When a child starts eating solid foods, the flavours of many vegetables seem strong to them. But if they've been breastfed, this can be less of a shock to the tastebuds. Every baby's sensory experience is unique, but the flavour of their first food, beginning in utero, is dependent on what mum is eating, said Julie Mennella, lead study author and a biopsychologist at the Monell Chemical Senses Cen ter in Philadelphia. The term BRIC'' was coined by Goldman Sachs Group's chief economist Jim O'Neill in 2001 when he identified Brazil, Russia, India and China as the top emerging markets where investors could put their money. A report published later in 2003 by the finance company stated that if things went in the right direction, this group would in the future be the largest financial bloc beating the dominance of western economies. The first BRIC summit took place in 2006 on the margins of the general debate of the 61st session of the UN General AsLEAR sembly. The first indewi pendent summit took THE T place in 2009 in Russia and it has become an annual affair since then. In 2010, when Chi na, while holding the chair, invited South Africa to join the group, it became BRICS. Within a decade of the coining of the word, the rosy picture started crumbling and in a 2015 interview, O' Neill said that he would no longer group the BRIC countries together. Is BRICS a grouping like G7?< The G7 was formed after 1973 oil crisis. In 1975, six industri alised economies France, West Germany, Italy, Japan, UK, and US -formed a forum to dis cuss economic issues. The group was joined by NG Canada and became the H G7, later called the G8 af ES ter the inclusion of Rus sia for a brief period. The Eu ropean Union is also repre sented at the G7 by president of European Commission and by leader of the country that holds EU presidency. EU, however, does not take part in political discussions. While BRICS is a group of emerging economies, the G7 is seen as a club of rich nations. What is the G20? The G20 is a forum of govern ments and central bank governors of industrialised as well as developing economies that gather to discuss key issues of the global economy. Along with G7 and BRICS members, it also has Argentina, Australia, Indonesia, South Korea, Mexico, Saudi Arabia and Turkey along with the EU as its members. The group was formed after the 1997 Asian financial crisis that threatened a widespread financial meltdown and it was felt that a forum where G7 nations could discuss the status of the global economy with emerging countries was needed. The group had its first meeting in 1999.

3 How big are these groups? Macroeconomic data shows the G7 indeed is a rich man's club as the group accounts for 10% of the world's population but 46% of global GDP. Naturally, its per capita income of $45,453 is the highest among the three groups. BRICS, on the other hand, represents 42% of the world's population and accounts for 22% of GDP. The G20 is biggest of the three in terms of population and GDP but lies between BRICS and G7 in terms of per capita income. (Representational image) Bhubaneswar: The swine flu death toll in Odisha has risen to 35 with two more persons succumbing to H1N1 virus on Sunday, according to official sources. On Sunday, four more people tested positive to the deadly virus out of 17 swab samples tested at the Regional Medical and Research Centre, Bhubaneshwar. The fresh deaths were reported from Sriram Chandra Bhanja Medical College and Hospital, Cuttack and Kalinga Institute of Medical Sciences, Bhubaneswar. So far, the total number of positive cases in the state increased to 345 out of 1,024 swab samples, a health department statement said. Health department director K.C. Dash said, A special isolation ward has been created in Capital Hospital, Bhubaneswar where doctors and paramedics are deployed round-the-clock

4 to attend to the patients. We are also taking preventive steps so that the virus doesn t spread to new areas. Earlier, the state government had issued guidelines to both government and private hospitals for treatment of swine flu patients. The Blue Whale challenge is an online game that goads teens to commit suicide. Over the past month, a dozen teen deaths have been linked to the game in India. There s need to shift discourse from the trigger, to address the underlying mental health vulnerabilities that are getting these children manipulated by an administrator who slowly works on strengthening dormant suicide ideation in someone with depression, said Dr Rahul Shidhaye, associate professor, mental health, Public Health Foundation of India (PHFI). The Blue Whale challenge is not the only indication of depression in teens and young adults. The triggers vary but the over-the-top reactions indicate underlying depression that drives them to extremes both withdrawing into a self-destructive shell and losing control over trivial issues. Last week, Haridwar Police rescued a girl, now 18, who locked herself up in her room and lived in voluntary solitary confinement for three years because her parents nagged her to

