2 The purpose of department Cardiology: The purpose of teaching is studying cardiovascular diseases (etiopatogenetic incidence, clinical features, diagnosis and treatment, prophylaxis, differential, prognosis) based on contemporary scientific achievements in cardiology, as well as in other areas (biology, genetics, physics, chemistry, immunology, biochemistry, Physiology, patomorfologie, pharmacology, internal medicine and other sciences); using different methods of investigation (teaching, clinical, laboratory, instrumental, functional, morphological, biochemical, hormonal, radioimunological etc. ). Solving for this purpose shall be carried out on the basis of the fundamental positions in cardiology, such as: -The unit of structure and function of the cardiovascular system; -Clinico-functional principle of appreciation of the functional and morphological disorders in the cardiovascular system, what are the methodological basis of the article and provides contact and relations with other new fundamental and clinical disciplines (anatomy, physiology and pathophysiology, biochemistry, pathology, pharmacology, internal medicine, neurology, dermatovenerology, surgery, obstetrics and gynecology, etc. ). The aim of discipline has several aspects: Studying etiology, pathogenesis, clinical manifestations typical (and only atypical variants) of cardiovascular disease, principles of treatment and prevention of these diseases; Enriching and deepening basic knowledge (obtained in previous studies) and their implementation in clinical practice; Strengthening and completion of the practical skills (obtained in the course of semiology) and carrying out clinical examination of patients; Learning and development of the methods of clinical reasoning: evaluation of the results of the clinical examination of the patient, presumptive diagnosis in cardiology, and argumentation of the laboratory investigations and consultations of other doctors-specialists, making the differential diagnosis within the limits of studied, positive diagnosis (clinic) developing a concrete and appropriate treatment, the prognosis of pathology at the patient by questions; Strengthening, implementation and enrichment in clinical practice of the knowledges in the field of ethics and medical ethics. The objectives of the training in Cardiology discipline: The level of knowledges and understanding Student must know the theoretical bases of the new (within the limits of nosologic entities studied in the IV year after the analytical program): definition of the disease; incidence and epidemiology; etiology; risk factors; pathogenesis; clinical manifestations and methods of investigation: symptoms, signs, clinical syndromes (in the order required by the examination of the patient), the data of laboratory and instrumental examination (noninvasive and invasive),
3 mechanism (pathogenesis) of symptoms, syndromes and change found at the laboratory and instrumental examination; the peculiarities of the clinical and paraclinical examination of the patient with pathology; the classification proposed by OMS experts, other classifications; criteria for classification, peculiarities of clinical manifestations for every form of/stage/degree/classification; clinical variants, atypical forms and their characterization; peculiarities of the pathology in question to elderly persons, ethyl, a narcotism, compromised immunity; the evolution of the disease; complications and their manifestations, mechanisms and the circumstances of their occurrence; any emergency status: clinic, the causes and mechanism of installation; -positive diagnosis: diagnostic criteria, diagnostic algorithm; differential diagnosis list and criteria of differentiation; the formulation of requirements; -treatment: indications for hospitalization, physical arrangements, food and diet, treatment of the etiological and treatment tactics in case of unidentified etiology, treatment, symptomatic pathogenetic (medicines, dosage, method of administration, mechanism of action, adverse effects and their prevention, contraindications, the duration of treatment); traditional methods of treatment (general principles), physical methods (principles of action, indications, and contraindications); the algorithm of treatment in emergency states. In chronic diseases: treatment in aggravation phase, maintenance treatment in remission period. Surgical treatment (general principles, indications, contraindications). Balneo-sanatorial treatment (balneosanatorial methods, natural curative factors, indications, contraindications); -Recovery (stages, methods); Prognosis for life; Primary and secondary disease prevention (chronic diseases-acutisation and/or complications). Application level The student must independently carry out: interviewing the patients, anamnesis collecting with data and assessment to demonstrate empathy towards patients, compliance with the principles of medical ethics and deontology; -clinical examination of patients with findings in the context of clinical reasoning of all signs and syndromes; argumentation of presumptive diagnosis; preparing and motivation of laboratory investigations and consultations of other doctorsspecialists; -clinical assessment of the results of laboratory and instrumental investigations to examine patients; differential diagnosis within the studied diseases; the formulation of diagnosis (clinical) specifically; argumentation of appropriate prescribing of drugs from the main groups; forecasting the evolution of pathology-established patients; preparation and drafting of the observation of the patients with the diseases studied.
