1 Needs Assessment Implementation Plan Advocate Good Samaritan Hospital (): Obesity Reduction Date Endorsed by Governing Council: March 27, 2014 PRIORITY AREA: Target Population: GOAL: Childhood Obesity Children ages 8 to 14 with a BMI in the 85 percentile or above in the Advocate Good Samaritan Hospital primary and secondary service areas. Reduce childhood obesity in identified target population of overweight and obese children Healthy People 2020 Alignment: Nutrition and Weight Status NWS-10: Reduce the proportion of children and adolescents who are considered obese. NWS-14: Increase the contribution of fruits to the diets of the population aged 2 years and older. NWS-17: Reduce consumption of calories from solid fats and added sugars in the population aged 2 years and older. PERFORMANCE MEASURES How We Will Know We are Making a Difference Short Term Indicators Source Frequency Wellness Increase muscle & decrease body fat and BMI. 80% of the participants attending Day 1 will complete 8-week session. 50% of the participants completing survey will recognize an improvement in their commitment to fitness at the conclusion of the session/year. Pre and Post Survey results ProActive Kids (PAK) End of Session summary PAK End of Year report the session Nutrition Create healthier habits in food selection, cooking and eating, nutrition knowledge. 50% of the participants completing survey will have a positive attitude change toward their diet and nutrition at the conclusion of the session/year. Lifestyle Build confidence, increase communication and teach coping skills. 50% of the participants completing survey will recognize improvement socially at the conclusion of the session/year. 2 CHNA Advocate Good Samaritan Hospital: Obesity Reduction OBJECTIVE #1: Increase commitment to fitness among program participants by implementing three ProActive Kids (PAK) Plan programs each year from for a total of 9 sessions. Evidence Base: PAK (ProActive Kids) Plan is a comprehensive, all inclusive, 8-week program to treat the whole child built around the stability and commitment of family. Sessions address nutrition, fitness and lifestyle. 1. Secure annual funding in support of the PAK program in budget process. 9/1/2013 Allocated funds in support of Community Health & Wellness Center, VP of Ancillary Services &, PAK funding 2. Secure 2014 contract with ProActive Kids organization. 1/5/2014 Contract approved by Corporate Legal & Wellness Center, PAK Executive Director Binding contract 3. Secure 2014 location for three 8-week sessions. 1/5/2014 Advocate Good Samaritan Hospital Health Center (HWC) Education Rooms Center, Coordinator of Wellness Center, Host site 4. Create mechanism for program promotion and referrals to secure participants for PAK program. 1/5/2014 PAK flyer PAK web site Physician Relations Kids Korner Social Media Advocate web sites Physician Offices & Wellness Center, Public Affairs & Marketing Department, PAK Executive Director 20 participants enrolled in each of three 8-week sessions 5. At beginning of each session, conduct participant/parent 6. At conclusion of each session, conduct participant/ parent 3/7/2014 and (Week 8 each view of diet and nutrition at conclusion of PAK session OBJECTIVE #3: Build confidence, increase communication & teach coping skills among program participants by implementing three ProActive Kids (PAK) Plan programs each year from for a total of 9 sessions. Through PAK curriculum, teach weekly lessons to participants to facilitate enhanced self-confidence, improved communication skills, and valuable coping skills. 1/13/2014 and Curriculum designed by Meier Clinics PAK Lifestyle Coach Participants will have greater levels of self-confidence, improved communication skills, and valuable coping skills 3. At conclusion of each session, conduct participant/ parent 3/7/2017 and (8 week each of self-confidence at conclusion of PAK session 4. Summary of session data for GSAM PAK site. 4/21/2014 (45 days after the last class of each PAK survey PAK Executive Director Session data to establish PAK program impact 5. Summary of annual data for GSAM PAK site. 12/31/2014 and Annual results and report PAK Executive Director Annual data to establish PAK program impact SUSTAINABILITY PLAN Reducing childhood obesity is part of Advocate Good Samaritan Hospital s commitment to the community. Childhood obesity was identified as a need through our comprehensive community health needs assessment. To sustain the ProActive Kids program, will continue its commitment by providing both physical and financial resources as outlined in Action Steps 1-4, under OBJECTIVE #1. Data which demonstrates continued improved health outcomes, engaged participants, and quality indicators will continue to build support for funding and staffing for this program. 4

Reduce falls among seniors 65 and older in the Advocate Good Samaritan Hospital primary service area. Healthy People 2020 Alignment: Injury and Violence Prevention IVP-23: Prevent an increase in fall-related deaths. IVP-23.2: Prevent an increase in fall-related deaths among adults aged 65 years and older. PERFORMANCE MEASURES How We Will Know We are Making a Difference Short Term Indicators Source Frequency Seniors learn to view falls and fear of falling as controllable. 65% of participants attending Day 1 will complete a minimum of 5 of the 8 Matter of Balance (MOB) classes (10 of 16 hours). 50% of the participants completing the last session survey will feel more comfortable talking with others about his/her fear of falling. Matter of Balance (MOB) End of Session survey data MOB End of Year report the Session Seniors set realistic goals to increase activity. 50% of the participants completing the last session survey will agree that he/she plans to continue exercising. 6 CHNA Advocate Good Samaritan Hospital: Matter of Balance OBJECTIVE #1: Seniors participating in Matter of Balance program learn to view falls and fear of falling as controllable. Evidence Base: Matter of Balance. A Matter of Balance is an evidence based program emphasizing practical strategies to reduce fear of falling and increase activity levels. Participants learn to view falls and fear of falling as controllable, set realistic goals to increase activity, change their environment to reduce fall risk factors, and exercise to increase strength and balance. Fall Prevention was identified as a need through our comprehensive community health needs assessment. To sustain the Matter of Balance program, will need to continue its commitment by providing both physical and financial resources as outlined in Action Steps 1, 4, 5 and 7 under OBJECTIVE #1. Data which demonstrates continued improved health outcomes, engaged participants, and quality indicators will help to support funding and staffing for this program.