5 study. Her 15-year-old brother followed her example in 2016 and locked himself in another room because he didn t want to go to school either. A similar example of over-the-top reactions have been reported from across India. A teenager shot his father dead in Allahabad for asking him to study, two teens stabbed a classmate to death in Delhi because he refused to drink with them, and a student slapped a teacher in Vadodara when asked to apologise for spilling coffee over another teacher. Angry and afraid Online interactions are far more superficial than interpersonal bonding, say experts, which make people lonely, depressed and often unable to vent about worries and fears. The result is loneliness, depression and behavioural problems, which are increasingly diagnosed after the damage is done.

6 Spending time with family and friends provides emotional and social support, but with virtual interactions replacing interpersonal friendships, coping skills get compromised and teens find themselves isolated in both the real and virtual world and find it harder to express their need for emotional support, said Dr Samir Parikh, director, department of mental health and behavioural sciences, Fortis Healthcare. Restricting interaction to social media feeds isolation in the urban milieu. While social networking does not directly lead to isolation or depression, it helps mask it. Sharing is superficial on social media, which is mostly used as a tool to vent. It lacks the empathy of real-life conversations, he said. Close to 10 million teens, aged 13-17, have mental health problems that need active intervention but diagnosis and treatment remains elusive. Oddly enough, the parents of the Haridwar siblings did not consider seeking psychiatric treatment. Most people identify sadness and crying with depression and do not think irritability, anger, change in behaviour, falling grades, sleeplessness and withdrawal from friends and family are also signs of depression, said Dr Parikh. Teen deaths in India: Top causes While deaths of youth in India have increased in the past decade, countries like China and Sri Lanka have achieved just the opposite Driven to death Even those who recognise symptoms don t know where or who to visit. With only 5,615 psychiatrists almost all of them in metros and state or district capitals registered with the Indian Psychiatric Society, there s a lack of professionals to diagnose and treat. The few psychiatrists we have are busy treating severe mental illnesses and very few step out to treat illnesses in the community, so early signs of sub-clinical depression are often missed, said Dr Shidhaye. Under-diagnosis has made suicide the biggest cause of death of 15- to 24-year-olds in India, followed by road traffic accidents, shows data from Global Burden of Disease Study that tracked death from 306 diseases, injuries and risk factors across 188 countries. Globally, road injury is the biggest killer in this age group. Deaths in youth (in India) have been rising for the past decade while other countries such as China and Sri Lanka have achieved the opposite. As a priority, the Centre must launch a national programme with active youth participation to address these causes of death and illness, said Dr Vikram Patel, professor of global health and social medicine at the department of global health and social medicine, Harvard Medical School.

7 Teenagers are more reckless than children and adults as their brains are wired to react rather than retreat from danger. This proclivity for risk due to changes in the brain during teenage and early 20s explains why more young people die in suicides and accidents than people in other age groups. In a run-up to the World Health Day, PM Modi also underlined the need to bring depression out of the closet in his Mann Ki Baat on March 26. We (in India) are afraid to talk about it (depression) openly, he said. Suppression of depression is not good. Expression is good. If depressed, share your feelings with others, it will make you feel better. It s not that people are not seeking treatment antidepressant sales are up by more than 30% over the past four years from Rs 760 crore in 2013 to Rs 1,093 crore in 2016 but those getting help is just the tip of the proverbial iceberg. Psychological first aid from friends and family can help youngsters be more resilient. In the absence of specialists, drawing support from the existing evidence-based cultural resources and social support mechanisms yoga, mindfulness/vipassna helps lower distress, said Dr Shidhaye. We can t wait for specialists or government to provide solutions, each of us has to do our bit to improve mental well-being in whatever way we can, he said. But the richest states in India have much higher infant mortality rate than many poorer states.

