ILE in 19 and ALPS in 6. The materials for bone graft were auto2ilium in 45 cases and titanic mesh filled with auto2bone fragments in 23. The recovery of neurological function was evaluated according to Frankel grades and ASIA scores. The correction on the angle of sp inal deform ity( Cobb angle ), the area of decomp ression, the position of bone mass or titanic mesh and bone fusion were all exam ined by images. Results A ll of the 68 cases had successful operations. Sixty2five cases were followed up for months( mean months ). The postoperative neurological function was recovered partly or comp letely in 61 cases and imp roved by Frankel grades on average. The postoperative ASIA scores in all cases rised evidently. The CT scans and MR I scans showed that the comp ression of sp ine cord was comp letely relieved. Both ilium and titanic mesh did not loose and shift. Three sorts of titanic p late did not loose, shift and rup ture. The sp inal deform ities were well corrected without correction loss and imp lant failure at follow2up. The sp inal fusion was excellent in all follow2up cases. Conclusion Anterior decomp res2 sion surgery can make the comp ression of sp inal cord to be comp letely and safely relieved for thoracolumbar fractures with sp i2 nal cord injuries, and be used for the recovery of postoperative neurological function. The titanic p late fixation and structural bone graft can recover the stability of injuried sp ine with excellent bony fusion.
