12 County Health Rankings Action Plan KEY ISSUE TO ADDRESS: HEALTH BEHAVIORS Health behaviors account for approximately 50% of an individual s health, and this should be the starting point for the GFHC to become a catalyst to improve health outcomes by increasing action on health and health behavior. DECEMBER, 2010 A Recommended (evidencebased) Action Plan & Business Care was finalized and approved by GFHC Board of Directors 12

16 THE COMMIT TO FIT! CAMPAIGN Commit to Fit! is a county-wide campaign focused on increasing the practice of healthy behaviors (especially among those with sedentary lifestyles) by mobilizing various organizations throughout the community to support and utilize a common message and strategy through their established communication channels aimed at improving health. COMMIT TO FIT! RESOURCES Commit to Fit! provides a significant amount of resources and tools available to all Genesee County residents and employers/organizations to make it easy to share the Commit to Fit! message with your employees, members, patients, students, and other key audiences.

17 PHYSICIAN COMMUNITY IS CRITICAL HOW TO ENGAGE PATIENTS TODAY TO HELP YOU REACH YOUR GOALS, WHILE IMPROVING THE COMMUNITY S HEALTH Patient Engagement Tools Assist with counseling Assist with motivation Assist with starting Health Behavior Tracking via Write a prescription for physical activity Share their progress at future appointments Educational Material Distribution and Display Referral to Commit to Fit! Track activity. Participate in challenges. Set goals. Achieve goals. Win incentives. Measure and reward participation within your specific group of employees, members, patients, students, customers, and other key audiences. 23