PTI file photo The most common causes of childhood death are the diseases that are readily preventable or treatable with proven, cost-effective and quality-delivered interventions. Infectious diseases and neonatal complications were responsible for the vast majority of deaths under five globally during the year 2015 when about 5.9 million children died below the age of five. India alone shared the burden of 1.4 million deaths (UNICEF, 2016). The majority of underfive deaths (about 45 per cent) occur within 28 days of birth, known as neonatal mortality, and 70 per cent occur within one year, i.e., in infancy. Infant mortality rate (IMR) is the probability of dying before one year of age, expressed per 1,000 live births. It is the most sensitive index of quality of life, as it reflects the state of public health, availability and accessibility of health care infrastructure, health policies and programmes, environmental sanitation, and cultural and socio-economic development of society. It is important to know that today almost all our neigbouring countries and those of the South-Asian region have lower IMR than India. Nepal has 29, Bangladesh 31, and Sri Lanka with a stellar performance of 8 per 1,000 live births is far ahead of India that has an IMR of 41. It may also be noted that 60 years ago, India and China had almost the same IMR, but at present in China it is just nine, which is less than one-fourth of India s IMR (hence the large population size cannot be an excuse for high mortality). It is not that child mortality has not decreased in India. It has shown a reduction of 53 per cent since Various interventions in terms of provision of vaccination against deadly diseases (immunisation), oral rehydration salts, intervention in child nutrition, etc., have shown desired results. But this progress is marked with large interstate variations. Within India, there are states which can equate in IMR to any country of Europe and America (Kerala with an IMR of 6 per 1,000 live birth) and also states which may be equated to subsaharan African countries (e.g. IMR and U5 mortality in Uttar Pradesh is 64 and 78; Chattisgarh 54 and 64; Madhya Pradesh 51 and 65, respectively). The vaccines for Japanese encephalitis (cause of infant deaths in Gorakhpur) were introduced under the Universal Immunisation Programme in But a 2015 survey by the Indian Council of Medical Research found that a large percentage of children in the Gorakhpur division have either not received it at all or have received only one of the two required doses. Nor have preventive measures to restrict the disease been taken, such as vector control, effective surveillance systems, early intervention, or dealing with the unsanitary conditions that contribute to the spread of the disease. One wonders how Kerala has attained these levels and why other states in India have not reached this level. Generally, economic growth and employment in gainful activities (prosperity) are considered one of the main factors by which health conditions in any society improves, as economic growth and employment provide the means of health care and nutrition within the reach of all sections of society.

9 In case of India, however, this does not seem to be true. Had this been the proposition, economic giants such as Maharashtra, Gujarat, Punjab and Haryana should have been on the top in terms of child health, but IMR in these states is quite high (ranging between 30 to 40 per 1,000). In fact, world over, many developing countries like Sri Lanka, Cuba, Costa Rica, Jamaica, and many low-income countries have achieved remarkable improvements in health conditions without much economic growth. It is the specific support of food consumption, community health, medical care, basic education and awareness that has transformed the general health conditions. In the case of Kerala and Tamil Nadu, economic growth and public support have worked independently of each other. The proportion of malnourished children in terms of stunting, wasting and being underweight in Kerala is 15 to 19 per cent against the national average of 35 to 38 per cent. Though malnutrition is not a cause of death on its own, but it magnifies the role of infectious diseases in causing mortality. Research indicates that malnutrition multiplies the number of deaths caused by infectious diseases rather than following an additive model. Studies have also revealed that the major reasons of high child mortality and more specifically of high IMR are high episodes of diarrhoea, pneumonia, low birth weight, low access to medical care and low levels of full immunisation, which are largely preventable in nature. The health and overall status of women which is associated with age at marriage, maternal literacy, ante natal care, birth spacing and utilisation of maternal and child health services translates into major risk factors in child mortality. It may be noted that these two southern states have achieved low IMR with the strength of these pillars. It also needs mention that 35 years ago, at the Alma Ata Conference in 1982, there was a paradigm shift in tackling health inequalities, and all countries joined the Primary Health Care Movement for better health to its population. Today the success story of various countries in terms of health suggests that the PHC has remained the benchmark for those countries discourse on health. The access to medicine and nutrition not only increases the survival, it also reduces the economic and social costs associated with poor health and low productivity. The perils of ignoring primary health care services (low health spending, lack of medicos and personnel, lack of infrastructural facilities) in many states of India are before us the most unfortunate are the infant deaths in Uttar Pradesh (Gorakhpur) and Jharkhand. All children possess the same right to grow up healthy, strong, well educated and capable of contributing to their societies. A fair chance of survival to every child is a matter of political commitment. It is also a matter of collective will, a matter of choice, which we can make. A choice we must make.