4 At the level of integration The student must demonstrate the ability to integrate into the process of clinical reasoning and knowledge acquired in the corresponding specialization of previous studies (I-III) and appropriated necessity of internal medicine, to diagnose illnesses and prescribe the treatment studied before; -skills in educating patients on health care in prevention of diseases, recurrences and complications; -ability to develop and implement research projects in the new field. Conditions and prior requirements: Cardiology is one of the disciplines in university physicians training that doesn't depends on the specialty that a student will choose later, that includes large group of integration and implementation of fundamental knowledges (anatomy, human physiology, pathophysiology, etc. in clinical practice. Within this discipline, the study of the etiology, pathogenesis, clinical manifestations, evolution, treatment and prevention of cardiovascular disease more common, practical skills specialist accumulates investigating patient and assessing the results obtained, based on clinical reasoning, which ensures a correct diagnosis and proper treatment. Practical skills. 19. Examination (testation, oral examination) 5 Total 100 hours Teaching and learning methods used: Cardiology is taught in the classical manner: with lectures and practical work. The lectures will be read during the theoretical course holders. The students will study practical subjects, patients methods in diagnosis and treatment on the topics in questions. The department reserves the right to place some practical work in an interactive manner. Suggestions for individual task: From the pedagogical point of view, one of the least effective methods of appropriation is passive listening, even if theiy are very efficient structured and illustrated. You can convince of this experience on your own. For example, if you want to cook a cake, how do you think, what will be even more effective, a good culinary program on television or to take the recipe and try to bake the cake in the kitchen? And it's not that the TV is not interesting or that you are not motivated enough to bake the cake. Simply for himself something it takes as many ways of processing of the material studied. Performance of a work is much more effective than reading about how to do it, but even more effective is to teach someone else to do the same. Here's why most well know discipline teachers, who teach this discipline. If you want to have success in Cardiology, you must work actively with the matherial. What does this mean:
6 1. Originally read the material, but not simply follow it with the diagonal. Make notes. Try to formulate your own main moments. Study the diagrams and pictures in the book and your notes. Reply to the tests laid down in notebooks. 2. Come to the lectures and practical work, but not to make attendance! If you do otherwise, unlikely that you meet the requirements. Conspectate the material careful. Getting information through self and ask yourself: do you agree with professor? Understand what it is about? Does it corresponds to the material taught in the book? 3. Ask questions! The teacher, each other, and your self. In the lecture room, hall, in the offices of teachers. The fact that you ask questions that try to understand and process the material taught and can only be a greatefull. We have special individual consultation hours: every saturday from 900 to Feel free to come to these consultations.. 4. Organize yourself in groups of 2-3 students to meet regularly for discussions of the material of the course and preparing for results. As a rule, in small groups, consolidating a much wider understanding and clearer than working individually. In addition, the ability to explain to colleagues the material mastered you will be very useful for the future. 5. Use your time rational. The discipline of Cardiology shall submit to a discerning necessities. Many disciplines taught in this year's study submitted to the same requirements. Therefore, you have to manage your time and find the appropriate balance "gold" of the effort to obtain knowledge, other responsibilities and personal life. In accordance with the requirements in force for each hour of work in direct contact with the teacher, the student must work individually 1-2 hours. In other words, for the appropriation of sufficient discipline cardiology to work individually at least 5-10 hours a week. Assessment methods: Current rating: assessment of current knowledge of students is carried out daily in the practical lessons through the assessment of theoretical knowledge (oral), by taking practical skills examination of the patient and the argumentation of the diagnosis; presentation of observation of patients, by controlling the teaching of clinical observation sheets, by solving problems-clinical situations; the results after the basic themes (oral, writing or testing). The computerized tests to each theme consisting of variants from 20 questions each (simple compliment and multiple compliment), checked up in the Ergos Tester. The student has 20 minutes to respond to the test. Assessment shall be carried out according to the criteria of consistency ("Test version" SMPhU "Nicolae Testemitanu"). Final assessment: final assessment of knowledge of students is carried out on the basis of average annual memo and note the promotion examination (or differentiated colocvium ) after the VIII semester that support in three steps (assessment of practical skills, testing by completing 100 tests and oral exam). At the examination are not admitted students with average mark under 5, as well as students who have not recovered absences from practical work. Promotional exam in Cardiology discipline is combined (brief assessment), consisting of practical skills assessment test test(the variant "Test Editor" SMPhU "Nicolae testemitanu") and oral test. Assessment of practical skills examination of the patient and the clinical diagnosis of argumentation is done through capacity shown by the analysis of the information collected at the patient's bedside and clinical observation sheet (laboratory and instrumental). Average clinical case presentation is made up by summarising the coefficients approved in the following aspects:
7 general-appearence, ethics etc.. (0.1), medical history and exam objective (0,2), patient presentation and argumentation of the presumptive diagnosis (0,2), the investigation and their argumentation with differential and clinical diagnostics (0.2), the program of treatment with doses, writing recipes and urgency (0.2), prevention and recovery (0.1) with the sample notes from 10 to 0. Testation consists of variations of 100 tests each of all the themes of the course of Cardiology, from which 40 tests are simple compliment, and 60 tests are multiple compliment. The student has at its disposal a total of 2 astronomical hours to answer tests. Sample note with notes from 10 to 0. The oral test shall be carried out by providing each student 5 questions from cardiology program. The student must answer correctly to the questions of the ticket and to respond orally to additional questions of the teacher. The student has to have 30 min to prepare response. Sample note with notes from 10 to 0. Exam topics (tests and a list of questions on the tickets) shall be approved at the session chair and bring to the attention of students with at least one month before the session. For the exam to be considered passed, the student must obtain from each sample at least mark 5, otherwise the test will be not valid. Final note to discipline Cardiology consists of 4 components: annual average (0.3), practical skills (0,2), sample test(0,2) and an oral (0.3). Assessment of knowledge must be assessed with a 10 to 1 with no decimal places, as follows: -Mark 10 or "excellent" (equivalent to ECTS - A) will be given to the appropriation of % of the material; -Mark 9 or "very good" (B) will be given to the appropriation of 80-90% of the material; -Mark 8 or "good" (C) will be given to the appropriation of 75-80% of the material; -Marks 6 and 7, or "satisfactory" (D) will be provided for the appropriation of the 61 65% and 66 70% of the material; -Mark 5 or "weak" (E) will be given to the appropriation of 51 60% of the material; -Marks 3 and 4 (FX equivalent) will be provided for the appropriation of 31-40% and 40-50% of the material; -Marks 1 and 2, or "unsatisfactory" (F) will be provided for the appropriation of 0 30% of the material. Students that were not presented to the examination without any reason will be recorded as "absent" and will be appreciated with mark 0 (zero). The student is entitled to 2 repeated support of reexamination. Teaching language: Romanian, Russian, English, French.