14 Back Pain Lower back pain may all be in the mind, study suggests (Medical News Today: ) New research suggests that persistent feelings of stiffness in the lower back may be some kind of self-protective 'trick' that the mind plays on us in order to prevent further injuries. A new study into the neuroscience of clinical pain suggests that perceptions of stiffness may not reflect the actual state of the spine and joints. The team's findings may pave the way for new therapies that help those with chronic pain in their lower back. Lower back pain is a leading cause of disability worldwide, affecting around 9.4 percent of the global population. Sometimes, people with lower back pain do not manage to get rid of the pain. In fact, it is estimated that for approximately 20 percent of those with the condition, the pain evolves into chronic pain - that is, pain that does not ease for 12 weeks or more. But might the discomfort all be a matter of perception? Could feeling pain not necessarily reflect the state that our body is in? A new study published in the journal Scientific Reports suggests so. The researchers were led by Dr. Tasha Stanton, who is a senior research fellow at the University of South Australia's School of Health Sciences in Adelaide. Studying feelings of lower back pain Dr. Stanton researches the neuroscience behind clinical pain, and her motivation for this new study stemmed from previous evidence that amputees feel pain in a limb that they no longer have, or that the subjective feeling of pain can be influenced by a range of other perceptual factors such as visual or auditory stimuli. She and her team recruited 15 people with self-reported feelings and symptoms of chronic lower back pain, and an additional 15 healthy, age-matched control participants. The team conducted three experiments. In the first one, they used an "established, customized device, validated in humans" that applies pressure to the spine and can objectively measure the resulting stiffness. 15 The researchers compared these measurements with what the participants reported to feel, using a scale from "not stiff at all" to "most stiff imaginable." In the second experiment, the participants were told that they would receive an applied force and were then asked to estimate as accurately as they could the magnitude of the force they received. Finally, the third experiment aimed to examine whether or not adding sounds to the perception of pressure would change how the stiffness is perceived. Feelings of stiffness: 'A protective response'? The researchers wanted to evaluate this last point based on the results from the first two experiments, which led them to the hypothesis that "feeling stiff may represent a protective perceptual construct." "People with chronic back pain and stiffness overestimated how much force was being applied to their backs - they were more protective of their back. How much they overestimated this force related to how stiff their backs felt - the stiffer [it] felt, the more they overestimated force. This suggests that feelings of stiffness are a protective response, likely to avoid movement." Dr. Tasha Stanton "In theory," Dr. Stanton adds, "people who feel back stiffness should have a stiffer spine than those who do not. We found this was not the case in reality. Instead, we found that that the amount they protected their back was a better predictor of how stiff their back felt." Dr. Stanton also explains the results of the third experiment, saying, "[We] found that these feelings could be modulated using different sounds. The feeling of stiffness was worse with creaky door sounds and less with gentle whooshing sounds." "This raises the possibility that we can clinically target stiffness without focusing on the joint itself but using other senses," she says, believing that the findings open up new therapeutic avenues for chronic back pain. "The brain uses information from numerous different sources including sound, touch, and vision, to create feelings such as stiffness," says Dr. Stanton. "If we can manipulate those sources of information, we then potentially have the ability to manipulate feelings of stiffness. This opens the door for new treatment possibilities, which is incredibly exciting," she concludes.

Researchers believe that there may be more to this connection than meets the eye. Researchers suggest that there may be a stronger link between ADHD and sleep problems than hitherto believed, and that the two may not be completely separate issues after all. Data reported by the Centers for Disease Control and Prevention (CDC) indicate that approximately 11 percent of children aged 4 to 17 are diagnosed with attention deficit hyperactivity disorder (ADHD) in the United States. Among the adult U.S. population, the National Institute of Mental Health report a 4.1 percent 12-month prevalence rate for the disorder. ADHD is typically characterized by hyperactivity, a short attention span, and difficulties in self-organization. Sometimes, the disorder can be accompanied by one or several other conditions, including dyslexia, anxiety, and depression. Another concern that has been flagged up in relation to ADHD is the existence of sleep problems, including sleep apnea and disturbed sleep patterns. Mostly, ADHD and sleep disturbances have been treated as separate issues, but Prof. Sandra Kooij, from the VU University Medical Centre in Amsterdam, the Netherlands, believes that the two may be fundamentally interconnected. Prof. Kooij explained for Medical News Today that, as a psychiatrist who specializes in ADHD, she has dealt with many cases in which the disorder seemed to be linked with sleep disturbances. This gave her the first impulse to look more closely into the connection. She told us, "I am a psychiatrist specialized in adult ADHD since 1995, and from the beginning, the sleep problems that most people with ADHD suffer from intrigued me. Most of them had a similar pattern of late sleep onset, and difficulty getting up in the morning, leading to fatigue during daytime and role impairment." "Sleep duration was usually short due to school or work obligations in the morning. This sleep loss seemed to increase the severity of ADHD symptoms as well," explained Prof. Kooij. The researcher presented her findings yesterday at Neuropsychopharmacology Congress, held in Paris, France. the European College of

17 Most people with ADHD have sleep problems Prof. Kooij and her colleagues have reviewed several studies that point to a link between sleep disturbances and ADHD, and they suggest that the evidence so far offers a strong basis for further evaluation. "If you review the evidence, it looks more and more like ADHD and sleeplessness are two sides of the same physiological and mental coin," explains Prof. Kooij. Prof. Kooij's investigation has revealed several interesting facts about the presence of sleep disturbances in individuals diagnosed with ADHD, and many of those have led her to form her hypothesis. She notes, in the first place, that the majority of people diagnosed with ADHD also exhibit a disturbed sleep pattern. The physiological aspects of sleep are also affected which, in turn, could lead to other, more severe health implications. "[W]e started to measure the onset of the sleep hormone melatonin in [the] saliva of people with ADHD with and without sleep onset problems. We found that late sleepers had their onset of melatonin 1.5 hours later than normal, correlating with the late sleep pattern," Prof. Kooij told MNT. "[A]lso, their movement patterns and temperature during 24 hours were delayed," she added. "The next question was which other physical processes might be delayed, and what this would mean for their health in general." 18 How do we recognize ADHD in adults? "We now aim to prevent this potential negative cascade of events for people with ADHD with lifetime sleep problems by advancing the late sleep phase using melatonin at night and/or light therapy in the morning, and measuring the effects on blood pressure, glucose levels, heart rate, and other biomarkers," Prof. Kooij told MNT. Photophobia, or oversensitivity to light, is also reported by 69 percent of adults diagnosed with ADHD. Prof. Kooij suggests that this oversensitivity leads them to wear sunglasses indiscriminately during the day, which, in turn, may increase the prevalence of sleep-related problems. She told us that she wondered whether "there is something going on in the eye that relates to ADHD and late sleep." Prof. Kooij and her team are conducting research to find out which treatment may be most helpful in the case of sleep disorders. "[W]e try to find out if a low dose of melatonin (0.5 milligrams) in the evening is as good as higher dosages (3 milligrams), and which is best: melatonin, placebo, or melatonin plus light therapy in the morning," she told MNT. She also shared with us some tips for managing sleep disturbances, advising that people concerned about their sleep patterns should "stop using the light of screens after 10 p.m.," and that they should aim to "get up at the same time every morning, and if necessary, use a strong lamp to wake up [the] brain." Where does the cause lie? Should the link between ADHD and sleep disorders, Prof. Kooij is interested in finding out which way the causal relationship lies. "If the connection is confirmed, it raises the intriguing question: does ADHD cause sleeplessness, or does sleeplessness cause ADHD? If the latter, then we may be able to treat some ADHD by non-pharmacological methods, such as changing light or sleep patterns, and prevent the negative impact of chronic sleep loss on health." Prof. Sandra Kooij She cautions that she and her colleagues do not suggest that disturbed sleep patterns are key to all ADHD diagnoses, but she still believes that the strong link requires more investigation. "We don't say that all ADHD problems are associated with these circadian patterns, but it looks increasingly likely that this is an important element," she says.
